FPPA FPPA offers active members and retirees a variety of voluntary insurance benefits. The list of available programs is constantly being reviewed in order to provide our members the best, most affordable benefits available. Optional Insurance Programs While some benefits can be purchased by simply completing an application form, others are more restrictive and require a review of current health status and approval by the insurance company. Retirees may be able to pay for coverage directly from pension checks. Please check with the provider and our office to see if this is possible. Individual Health Insurance Plans Insurance Shopping Service Finding health insurance when not Medicare-eligible can be difficult and plans offered may be expensive. To assist with this process, FPPA members can contact HUB International, a local brokerage firm, for assistance in finding an individual or family health plan. Since each company has differing policies, it is helpful to use a consultant that knows and can shop the market. A HUB International individual health representative will contact multiple insurance companies. This shopping service may be used to find many types of insurance plans including health, life, and long-term-care. Contact HUB International Insurance Services, Inc. at: (303) 893-0300 in the Denver metro area or (888) 795-0300 nationwide. Member Eligibility The Insurance Shopping Service is available to all FPPA active members, retirees and survivors. Those under age 65 or otherwise ineligible for Medicare may find it especially helpful in locating coverage. Premium Payments Generally, premiums for individual policies are paid by the insured directly to the insurance company. However, premiums for individual policies for retirees and survivors may be deducted from your FPPA pension check if the insurance company agrees to comply with the following FPPA requirements: 1. A Public Safety Officers Insurance Carrier Agreement must be completed and submitted by the insurance company. These can be found on the FPPA website, FPPAco.org. 2. After FPPA has entered into the agreement with the insurance carrier, the carrier s name will be added to the third party insurance provider list posted on our website. 3. Once the carrier is on the third party insurance provider list, download the following form: Public Safety Officer Authorization for Insurance Premium Deduction. Complete the form and submit it to FPPA. FPPA will notify you as to whether you qualify for the program and if you do, the date your premiums will start to be deducted from your FPPA pension. Please Note: Individual policies differ from group policies in that you must qualify and be approved by the insurance company. The application for coverage may include questions about you and your medical history; in addition, your medical records may be reviewed as part of the application and approval process. Health Insurance Options for Medicare Eligible Members SecureHorizons HMO Retiree (MedicareComplete HMO) This is a group plan sponsored by FPPA and is a comprehensive enhanced Medicare plan, insured by UnitedHealthCare Insurance Company. Through this plan, care is provided by SecureHorizons In-network Primary Care Physicians who arrange for specialists if needed. All medical care costs, including co-payments, are coordinated through the SecureHorizons benefits program. For more information and/or to enroll, please call the FPPA Payroll Specialist at (303) 770-3772 in the Denver metro area or (800) 332-3772 toll free nationwide. The FPPA retiree, their spouse or survivor, age 65 or older who qualify for Medicare Part A & Part B. A deduction from your pension check is required to participate in the program. Please note: In order to participate in the plan, a pension payment from FPPA must be large enough to cover the entire premium. Humana Medicare Advantage Plan The Humana Medicare Advantage Plan provides coverage for office visits, inpatient treatment, preventive care, prescription drugs, and emergency treatment worldwide. Additional services include Silver Sneakers or Silver & Fit depending on where you reside. If you have questions, or to inquire about rates, please contact Humana at (866) 396-8810 (TTY: 711). When calling Humana all you need to do is let the representative know that you are with the Fire & Police Pension Association of Colorado, or the FPPA. The FPPA retiree, their spouse or survivor, age 65 or older who qualify for Medicare Part A & Part B. A deduction from your pension check is required to participate in the program. Please note: In order to participate in the plan, a pension payment from FPPA must be large enough to cover the entire premium. AARP Medicare Supplement Plan The AARP Medicare Supplement Insurance Plans, insured by UnitedHealthCare Insurance Company, are issued on an individual basis and may require the completion of a health questionnaire to determine eligibility and/or rates. Contact AARP Health Care Options at: (800) 545-1797 to obtain enrollment information. When speaking with the AARP representative, please request to be enrolled in The Fire & Police Pension Association s Medicare Supplement Plan Group #941. The FPPA retiree, their spouse or survivor, age 65 or older who qualify for Medicare Part A & Part B. Payroll deduction of premiums is optional for this plan. FPPA members who receive a benefit large enough to cover the entire premium may elect to have it deducted by following the procedure below: Once enrolled and AARP medical cards received, contact FPPA to obtain the appropriate form required to deduct premiums from your monthly pension check. Complete the form and return it to FPPA s Payroll Specialist. Members may have to pay the first few monthly premiums directly prior to having the deduction set up on the payroll system. Prescription Coverage For Medicare Eligible Members AARP MedicareRx Plans The AARP MedicareRx Plans, insured by UnitedHealthcare Insurance Company, are Medicare prescription drug plans. Contact AARP MedicareRx Plans at: (800) 545-1797 to obtain enrollment information. When calling, it is important that you request to be enrolled an AARP MedicarRx Plan, and identify yourself as a retiree of The Fire & Police Pension Association of Colorado, Group #472. The FPPA retiree, their spouse or survivor, age 65 or older who qualify for Medicare Part A & Part B. Payroll deduction of premiums is optional for this plan. FPPA members who receive a benefit large enough to cover the entire premium may elect to have it deducted by following the procedure below: Once enrolled and AARP prescription cards received, contact FPPA to obtain the appropriate form required to deduct premiums from your monthly pension check. Complete the form and return it to FPPA s Payroll Specialist. Members may have to pay the first few monthly premiums directly prior to having the deduction set up on the payroll system. Premiums for this plan will vary by individual and region. For all FPPA Active Members, Retirees, Survivors and their Families. HealthTrans Prescription Discount Card This FREE discount prescription drug card provides discounts on prescription drugs at more than 58,000 participating pharmacies. Saving 10% - 85% immediately on the cost of both brand-name and generic drugs. Be sure to use the Pharmacy Locator and Medication Pricing tools at www.fppaco.org, click on Retiree Insurance then the link for the FPPA Contracted Insurance Service Provider, to find the closest pharmacy, as well as those that offer the lowest prices for medication. No cost to sign up No obligations No limits on number of prescriptions Can be used by family and friends. This is NOT insurance. It is a discount prescription drug program. This prescription discount card through HealthTrans is available to all FPPA active members, retirees, survivors and their families. There is no cost for this program. Dental Insurance Delta Dental Insurance Retirees may choose to participate in a Delta Dental of Colorado Retiree Dental Plan. This plan offers high, medium and low options; has a large nationwide network of dentists and allows for out of network dentists. Preventative, basic and major services are covered at various benefit levels, annual maximums, and deductibles. Member Eligibility The FPPA retiree, their spouse and/or dependants to age 26, or a survivor if covered before the death of the member. Waiting periods will not apply for retirees who enroll within 60 days of retirement. Premium Payments A deduction from your pension check is required to participate in the program. Please note: In order to participate in the plan, your pension payment from FPPA must be large enough to cover the entire premium. Vision Insurance VSP and Eyemed Vision Insurance FPPA currently offers two different vision programs, Vision Service Plan (VSP) and EyeMed Vision Plan. You may enroll in these programs at any time. Both VSP and EyeMed have an extensive list of participating doctors from which to choose. Be sure to check the literature to compare benefits and locate a provider near you. Long-term Care Insurance A serious illness, injury or disability can strike at any age. If something were to happen, the physical, emotional and financial consequences could be devastating to your family and to your retirement plan. This policy helps protect your family s assets by providing coverage for not only nursing homes but assisted living facilities and home health care options. Contact a HUB International Representative. See info below. Member Eligibility An FPPA retiree, their spouse, parents, parents-in-law, stepparents, and step-parents-in-law, children and stepchildren between the ages of 18 and 84 (may vary by state). Life Insurance Looking for life insurance? The various life insurance vehicles on the market can make it confusing. HUB International can make the process easier. A representative will discuss your particular situation then shop various companies and policies to find a plan that best fits your needs. Contact a HUB International Representative. See info below. Supplemental Plans Cancer Insurance Helps offset the medical expenses related to cancer treatment. In the U.S., men have a little less than a 1-in-2 lifetime risk of developing cancer; for women the risk is a little more than 1-in-3. Over $37 billion is spent each year to treat cancer. Intensive Care Insurance Covers confinement in hospital intensive care. ICU costs can soar well above those of a general room, as well as above the benefit levels of standard health insurance policies. Accident / Sickness / Short-Term Disability Insurance Helps provide a financial cushion beyond an accident to include sickness and off-the-job disability riders. Connect for Health Colorado Connect for Health Colorado is a new non-profit entity established by a state law, Senate Bill 11-200, that was passed in 2011. Connect for Health Colorado is a health insurance marketplace, scheduled to open for business in October 2013 for individuals, families and small employers across Colorado. This one-stop online marketplace will include a customer support network of Customer Service Center Representatives, Health Coverage Guides and licensed brokers to help Coloradans find the best health plan for their needs. Connect for Health Colorado will also be the only place where Coloradans can find out if they are eligible for new federal financial assistance, based on income, and use the benefits right away. Coverage begins January 1, 2014. Through Connect for Health Colorado you may: Window shop to get a sense of health plan benefits and prices Get a quick estimate of how much financial assistance, based on your income, you could be eligible to use to reduce the cost of health insurance. Shop and enroll in health plans without financial assistance. Fill out the full online application for financial assistance and use those benefit right away to purchase health insurance.
