Meet Our Team Marketing Directors & Clinical Evaluators Michael Zavacky As the Director of Marketing, South for NeuroRestorative, Michael works with families, case managers, clinicians, payors, workers compensation providers and other referral sources to successfully admit participants into our post-acute brain injury programs. Michael brings nine years of brain injury rehabilitation stakeholder relationship experience to NeuroRestorative with strategic development, marketing and communications expertise. Mike Riemenschneider Mike and his team work with their referral sources and families to increase accessibility to rehabilitation for those with brain injuries. He and his team identify new and partner with existing referrers to provide advocacy for the patients they serve. Mike has been in this role for the past three years with NeuroRestorative and holds a Master s degree in Health Administration. He loves his role with NeuroRestorative because it allows him to apply his sales background and education in a way where he can make a difference in the lives of those with brain injuries. Andre Kerr Brad Hendrix Andre works with case managers, payors, workers compensation providers and other referrals sources to successfully admit participants into our post-acute brain injury programs. As a Clinical Evaluator, Andre s territories range from Northern New Jersey to the greater New York City area and its surrounding counties. Andre has over eight years of marketing and admissions experience in the healthcare industry including serving as the Admissions Director of Geriatric Admissions for a 227 bed sub-acute/skilled nursing facility. He also assisted with marketing a long-term pediatric unit specializing in serving young adults with special needs. Brad works with case managers, payors, workers compensation providers and other referral sources in Oklahoma, Southern Kansas and the North Texas regions to successfully admit participants into our postacute brain injury programs. A veteran of the healthcare industry, Brad has nearly 20 years of sales and marketing experience including spending 11 years as a Clinical Evaluator for NeuroRestorative. He has also served as a board member with the Brain Injury Association of Oklahoma and the Case Management Society of America in Oklahoma City. Brad is a proud veteran of the United States Marine Corps. Cheryl Burda As the Clinical Evaluator for NeuroRestorative Michigan, Cheryl helps provide opportunities for participants to access NeuroRestorative s programs and performs clinical evaluations to determine initial treatment goals and eligibility. Cheryl brings 27 years of brain injury management experience to NeuroRestorative with operations, project management and communications expertise. Prior to her current role with NeuroRestorative Michigan, Cheryl was Vice President of Programs and Operations at the Brain Injury Association of Michigan.

Christine Hallermann Chris s territory ranges from North Central to Southern Illinois and Central to Eastern Missouri. Chris performs clinical evaluations for potential participants to assist in determining eligibility for NeuroRestorative s programs. As a Clinical Evaluator, Chris brings over 16 years experience as a comprehensive nurse case manager for pediatric, adolescent and adult populations with complex medical issues. Chris is working towards becoming a Brain Injury Specialist. Colm Roe Colm has served as a Clinical Evaluator for NeuroRestorative s Florida Region since 2006. He obtained a degree in Physical Therapy in 1994 from Miami Dade College and is a Certified Brain Injury Specialist. He has worked primarily with neurologically-involved individuals since graduation. Colm has also been employed as an instructor in the Physical Therapy program at Seminole State College. He served a threeyear term on the board of directors for the Brain Injury Association of Florida and currently serves on the board of directors for the Central Florida Brain Injury Support Group. Deborah Gutteridge Deborah is a Clinical Evaluator with NeuroRestorative, and is currently in her 16th year of service. She possesses a Master s of Science Degree in Counseling Psychology and is a Certified Brain Injury Specialist Trainer. She has worked in the field of brain injury for the past 28 years, and currently covers the territories of Northwest Missouri, Kansas, Iowa, Nebraska, Colorado, and North and South Dakota. Donna Kuhlmann As a Clinical Evaluator for NeuroRestorative Texas, Donna s territory includes all of East Texas between Dallas and Houston. Donna provides clinical evaluations and resources to determine eligibility and treatment goals for those seeking care through NeuroRestorative s in-patient and out-patient programs. Donna is a Licensed Medical Social Worker with 20 years of experience in the healthcare industry. In addition to her role with NeuroRestorative Texas, Donna serves on the board for the Brain Injury Alliance of Texas. Furthermore, Donna s son is a brain injury survivor, giving her a unique perspective into the recovery process. Elizabeth Lauzon With 25 years of experience in the field of brain injury, Liz has worked in long-term settings, out-patient rehabilitation, and in-patient rehabilitation programs. As a Clinical Evaluator, Liz is focused on admitting individuals to NeuroRestorative Massachusetts Supported Living Program for the MassHealth Home and Community Based Waivers. Liz evaluates and transitions waiver recipients out of long-term nursing facilities and into our community-based Supported Living Homes. Gary Graham Gary s territories include Northwest and Southwest Arkansas, Northern Louisiana, Eastern Tennessee, and Northern Mississippi in his role as a Clinical Evaluator. Gary provides opportunities for participants to access NeuroRestorative s programs by performing clinical evaluations to determine treatment goals and eligibility. Gary has over 28 years of experience in the field of brain injury rehabilitation working as a facility case manager, program representative, and clinical evaluator for NeuroRestorative Timber Ridge. Gary is also a Certified Brain Injury Specialist.

Tona Kohn Tona s territories are Minnesota, Wisconsin, Northern Illinois, including Cook, Will, Kane and DuPage counties, and Northern Indiana in her role as a Clinical Evaluator. Tona helps provide opportunities for participants to access NeuroRestorative s programs and performs clinical evaluations to determine initial treatment goals and eligibility. Tona has over 20 years of case management, sales and marketing experience within the healthcare arena, including roles in acute rehabilitation facilities, hospitals and schools working with both adults and children. She is a Certified Brain Injury Specialist. Greg Walton Greg is a Clinical Evaluator for NeuroRestorative San Antonio, covering South and Central Texas. He has 35 years of experience serving individuals with brain injury and other disabilities. He is a Certified Brain Injury Specialist, an Assisted Living Manager, an Eden Educator, Vice President of the Texas Brain Injury Alliance, and a Master s level psychologist. He is a founding member, and current board member, of the Texas Culture Change Coalition. Tommy Tredway As a Clinical Evaluator for NeuroRestorative, Tommy helps provide opportunities for participants to access NeuroRestorative s programs. Tommy s territories range from Kentucky and Southern Ohio, to Eastern West Virginia. He has over 13 years of healthcare experience, and has been working in the brain injury rehabilitation field since 2004. In addition to his role with NeuroRestorative, Tommy is the Exhibitor Chair for the Northern Kentucky Traumatic Brain Injury Conference, and serves on the board of directors for Bridges, Inc. Northern Kentucky Brain Injury Support Group. Janet Keith As a Clinical Evaluator for Maine, New Hampshire, Rhode Island and Vermont, Janet is also an Occupational Therapist and Certified Case Manager with eighteen years of experience in direct service delivery, program management, case management, clinical evaluation and admissions/marketing, within the neurorehabilitation industry. She is an active advocate and educator for brain injury services and client/ family supports, demonstrated by her involvement for many years with efforts in Rhode Island, New Hampshire and Maine. She is currently the BIAA-ME conference committee chairperson. Jared Pfankcook As a Clinical Evaluator for NeuroRestorative, Jared s territories range from the central and southern parts of New Jersey to the southeastern portion of Pennsylvania including Philadelphia. Jared helps provide opportunities for participants to access NeuroRestorative programs and performs clinical evaluations to determine initial treatment goals and eligibility. Jared has over seven years of experience with NeuroRestorative and individuals with brain injury. He recently completed a Master s degree in Applied Psychology and has been working on obtaining his Board Certified Behavior Analyst certificate. Laurie Silva Laurie is the Clinical Evaluator for Virginia. She meets with families, treatment team members and candidates to determine initial treatment goals and eligibility for NeuroRestorative programs. Laurie has over 20 years of experience working in the healthcare arena, and 15 years of experience working in the field of traumatic brain injury.

Maureen Butler Maureen is a Clinical Evaluator for Massachusetts and Southern New England. She is a Master slevel Rehabilitation Counselor with over 25 years of professional experience in post-acute brain injury rehabilitation. Prior to her role in outreach, education and clinical evaluation with NeuroRestorative, Maureen worked in both facility and community-based brain injury programs. Her experience includes vocational rehabilitation, neurobehavioral services, clinical supervision and program management. Michele Foreman Michele serves as the Clinical Evaluator for Maryland and the District of Columbia. Michele has over 20 years of experience in healthcare management, holding positions in hospital operations, long-term care, case management, and for commercial insurance carriers. She has extensive experience completing indepth clinical evaluations and determining effective rehabilitation treatment plans for injured workers and others impacted by brain injury. Michele partners with workers compensation case managers, physicians, rehabilitation and trauma hospitals, military and veterans hospitals. Nancy Ritter Nancy has served as a Clinical Evaluator with NeuroRestorative for over six years working with individuals in central and southern Indiana, as well as central Ohio. In addition to her role with NeuroRestorative, Nancy serves as a representative on the Indiana Brain Injury Leadership Board, the Ohio Valley Center Advisory Council, and is a past president of the Brain Injury Association of Indiana. Nancy is a Certified Brain Injury Specialist. Rita Crawford In her role as a Clinical Evaluator for NeuroRestorative San Antonio, Rita is frequently the initial contact for families and referral sources inquiring about available services. She evaluates, educates and assists in coordination of appropriate care for brain injury survivors. Rita has over 25 years of nursing experience including acute rehabilitation and long-term acute care. Rosie Ferrett Rosie is a Clinical Evaluator for NeuroRestorative Florida s programs and is privileged to serve individuals with brain and spinal cord injury within the Southwest and Southeast Florida regions. She has been working in the medical community as a liaison and evaluator for over 11 years. In addition, Rosie attended University of Miami in Coral Gables, Florida where she earned her Bachelor s degree in Business Administration. Sara McDonald As a Clinical Evaluator for NeuroRestorative Timber Ridge, Sara networks with healthcare professionals in Central Arkansas, Northeast Arkansas and part of Southern Missouri assisting them in placement of individuals with brain injury. She also evaluates individuals who are seeking placement in our post-acute rehabilitation program. She has over 30 years of experience as a Speech-Language Pathologist in a variety of settings and has spent the last 13 years specializing in post-acute brain injury services. Sara is a Certified Brain Injury Specialist.

Sarah Bomberger As a Clinical Evaluator for NeuroRestorative, Sarah s territory is North Carolina. Sarah helps provide opportunities for participants to access NeuroRestorative s programs and performs clinical evaluations to determine initial treatment goals and eligibility. Sarah has nine years of sales and marketing experience within the healthcare industry, as well as being a Master s level Clinical Social Worker and a Certified Brain Injury Specialist. Teah Gulley As a Clinical Evaluator for NeuroRestorative, Teah s territory is the Tampa Bay area of Florida. Teah helps provide opportunities for participants to access NeuroRestorative s programs and performs clinical evaluations to determine initial treatment goals and eligibility. Teah has over five years of case management and clinical experience within the healthcare industry, including roles in rehabilitation facilities and workers compensation programs. Teah is a Certified Rehabilitation Counselor and a Certified Brain Injury Specialist. Alicia Williams Amber Balogi Alicia serves Georgia and South Carolina in her role as a Clinical Evaluator. In addition, she assists with providing opportunities for participants to access programs in other areas throughout the country. Alicia is the primary contact for initiating referrals and completing clinical evaluations to determine which programs and treatments are appropriate for participants. Alicia is a dedicated clinician with more than 20 years of experience in healthcare, including healthcare marketing/sales, rehabilitation, management long-term care and acute care. Alicia is currently pursuing a Brain Injury Specialist certification. Amber s territories include Western Pennsylvania, Northeast Ohio and Northern West Virginia in her role as a Clinical Evaluator. Amber helps provide opportunities for participants to access NeuroRestorative s programs and performs clinical evaluations to determine initial treatment goals and eligibility. Amber has over 10 years of sales and marketing experience within the healthcare arena, including roles in rehabilitation facilities and long-term care. Amber is working towards becoming a Brain Injury Specialist.
