DISCLOSURE STATEMENT SINGLE PREMIUM IMMEDIATE ANNUITY In connection with the purchase of my Security Mutual Life Single Premium Immediate Annuity I acknowledge and understand the following: 1. I understand the start date for annuity income payments cannot be changed after issue. 2. I understand the annuity option which I select cannot be changed after issue. 3. I understand that if I have selected an annuity that does not include a guarantee period, the annuity income payments will cease when the annuitant dies or if this is an annuity that is based upon two lives, upon the death of the annuitant last to die. 4.
