INTRODUCTION The Special Education Student Services Delivery Continuum was created to assist districts in the process of exploring appropriate placement alternatives for their students who require either more or different support than exists within the district program. Programs included are organized according to: Center based sites with their satellite, included sites Geographical location Student needs Elementary Secondary Academic/Behavioral Developmental In addition to assisting districts in identifying an appropriate placement, the guide offers insight into where a student might move after initial placement, if he/she ages out of his/her current setting, or if he/she achieves to the point of readiness for a less restrictive (included) placement. In reviewing the Student Profiles that describe the population enrolled in any program, it should be noted that descriptors such as mild, moderate, severe are used in a different context by Eastern Suffolk BOCES than they would be by component district personnel. It should be assumed that a student whose disability is described as minimal within a local district setting would not typically be considered for a BOCES placement. In order to be recommended for a BOCES placement, a district would most likely perceive a student as having moderate to severe disabilities in comparison to their average district population with disabilities. As such, interpretation of Eastern Suffolk Student Profile descriptors should include an understanding of these adjectives as being used within the context of a population with significant disabilities.

New York State Standards are presented through individualized, goal and data driven instructional programs that foster the development of communication and social skills while increasing independence in academic, prevocational and social skills, through a hierarchy of structured learning opportunities. SERVICE DESCRIPTION The Masera Learning Center provides instruction to children with moderate to severe developmental disabilities within the Autism Spectrum. We work as a team, in cooperation with the parents, to ensure that each student develops to his/her maximum potential academically, socially, and physically and achieves his/her highest level of independence. Multi-modality approaches to learning best serves our students while striving to achieve their goals. SERVICE DESCRIPTION CENTER BASED FUNCTIONAL ACADEMIC PROGRAM The Functional Academic Program at Premm Learning Center services students with moderate to severe developmental disabilities. The primary goal of the program is to maximize each student s potential and to teach skills that will enable them to become as independent as possible within the confines of their disability. Program components include academics with an emphasis on functional skill development. Behavior management, independent living and social skills are also addressed. Functional assessments of behavior and behavior intervention plans are implemented for support when needed. CLASSIFICATIONS Multiple Disabilities Intellectual Disability Other Health Impairment Speech Impairment Visual Impairment Autism Hearing Impairment 12:1:4 PROGRAM DESCRIPTION The 12:1:4 Program at Premm Learning Center emphasizes a multi-sensory approach to learning. Emphasis is placed on improving cognition, sensory motor skills development and use of adaptive equipment to aid movement and independence. Students placed in the 12:1:4 class component are non-ambulatory and are dependent upon staff for meeting their needs throughout the school day. Classroom instruction is provided with hand over hand assistance. Related services of speech, occupational therapy and physical therapy are provided on a group push-in basis. All students are assessed using a New York State Alternate Assessment. CLASSIFICATIONS Multiple Disabilities 9

PREMM (Continued) INCLUSIVE CLASS SATELLITE SERVICES The Premm Learning Center offers inclusion class components for elementary and middle school aged students within off site district integrated programs. The students serviced within the inclusive programs experience learning disabilities but exhibit minimal to no behavioral concerns. The primary goal of the inclusion program is to maximize each student s potential and to act as the bridge to possible return to home school district. Instruction in these classes emphasizes a modified New York State curriculum, behavior management, and self-advocacy and social skills, while also providing for the opportunity to be integrated with the mainstream population for special subject classes, lunch, assemblies and academic subjects when appropriate. Program components include academics with an emphasis on functional skill development. Behavior management, independent living, and social skills are also addressed. Where needed, discrete trial instruction and applied behavioral analysis are utilized in conjunction with grade level curriculum. Functional assessments of behavior and behavior intervention plans are put in place for support when needed. Emphasis is placed on improving cognition, sensory motor skills development and use of adaptive equipment to aid movement and independence. Students placed in the 12:1:4 class component are non-ambulatory and are dependent upon staff for meeting their needs throughout the school day. Classroom instruction is provided with hand over hand assistance. Related services of speech, occupational therapy and physical therapy are provided on a group push-in basis.
