Foreword From The Author Author: Antonio Chirumbolo O.D. Antonio attended the SUNY College of Optometry and returned to his hometown, Pittsburgh, Pennsylvania to practice shortly after graduating. He now works in a large private practice setting providing care to his local community. In addition to patient care, Antonio possesses a marked interest in optometric industry. Introduction A license granted by each state s regulatory board of optometry must be obtained upon graduating from an accredited optometry school for optometrists to legally practice in the United States. Obtaining a license varies from state to state in regards to fees, requirements, and application processes. The following is meant to serve as a guide to help new graduates of optometry obtain the proper information to successfully complete the licensing application process based on state. Note: Fees are approximations. There may be additional fees or different fees for obtaining fingerprinting if not residing in the state you are applying for a license. There may also be other fees associated with obtaining documents, having scores and/or transcripts sent, or notarizing documents. Remember licensing is half the battle. Alabama Examinations Required: NBEO Part I, II, III and completion of a written exam given by the Alabama Board of Optometry Fees: $500 1. Complete the Alabama Board of Optometry Application. 2. Submit official transcripts from the schools and colleges attended for optometry, undergraduate, and high school attended. 3. Complete the background check requirement (Instructions are within application). 4. Submit proof that you have passed NBEO Parts I, II, and III. 5. Pass the standard examination given by the Alabama Board of Optometry (information regarding exam can be found in the application). 6. Complete and submit to the Board for approval a protocol furnished by the Board for the use of pharmaceutical agents in the treatment of the eye and its adjacent structures upon Alaska passing standard examination given by Alabama Board of Optometry. Examinations Required: NBEO Part I, II, III, TMOD, ISE, and completion of law exam given by the Alaska Board of Optometry. Fees: $500 1. Complete the Alaska Board of Optometry Application. 2. Authorize release of records for use by the Alaska Division of Corporations, Business and Professional Licensing and its investigators. 3. Submit official transcripts from optometry school. 4. Provide three notarized reference letters submitted directly from the person of reference. 5. Submit proof that you have passed NBEO Parts I, II, III, TMOD, and ISE. 6. Submit a copy of a valid DEA if appropriate. 7. Pass the Alaska State Optometry Law Examination (information regarding exam can be found in application) after approval of application and receipt of its contents. 8

Arizona Examinations Required: NBEO Part I, II, III, TMOD, and completion of law exam given by the Arizona Board of Optometry Fees: $572 1. Complete the Arizona Board of Optometry Application. 2. Provide copies of documents pertinent to applications statement of citizenship or alien status for state public benefits requirement. 3. Fingerprints taken by a criminal justice agency. 4. Submit payment to obtain a state and federal criminal records check. 5. Submit proof that you have passed NBEO Parts I, II, III, and TMOD 6. Submit official transcripts directly to the Board for undergraduate and optometry school. 7. Submit a self-query from the National Practitioner Data Bank-Healthcare Integrity and Protection Data Bank made 3 months before the date of the application (more information regarding this process within the application). 8. Submit a copy of CPR certification. 9. Pass the Arizona State Optometry Jurisprudence Examination (information regarding exam can be found in application) that can be taken at any time during the application process. Arkansas Examinations Required: NBEO Part I, II, III, TMOD, and completion of law and pharmacology exam given by the Arkansas Board of Optometry. Fees: $400 1. Submit an application to the Secretary of the Board dictating his/her name, address, date of birth, educational history with locations and dates of attendance. 2. Provide three non-related personal references to attest to the applicant s character. 3. Successfully pass the mandated Arkansas State Board examination that is administered at least twice a year in either oral, written or both oral and written format. 9

California Examinations Required: NBEO Part I, II, III, and completion of law exam given by the California Board of Optometry. Fees: $409 1. Complete the California Board of Optometry Application. 2. Fingerprints taken by a criminal justice agency. 3. Submit proof that you have passed NBEO Parts I, II, and III. 4. Submit official transcripts directly to the Board for optometry school. 5. Pass the California State Optometry Law Examination after processing of application and its contents. Colorado Examinations Required: NBEO Part I, II, III, TMOD and completion of law exam given by the Colorado Board of Optometry. Fees: $200 1. Complete the Colorado Board of Optometry Application. 3. Submit official transcripts directly to the Board for optometry school. 4. Complete an online healthcare professions profile (more information can be found in the application). 5. Complete the Colorado Board of Optometry take home law exam (part of application) Connecticut Examinations Required: NBEO Part I, II, III, and TMOD Fees: $565 10

1. Complete the Connecticut Board of Optometry Application. 3. Submit official transcripts directly to the Board for optometry school. Delaware Examinations Required: NBEO Part I, II, III, and TMOD Fees: $268 1. Complete the Delaware Board of Optometry Application. New graduates that wish to practice in Delaware must complete a six-month internship in optometry that consists of 35 hours a week for a six-month period under proper supervision. 2. Gain pre-approval to complete the six-month internship, which involves supervision by an optometrist approved by the Board (more information can be found within the application). 3. Submit proof that you have passed NBEO Parts I, II, and III and TMOD. 4. Submit official transcripts directly to the Board for optometry school. 5. Submit a copy of CPR Certification. 6. Submit a self-query from the National Practitioner Data Bank-Healthcare Integrity and Protection Data Bank (more information regarding this process within the application) 7. Successfully complete the six-month internship period and submit notarized Statement of Supervising Doctor forms for any and all doctors who supervised your during the internship period to the Board (forms found within application). District of Columbia Examinations Required: NBEO Part I, II, III, and TMOD Fees: $483 1. Complete the District of Columbia Board of Optometry Application. 3. Submit official transcripts directly to the Board for optometry school. 4. Complete a criminal background check. 11

Florida Examinations Required: NBEO Part I, II, III, TMOD, and completion of law examination Fees: $655 plus cost of law exam fee at time of testing ($58.00) 1. Complete the Florida Board of Optometry Application. 2. Complete a two-hour course on the prevention of medical errors. More information regarding where these courses can be taken and what courses are approved for Florida licensure can be found at CEBroker. 3. Submit proof that you have passed NBEO Parts I, II, and III and TMOD. 4. Submit official transcripts directly to the Board for optometry school. 5. Complete licensure verification form for each state you currently hold a license in. 6. Pass the Florida State Board law examination after processing and approval of application that will need to be scheduled at a Prometric testing center. Georgia Examinations Required: NBEO Part I, II, III, TMOD, and completion of law examination administered by Georgia Board of Optometry Fees: $300 1. Complete the Georgia Board of Optometry Application, which can also be completed online. 3. Submit official transcripts directly to the Board for optometry school. 4. Submit a copy of CPR Certification. 5. Pass the Georgia rules and laws examination (more information regarding these exams is contained within the application documents). 6. Obtain a minimum of $1,000,000 in malpractice insurance coverage upon licensure. 12

Hawaii Examinations Required: NBEO Part I, II, III, and TMOD Fees: $75 in an odd numbered year, or $160 in an even numbered year 1. Complete the Hawaii Board of Optometry Application. 3. Submit official transcripts directly to the Board for optometry school. Idaho Examinations Required: NBEO Part I, II, III, TMOD, and completion of law examination administered by the Idaho Board of Optometry Fees: $110 1. Complete the Idaho Board of Optometry Application. 3. Submit official transcripts directly to the Board for optometry school. 4. Submit a copy of CPR certification 5. Complete the law examination (open book and contained within the application). Illinois Examinations Required: NBEO Part I, II, III, and TMOD Fees: $500 1. Complete the Idaho Board of Optometry Application. 3. Submit official transcripts directly to the Board for optometry school. 13

Indiana Examinations Required: NBEO Part I, II, III, TMOD and completion of law exam given by the Indiana Board of Optometry. Fees: $200 1. Complete the Indiana Board of Optometry Application. 2. Obtain fingerprinting to complete a national criminal history background check at a cost to the applicant (more information can be found here). 3. Submit proof that you have passed NBEO Parts I, II, and III and TMOD. 4. Submit official transcripts directly to the Board for optometry school. 5. Submit a copy of a current Basic Life Support certification card. 6. Pass the Indiana State Optometry Law Examination after processing of application and its contents. Iowa Examinations Required: NBEO Part I, II, III, TMOD Fees: $300 1. Complete the Iowa Board of Optometry Application. 3. Submit official transcripts directly to the Board for optometry school. Examinations Required: NBEO Part I, II, III, TMOD and completion of law, written, practical examination and interview given by the Kansas Board of Optometry. Fees: $150 1. Complete the Kansas Board of Optometry Application. 2. Obtain fingerprinting to complete a criminal history background check (applicant must contact the board to obtain fingerprinting packets). 3. Submit proof that you have passed NBEO Parts I, II, and III and TMOD. 4. Submit official transcripts directly to the Board for optometry school. 5. Provide three personal character references. 6. Pass the Kansas State Board of Optometry written, practical, and law examinations and successfully interview after processing of application and its contents. More information will be provided to you regarding the examinations once your application is received. Kentucky Examinations Required: NBEO Part I, II, III, TMOD and completion of law examination given by the Kentucky Board of Optometry. Fees: $525 1. Complete the Kentucky Board of Optometry Application. 2. Obtain fingerprinting to complete a criminal history background check. 3. Submit proof that you have passed NBEO Parts I, II, and III and TMOD. 4. Submit official transcripts directly to the Board for optometry and undergraduate schools. 5. Provide two personal character references, one by an optometrist. 6. Pass the Kentucky State Board of Optometry law examination after processing of application and its contents. Louisiana Examinations Required: NBEO Part I, II, III, TMOD and completion of practical examination given by the Louisiana Board of Optometry. 15

Fees: $100 1. Complete the Louisiana Board of Optometry Application. 3. Submit official transcripts directly to the Board for optometry, undergraduate, and high school. 4. Pass the Louisiana State Board of Optometry practical examination after processing of application and its contents. Maine Examinations Required: NBEO Part I, II, III, TMOD and completion of oral examination given by the Maine Board of Optometry. Fees: $300 1. Complete the Maine Board of Optometry Application. 4. Pass the Maine State Board of Optometry oral examination after processing of application and its contents, which is given only four times a year. Maryland Examinations Required: NBEO Part I, II, III, TMOD and completion of law examination (open book / take home) given by the Maryland Board of Optometry. Fees: $300 Note: several forms require notarization. 1. Complete the Maryland Board of Optometry Application. 4. Provide four professional letters of reference attesting to personal character. 5. Complete application for DPA and TPA certification. 16

6. Provide proof of CPR certification. 7. Pass the Maryland State Board of Optometry law examination which is open book and take home. Massachusetts Examinations Required: NBEO Part I, II, III, TMOD and completion of law examination given by the Massachusetts Board of Optometry. Fees: $408 1. Complete the Massachusetts Board of Optometry Application. 4. Pass the Massachusetts State Board of Optometry law examination. Michigan Examinations Required: NBEO Part I, II, III, and completion of law examination given by the Michigan Board of Optometry (take home). Fees: $405 1. Complete the Michigan Board of Optometry Application. 4. Obtain fingerprinting to complete a criminal history background check (instructions included in application). 5. Provide proof of DPA and TPA certification 6. Provide proof of CPR certification. 7. Pass the Michigan State Board of Optometry law examination that is included in the application. 17

Minnesota Examinations Required: NBEO Part I, II, III, TMOD and completion of law examination given by the Minnesota Board of Optometry. Fees: $145.70 1. Complete the Minnesota Board of Optometry Application. 3. Submit official transcripts directly to the Board for optometry school. 4. Complete applications for TPA certification. 5. Pass the Minnesota State Board of Optometry law examination prior to the boards review of your application, and the exam can be scheduled at the office of the Minnesota Board of Optometry by appointment or through NBEO at www.optometry.org. Mississippi Examinations Required: NBEO Part I, II, III, TMOD and completion of law and pharmacology examination given by the Mississippi Board of Optometry. Fees: $200 in state or $300 out of state 1. Complete the Mississippi Board of Optometry Application. 4. Submit two letters of recommendation attesting to personal character. 5. Pass the Mississippi State Board of Optometry law and pharmacology examination that will be scheduled after processing of application and its contents. The exam is administered on the second Saturday in January and July of each year. 18

Missouri Examinations Required: NBEO Part I, II, III, TMOD and completion of law examination (open book) given by the Missouri Board of Optometry. Fees: $408 1. Complete the Missouri Board of Optometry Application. 4. Obtain fingerprinting to complete a criminal history background check (more information can be found in application and an additional fee of $36.50 applies). 5. Pass the Missouri State Board of Optometry law examination that can be scheduled at the office of the Missouri Board of Optometry by appointment or through NBEO at www.optometry.org. Montana Examinations Required: NBEO Part I, II, III, and TMOD Fees: $250 1. Complete the Montana Board of Optometry Application. 3. Submit official transcripts directly to the Board for optometry school. 4. Provide three reference letters of moral character. 5. Complete a National Practitioner s Databank (NPDB) self query. Nebraska Examinations Required: NBEO Part I, II, III, TMOD and completion of law examination given by the Nebraska Board of Optometry. Fees: $166 19

1. Complete the Nebraska Board of Optometry Application. 2. Obtain fingerprinting to complete a criminal history background check (an additional fee of $38 applies). 3. Provide proof of age and citizenship, lawful residency, or immigration status. 6. Pass the Nebraska State Board of Optometry law examination after processing of application and its contents. Nevada Examinations Required: NBEO Part I, II, III, and completion of law examination given by the Nevada Board of Optometry. Fees: $500 1. Complete the Nevada Board of Optometry Application. 2. Submit proof that you have passed NBEO Parts I, II, and III. 4. Provide a copy of a current CPR certification card. 5. Pass the Nevada State Board of Optometry law examination after processing of application and its contents. New Hampshire Examinations Required: NBEO Part I, II, III, TMOD and completion of law and clinical proficiency examination given by the New Hampshire Board of Optometry. Fees: $300 1. Complete the New Hampshire Board of Optometry Application. 20

4. Provide two personal character references by practicing optometrists. 5. Provide a copy of level C CPR certification. 6. Pass the New Hampshire State Board of Optometry law examination and clinical proficiency examination given by NERCOATS or any other successor agency approved by the board after processing of application and its contents. Obtain fingerprinting to complete a criminal history background check. 3. If licensed in another state, complete a self-query with the National Practitioners Databank. 4. South Dakota Examinations Required: NBEO Part I, II, III, TMOD and completion of law examination given by the South Dakota Board of Optometry.
