CBP Traveler Entry Forms CBP Declaration, I-94, and I-94W

Welcome to the United States Whether you are a visitor to the United States or US citizen, each individual arriving into the United States must complete one or more of US Customs and Border Protection s (CBP) entry forms This publication will provide you with detailed instructions on filling out those entry forms Every traveler must complete the CBP Declaration Form 6059B This form provides CBP with basic information about who you are and what you are bringing into the country, such as agriculture products and whether or not you have visited a farm prior to traveling to the United States If you are traveling with other immediate family members, you can complete one form for your entire family Some travelers will need to complete a CBP Form I-94 This form must be completed by all travelers ecept US citizens, returning resident aliens, aliens with immigrant visas, and Canadian citizens who are visiting or in transit Nonimmigrant visitors who are seeking entry to the United States under the Visa Waiver Program must fill out the CBP Form I-94W If you have questions about your form that are not answered in this publication, please don t hesitate to ask a CBP officer for help

CBP Declaration Form Instructions Side 1 4 9 0 Print your last (family) name Print your first (given) name Print the first letter of your middle name Print your date of birth in the appropriate day/ month/year boes Print the number of family members traveling with you (do not include yourself) Print your current street address in the United States If you are staying at a hotel, include the hotel s name and street address Print the city and the state in the appropriate boes Print the name of the country that issued your passport Print your passport number Print the name of the country where you currently live Print the name of the country(ies) that you visited on your trip prior to arriving to the United States If traveling by airline, print the airline s name and flight number If traveling by vessel (ship), print the vessel s name Mark an X in the Yes or No bo Are you traveling on a business (work-related) trip? Mark an X in the Yes or No bo Are you bringing with you: A Fruits, plants, food or insects? B Meats, animals, or animal/wildlife products? C Disease agents, cell cultures, or snails? 4 Mark an X in the Yes or No bo Have you or any family members traveling with you been in close proimity of (such as touching or handling) livestock outside the United States? Mark an X in the Yes or No bo Are you or any family members traveling with you bringing $10,000 or more in US dollars or foreign equivalent in any form into the United States? Read definition of monetary instruments on the reverse side of the form Eamples: coins, cash, personal or cashier s check, traveler s checks, money orders, stocks, bonds If yes, you must complete the FinCen 105 Currency Reporting Form Mark an X in the Yes or No bo Are you or any family members traveling with you bringing commercial merchandise into the United States? Eamples: all articles intended to be sold or left in the United States, samples used for soliciting orders, or goods that are not considered personal effects If you are a US resident, print the total value of all goods (including commercial merchandise) you or any family members traveling with you have purchased or acquired abroad (including gifts for someone else, but not items mailed to the United States) and are bringing into the United States Note: Visitors (non-us Residents) are normally entitled to a duty-free eemption of $100 on items accompanying them If you are a visitor (non-us Resident), print the total value of all goods (including commercial merchandise) you or any family members traveling with you are bringing into the United States and will remain in the United States 4

Note: US residents are normally entitled to a duty-free eemption of $800 on items accompanying them Declare all articles on this form For gifts, please indicate the retail value Use the reverse side of this form if additional space is needed to list the items you will declare A CBP officer will determine your duty Duty will be assessed at the current rate on the first $1,000 above the eemption Read the notice on the reverse side of the form Sign the form and print the date Keep the complete form with you and hand it to the CBP officer when you approach the US Customs and Border Protection area CBP Traveler Entry Forms

CBP I-94 Form Instructions For nonimmigrant visitors entering the United States with a visa, there is a requirement to fill a Form I-94 (white form) This form has two specific perforated sections to it The visitor or the carrier representative fills out Form I-94 upon arrival in the United States The bottom section of Form I-94 is a departure coupon and must be returned to US officials upon eiting the United States The information requested on Form I-94 includes: Full Name Date of Birth Gender Country of Birth Country of Citizenship Passport Number Airline and Flight Number City Where Boarded Address While in the United States In addition, there are several fields on the back upper portion designed for immigration related issues such as alien number, student number, and receipt number, which also need to be completed upon arrival to the United States The back lower portion requests: Full Name Birth Date Country of Citizenship After the successful completion of processing the applicant, a CBP officer stamps the applicant s arrival and departure portions of the completed Form I-94, the passport and the customs declaration The officer retains the arrival portion

of the Form I-94 and returns the departure portion of the Form I-94 and passport to the applicant The departure portion of Form I-94 and passport is to be in the applicant s possession at all times until the applicant departs the United States If an applicant boards a commercial conveyance to depart the United States, the transportation carriers are: To remove the departure portion of the Form I-94 from the applicant s passport Annotate the reverse of the departure portion of the Form I-94 with the facts of the applicant s departure from the United States Return the departure portion of the Form I-94 to the nearest CBP office It is the applicant s responsibility to make sure the transportation carriers collect the applicant s departure portion of the Form I-94 If an applicant departs the United States by way of land through Canada or Meico, it is the applicant s responsibility to return the departure portion of the Form I-94 to CBP prior to his/her departure CBP Traveler Entry Forms 9

CBP I-94W Form Instructions The Form I-94W (green form) is for all nonimmigrant visitors seeking entry to the United States under the Visa Waiver Program Applicants applying for admission to the United States using a Form I-94W are applicants who are eempt from having a visa Contact your travel agent, airline or US Customs and Border Protection (CBP) for more specific instructions regarding US visa requirements In addition to the Form I-94 requirements, the Form I-94W includes specific questions related to inadmissibility issues If the applicant answers Yes to any of the questions on the reverse side of the Form I-94W, the applicant should contact the United States Embassy/Consulate in his/her country to obtain a visa prior to travel The applicant must sign and date the Form I-94W, which indicates agreement to waive his/her right to a hearing before an immigration judge, if found inadmissible After the successful completion of processing the applicant, a CBP officer stamps the applicant s arrival and departure portions of the completed Form I-94W, the passport and the customs declaration The officer retains the arrival portion of the Form I-94W and returns the departure portion of the Form I-94W and passport to the applicant The departure portion of the Form I-94W and passport is to be in the applicant s possession at all times until the applicant departs the United States If an applicant boards a commercial conveyance to depart the United States, the transportation carriers are: To remove the departure portion of the Form I-94W from the applicant s passport 12
