ORIENTATION We have assembled these Orientation materials to familiarize you with our policies, procedures and requirements and to provide the necessary information you will need as a professional field staff member of Attorney Resource/Houston (Attorney Resource). Your signature acknowledges receipt, understanding and acceptance of these policies, procedures and requirements. WELCOME Some history of Attorney Resource, a description of how we operate and what is expected of you as a member of our staff. SAFETY PROGRAM AND PRACTICES Provides tips and information to help prevent accident and/or injury and maintain on-the-job safety. SUBSTANCE ABUSE POLICY Prohibits use of alcohol, inhalants or illegal drugs by an employee during work hours or on company premises. WELCOME Attorney Resource is proud to welcome you as a member of our temporary staff personnel. We have been working with the legal profession since 1972. The secret to our success and longevity is honesty, integrity and professional care among ourselves, our clients and most importantly YOU, our field staff. As a part of our program, we have listed the following policies and procedures which apply to all our employees. All members of the temporary staff at Attorney Resource are expected to be professionals in their field. A professional, cooperative attitude is necessary for you and Attorney Resource to maintain a congenial and mutually beneficial working relationship. By following these four Rules of Courtesy you will earn you the respect you deserve from Attorney Resource as well as the clients you serve: PROFESSIONALISM As a member of our field staff, we require that you maintain a professional attitude in manner and in dress. PUNCTUALITY Arrive at the given time for your assignment. Our clients call us because they need assistance. We must be there to fill that need. SAFETY PROGRAM & PRACTICE BASIC SAFETY POLICY The personal safety and health of each employee of Attorney Resource is of primary importance. Accordingly, all employees must comply with all rules and regulations of our safety program and continuously practice safety while at work. Failure to observe any part of these rules or other common safe work practices will result in disciplinary action and/or termination. GENERAL SAFETY RULES FIRST AID: Familiarize yourself with first aid locations in your building. FIRE EXITS: Be familiar with locations of fire alarms, exits and evacuation plans for the building in which you work. SAFETY SIGNS AND POSTERS: be followed. These are for your information and protection and must INJURIES: Report all injuries to Attorney Resource no matter how slight. UNSAFE CONDITIONS AND WORK PRACTICES: Attorney Resource immediately. Report all unsafe acts or conditions to HOUSEKEEPING AND WORK PRACTICES: Good housekeeping is the foundation of a safe workplace. Keep your work area and equipment clean and neatly organized. Report all unsafe acts or conditions to Attorney Resource immediately. LIFTING HEAVY MATERIALS: Stand close to the load with your feet apart for good balance. Get into a comfortable lifting position by placing one foot slightly behind the other. Bend your knees, not your waist, and get a good grip. Lift steadily and smoothly with your leg muscles, flexing your knees and your body when moving heavy objects. If in doubt, get assistance. IMPROPER USE OF OFFICE EQUIPMENT: Be especially careful when moving objects stored overhead. Do not stand on chairs, tables, desks, boxes or other makeshift supports to obtain or reach overhead objects. WALK AT ALL TIMES: Use handrails on stairways. Be particularly careful when wearing high heels. Open doors slowly and do not enter rooms which are not properly lighted. Watch for conditions which may cause falls, such as electrical outlets in floors or other objects. Page 3 of 9

SUBSTANCE ABUSE POLICY Attorney Resource has adopted, maintains and strictly enforces the following drug abuse policy: Being under the influence of or any unlawful use, possession, purchase, distribution, dispensation or manufacture of any controlled substance, alcohol or dangerous drug (as defined by the Texas Controlled Substance Act) while on the premises of Attorney Resource or the premises of our clients is prohibited. Reporting to work or attempting to perform services for Attorney Resource while in a condition of intoxication is prohibited. Any employee who uses prescription drugs must so inform that employee s supervisor if such use may impair the employee s ability to perform any assigned job duty safely and efficiently. For the purpose of this policy, the term drug shall be deemed to include alcoholic beverages, inhalants, illegal drugs and the use of legal drugs. Compliance with this policy by every employee of Attorney Resource, whether full time or temporary, is a condition of continued employment. Any violation of this policy is inconsistent with the interests of Attorney Resource. Violation of any prohibition or requirement of this policy is grounds for immediate disciplinary action including: Immediate dismissal of employment and removal from the premises of Attorney Resource or any client of Attorney Resource. Attorney Resource does not provide, sponsor or endorse any alcohol or drug rehabilitation program. Such programs are available through public and private health care facilities. Affected employees are encouraged to seek assistance for themselves before the situation affects job performance or becomes a matter of employment-related concern. Attorney Resource does not routinely require drug testing unless such testing is required by a client of Attorney Resource. Page 4 of 9

PERSONAL CONDUCT AND COMMUNICATIONS CONFIDENTIALITY As a member of our professional field staff, you will be assigned to work as an Attorney Resource assigned employee in office locations provided by our clients. As such, you must: Comply with office procedures and policies required by the Client/Customer. Maintain business records, information and confidential materials of the Client/Customer which are disclosed or made available to you in strict confidence. Refuse to accept any assignment which would require you to use, disclose or otherwise compromise any duty of confidentiality owed to a former employer or Client/Customer. CONTINUED BUSINESS Personal business cards will be provided for you. Fill in your name on the line provided. At the conclusion of your assignment, give your Client/Customer supervisor one of your personal business cards and express your interest in working with them again as an Attorney Resource Assigned Employee. CONTACT US You are required to notify Attorney Resource immediately by phone and/or email: If you are ill or circumstances prevent you from going to your assignment. Failure to appear for an assignment without prompt and proper notice to Attorney Resource will be grounds for termination. If you are going to be late to an assignment. Tardiness for an assignment without excuse will be grounds for termination. If you encounter problems with your assignment or you are able to complete an assignment. If you have questions regarding your assignment or your assignment responsibilities are changed. You must notify Attorney Resource by contacting your Assigned Employee Coordinator: Rhonda A. AVAILABILITY REPORTING You must report to our office in person, by phone, by email or online to report your availability for work assignment: When your assignment is completed. If you fail to report your availability within twentyfour (24) hours after completion of an assignment, your employment by Attorney Resource will be considered voluntarily terminated. Such voluntary termination may adversely affect a claim for unemployment benefits. Every week. If Attorney Resource does not hear from you weekly, we will assume you are otherwise employed and unavailable for work assignment. Failure to report your availability weekly may result in denial of unemployment benefits. Our responsibility to you does not end at 5:30 p.m. Our business phone transfers to an active voice mail system at close of the business day. TIME REPORTING AND PAY WEEKLY TIME RECORD (copy attached). You will be paid based on the information supplied in your Weekly Time Record. Please take a copy with you to each assignment or save it in your email and access it as needed. (This document is available on our website attorneyresource.com) You must deliver your Weekly Time Record to Attorney Resource no later than NOON EACH MONDAY for the prior work week. We will not release your paycheck until we receive a Weekly Time Record approved by the Client/Customer. You may: a) Email your signed approved Weekly Time Record before noon on Monday to Rhonda A. Parker at rap@attorneyresource.com; or b) Fax your signed approved Weekly Time Record before noon on Monday to Rhonda A. Parker at 713.980.7052. You cannot be paid unless we receive a Weekly Time Record signed and approved by the Client/Customer. PAY PERIOD The work week ends on Sunday and paychecks are issued weekly. Paychecks for the preceding work week are ready for distribution each Wednesday. However, if Monday is a holiday your paycheck will be issued on Thursday. You may choose one of the following: 1. Direct Deposit If you complete, sign and submit the proper authorization form, your paycheck will be deposited to your designated bank account on Wednesday. 2. Mail We will mail your check on Wednesday by 3:00 p.m. unless you have authorized direct deposit or marked your Weekly Time Record pick-up. 3. Pick Up If you mark the Pick Up option on your Weekly Time Record, you may pick up your check at our office between 12:30 p.m. and 3:00 p.m. on Wednesday. Otherwise, your paycheck will be mailed. If your approved Weekly Time Record is not received by noon on Monday, you will not receive a paycheck until the following week. Page 7 of 9

BENEFITS, BONUSES & INCENTIVES VACATION PAY You will receive one week s pay as vacation pay based on your average weekly pay for 40 hours per week when you have accumulated 2,000 hours on worked paid assignments. Thereafter, you will receive one week s vacation pay for every 1,000 hours worked. PAID HOLIDAYS New Year s Day Memorial Day Independence Day Labor Day Thanksgiving Day Christmas Day In order to be paid for these holidays, you must: Have accumulated 1,000 hours of paid work; and Have worked all business working days of the holiday week. To receive pay for New Year s Day you must: Have been paid for both the Thanksgiving and Christmas holidays and Have worked the week prior to the New Year s Day holiday and the next business working day after New Year s Day. REFERRAL BONUS For each candidate employee you refer to Attorney Resource, you will receive $50.00 after your referral has worked 80 full hours or has been placed in a full-time position and fulfilled the guarantee period with an Attorney Resource client. HEALTH BENEFITS AFFORDABLE CARE ACT Attached is a copy of Form OMB No. 2010-0149 which informs you of the health care options available to you in the open marketplace. Attorney Resource does not offer paid health care coverage. By signing this Orientation you acknowledge that you have received Form OMB No. 2010-0149. ASSIGNED EMPLOYEE BENEFITS As a member of our Attorney Resource field staff you will be assigned to perform work at locations specified by our clients. However, you will remain employee of Attorney Resource and report directly to the Assigned Employee Coordinator listed herein. As an assigned employee, you will not be an employee of the Client/Customer and will not be entitled to any employee benefits offered by the Client/Customer to its employees. You will be entitled to the benefits offered by Attorney Resource listed herein. Page 8 of 9

LOSS OF BENEFITS You will be automatically disqualified from all benefits and incentives offered if you: Are inactive for over sixty (60) days. Accept full-time employment elsewhere Terminate from your employment with Attorney Resource either orally or in writing. Fail to report your availability within twenty-four (24) hours of completion of an assignment and thereafter on a weekly basis. Page 9 of 9

The assigned employee s time charges are correct as listed above; the assigned work was performed in a timely and satisfactory manner; and Client/Customer has agreed to pay Attorney Resource a sum equal to the product of the hours listed times a prearranged hourly rate. 2. All charges based on this Weekly Time Record are due and payable to Attorney Resource in Harris County, Texas upon receipt of invoice. 3. Any fees or expenses incurred by Attorney Resource in collecting any amounts due pursuant to this Weekly Time Record shall be reimbursed by Client/Customer and venue for any litigation arising regarding the services provided or non-payment of any invoice based on this Weekly Time Record shall be in Harris County, Texas. 4. Unless employment of the above-named employee is subject to a Temp to Hire Conversion agreement between Client/Customer and Attorney Resource, neither Client/Customer nor anyone controlled by Client/Customer will, within six (6) months following either the date on which Attorney Resource last provided services to Client/Customer or the date on which employment of the above-named employee by Attorney Resource was terminated, whichever date is later, employ or offer to employ the above-noted employee in any capacity other than as an assigned employee of Attorney Resource. If the above-named employee becomes employed, directly or indirectly, by Client/Customer or an entity controlled by Client/Customer in violation of the terms hereof, Client/Customer will pay to Attorney Resource a sum equal to twenty-five percent (25%) of such employee s anticipated annual salary. 5. Attorney Resource is the employer of record ( common law employer ) of the above-named assigned employee. As the legal employer, Attorney Resource has assumed employment-related responsibilities including, but not limited to, payment of wages and benefits (including ACA compliance) and payment of federal, state and local payroll taxes (including FUTA and SUTA). Attorney Resource also provides workers compensation insurance; establishes and enforces employment policies concerning job performance and conduct; and maintains the sole right to reassign, discipline or terminate employment of its assigned employees. v.1511

New Health Insurance Marketplace Coverage Options and Your Health Coverage Form Approved OMB No. 1210-0149 (expires 1-31-2017) PART A: General Information When key parts of the health care law take effect in 2014, there will be a new way to buy health insurance: the Health Insurance Marketplace. To assist you as you evaluate options for you and your family, this notice provides some basic information about the new Marketplace. What is the Health Insurance Marketplace? The Marketplace is designed to help you find health insurance that meets your needs and fits your budget. The Marketplace offers "one-stop shopping" to find and compare private health insurance options. You may also be eligible for a new kind of tax credit that lowers your monthly premium right away. Open enrollment for health insurance coverage through the Marketplace begins in October 2013 for coverage starting as early as January 1, 2014. Can I Save Money on my Health Insurance Premiums in the Marketplace? You may qualify to save money and lower your monthly premium, but only if your employer does not offer coverage, or offers coverage that doesn't meet certain standards. The savings on your premium that you're eligible for depends on your household income. Does Employer Health Coverage Affect Eligibility for Premium Savings through the Marketplace? Yes. If you have an offer of health coverage from your employer that meets certain standards, you will not be eligible for a tax credit through the Marketplace and may wish to enroll in your employer's health plan. However, you may be eligible for a tax credit that lowers your monthly premium, or a reduction in certain cost-sharing if your employer does not offer coverage to you at all or does not offer coverage that meets certain standards. If the cost of a plan from your employer that would cover you (and not any other members of your family) is more than 9.5% of your household income for the year, or if the coverage your employer provides does not meet the "minimum value" standard set by the Affordable Care Act, you may be eligible for a tax credit. 1 Note: If you purchase a health plan through the Marketplace instead of accepting health coverage offered by your employer, then you may lose the employer contribution (if any) to the employer-offered coverage. Also, this employer contribution -as well as your employee contribution to employer-offered coverage- is often excluded from income for Federal and State income tax purposes. Your payments for coverage through the Marketplace are made on an aftertax basis. How Can I Get More Information? The Marketplace can help you evaluate your coverage options, including your eligibility for coverage through the Marketplace and its cost.
