Our clinical social workers provide you and your family emotional support, counseling services, access to community resources and help with paperwork and end-of-life planning. Talking about hospice care sooner means more quality time with those you love. Our philosophy is to guide you and your family through the changes ahead, helping you understand where you are in your journey together. Hospice caregivers monitor changes in your health, and address them with a gradual, compassionate approach. This results in a much less stressful experience for the patient and family. The sooner you or your loved one receives the special care hospice offers, the more time you can focus on what really matters. Hospice care will enable you to remain independent for as long as possible, live comfortably and ensure that your wishes are followed. For information or referrals call 912-265-4735 www.hospice.me

Our bereavement coordinator and counselors help your loved ones grow through this experience. Beginning at admission, our doctors, nurses, chaplains, social workers, bereavement coordinator and volunteers educate families on what to expect during the first year after the death of a loved one. Because each person copes with grief differently, we offer individual and family counseling, support groups, workshops, and a bereavement camp for adults and children. As a member of our Hospice family, loved ones may continue their grief support at our in-house Community Bereavement Center for up to 13 months after the death of the hospice patient. Bereavement Support: m Host grief workshops m Conduct caregiver support groups m Provide adult and children s bereavement books m Offer bereavement day camps m Distribute quarterly Grief Support and Holiday newsletters m Conduct annual Lights of Love Community Memorial Service For Grief or Bereavement Services call 912-267-5462. We promote comfort and dignity. As a person s health declines, care needs often increase. Hospice care provides you with the peace of mind that our specialists continuously assess your comfort. We are experts in skillfully and compassionately managing pain and symptoms to meet our patients goals while maintaining their mental clarity. You or your loved one s personal needs are gently tended to as well, including bathing, grooming and dressing, to preserve respect and dignity. Our goal is to help you achieve the best quality of life and to make every moment count. You matter because you are you.
