4.2-1 REPORT Meeting Date: 2015-07-09 Water Fluoridation Committee For Information DATE: REPORT TITLE: FROM: STRUCTURE OF PROPOSED EDUCATIONAL COUNCIL MEETING ON WATER Janette Smith, Commissioner of Health Services Eileen de Villa, MD MBA MHSc CCFP FRCPC, Acting Medical Officer of Health OBJECTIVE To obtain the Water Fluoridation Committee s direction on the structure and format of the upcoming educational Council Meeting on Water Fluoridation based on the proposal and options outlined in this report. REPORT HIGHLIGHTS The Water Fluoridation Committee s mandate is to organize an educational workshop for Regional Councillors to review the existing body of research regarding the practice of water fluoridation. Consistent with the Water Fluoridation Committee s direction, staff has prepared a proposed outline and two options for the structure of the educational meeting, and is requesting the Committee s direction. As well, in response to the Committee s request, this report includes proposed questions to be answered at the meeting by expert presenters. The proposed educational meeting will be a closed session Council Meeting, consistent with section 239 (3.1) of the Municipal Act, 2001. DISCUSSION 1. Background At its March 26, 2015 meeting, the Water Fluoridation Committee approved the key components of an educational meeting on community water fluoridation for Regional Councillors and directed staff to bring forward recommendations for the specific meeting structure. In accordance with the Committee s direction, a number of external presenters have been invited to present at the meeting. Eight individuals were contacted and four of them have accepted the invitation. The Committee identified that they would like to provide a list of questions to the presenters to frame their remarks at the meeting (refer to Section 3 below).

4.2-2 2. Meeting Details It is proposed that the meeting be held mid-fall 2015. A specific date will be chosen based on Council and presenter availability. The meeting details proposed below are guided by: a) Ensuring a comprehensive review of existing research evidence on water fluoridation, including concerns raised in the literature. b) Ensuring Council is well-informed on Peel s water fluoridation process and policy decision. c) Specific areas of expertise of the external presenters. Proposed Meeting Outline Meeting Facilitator: Dr. de Villa, Acting Medical Officer of Health 1. Water Fluoridation Dr. de Villa, Acting Medical Officer of Health, presentation on Context and Toxicology water fluoridation as a public health policy. Present the water fluoridation policy and process at the Region of Peel as well as information on fluoride toxicology. 2. External Presenters Present the research evidence related to current water fluoridation policies and concerns with water fluoridation. 3. Summary and Next Steps Public Works staff presentation on Peel s water fluoridation process. Fluoride toxicology expertise provided by leading local toxicologist. Expertise provided by external presenters, guided by key questions (see Section Three below for list of questions). Dr. de Villa will provide summary remarks and facilitate dialogue on next steps. Highlight key points and planned next steps of the Water Fluoridation Committee. - 2 -

4.2-3 Meeting Structure Options Key Considerations Option One: One-day session Focuses the presentations to a few select individuals. May not provide sufficient time and opportunity for presenters to cover complex information and for Councillors to reflect on presentations and ask questions. Option Two Two consecutive half-day sessions Provides an opportunity for more in-depth coverage of information and for Councillors to reflect on the information received. Potential challenges with Council and presenter availability over two consecutive days. 3. Focus of External Presentations The WFC agreed that external presenters (i.e., individuals with verifiable credentials and published peer-reviewed literature on the topic) should focus their remarks on specific subject areas, in particular the dental and health effects of water fluoridation. The following questions are recommended to frame the presentations at the meeting. Dental a) Based on the body of evidence, is water fluoridation an effective oral health strategy? Why? Why not? b) Based on the body of evidence and current oral health behaviors and context, is fluoridated water still needed? Why? Why not? Health a) Based on the body of evidence, is human consumption of fluoridated drinking water safe? Why? Why not? b) Based on the body of evidence, does human consumption of fluoridated drinking water result in any adverse health effects? FINANCIAL IMPLICATIONS The approximate cost related to participation of external presenters is $20,000-$25,000, depending on the final list of presenters and the duration of the meeting. This cost will be accommodated within the 2015 Public Health budget. - 3 -

4.2-4 CONCLUSION Staff is requesting the Water Fluoridation Committee s direction regarding the educational Council Meeting On Water Fluoridation, in order to organize the meeting for the Fall.
