ABOUT MOVEMBER DESCRIPTION BACKGROUND AND OBJECTIVES 02 ABOUT MOVEMBER During November each year, Movember is responsible for the sprouting of moustaches on thousands of men s faces, in Denmark and around the world. With their Mo s, these men raise vital funds and awareness for men s health, specifically prostate cancer and men s mental health initiatives. On Movember 1st, guys register at movember.com with a clean-shaven face. For the rest of the month, these selfless and generous men, known as Mo Bros, groom, trim and wax their way into the annals of fine moustachery. Supported by the women in their lives, Mo Sistas, Movember Mo Bros raise funds by seeking out sponsorship for their Mo-growing efforts. Mo Bros effectively become walking, talking billboards for the 30 days of November. Through their actions and words, they raise awareness by prompting private and public conversation around the often ignored issue of men s health. DESCRIPTION With this funding announcement, Movember invites applications for high quality, internationally recognized teams of investigators to complete a translational research project. The proposed research must have significant impact on our understanding of prostate cancer and aim to accelerate the process to a change in clinical practice or policy. Funds available in this competition are to support collaborative, multi-disciplinary, multi-institutional teams that address important research questions in prostate cancer in Denmark. Movember will provide up to 180,000 per year for 2 years ( 360,000 in total). The full 180,000 does not have to be requested for each year nor does the full 360,000 for this call have to be requested over the term of the grant. Anticipated expenditures can exceed 180,000 in a given year but the total over the term of the grant cannot exceed 360,000. The total number of teams that will be supported will be one (1). This is a two step application process that includes: 1) an administrative notice of Intent to Apply and 2) the Full Application. BACKGROUND AND OBJECTIVES Movember seeks applications for a new research project in Denmark in order to support strategic investments in collaborative, multidisciplinary, and multi-institutional research teams. This initiative recognizes that there is tremendous research capacity in Denmark in the field of prostate cancer research. Therefore, the opportunity is there to leverage this existing capacity and better coordinate and support research to address important and complex prostate cancer research issues. Specific objectives of the translational research project are: 1. To support high quality, innovative translational prostate cancer research that will lead to significant advancement in our understanding of prostate cancer. 2. To support multi-disciplinary and multi-institutional groups of talented prostate cancer researchers. 3. To support high quality research that addresses an important prostate cancer problem or issue which is best approached through collaborative teams. 4. To provide a superior research training and mentorship environment; 5. To support the creation of new knowledge leading to the translation of research findings into improvements in the health of Danish men suffering from prostate cancer.

BACKGROUND OBJECTIVES ELIGIBILITY 03 The project emphasis is on the production of new knowledge, and the translation and transfer of research findings to clinical practice to optimize impact. It is expected that the questions/issues addressed will be complex, and that results will only be achieved more rapidly and efficiently through a multi-disciplinary, collaborative effort as opposed to a series of separate, non-collaborative operating grants. Proposals that represent active, meaningful partnerships between research teams based in institutions in Denmark are required. ELIGIBILITY Each eligible translational research project will include: A Team Leader(s): the Team Leader(s) must be an established researcher with proven leadership skills and experience who will act as the Project Director, and who will assume administrative responsibility for the award. It is expected that the team leader(s) will devote a significant and appropriate portion of his/her time to the effort. A minimum of two additional Independent Investigators: Each project award application submitted in response to this call for proposals must consist of at least two additional independent investigators based in different institutions from each other and from the team leader. Additional team members, who collectively have an extensive record of success, are creative and original in their approach to research. The specific contribution of each team member must be described in the proposal. Representation from more than one research discipline and from at least two research themes biomedical, clinical, health system and services, population health, is required. In addition, the following special conditions apply: The members of the team may pursue other research in addition to their commitment to this project, but each individual researcher must contribute sufficient time to the team award to ensure the achievement of the project s research aims. Collaborators, from outside Denmark, who make a substantial intellectual contribution to the project award may be listed as team members. Specific and justified requests for funding to be used outside of Denmark will be considered; however, it is anticipated that the great majority of the funds will remain in Denmark. The area of research to be pursued via this translational research award must be relevant to one of a number of areas of special interest which have been identified by the Danish Movember Advisory Board in collaboration with other stakeholders in Denmark. These areas are: Imaging MRI and or PET CT or PET MRI in the diagnosis and staging of prostate cancer Genomic and Proteomic Profiling in Prostate Cancer Radiation Therapy New treatment Modalities Integration of cancer registries to improve management of prostate cancer The full application must clearly identify under which of these theme areas the proposed research initiative falls and how the proposed research is relevant to the theme. Potential team leaders are encouraged to put their specific questions, in writing and email it to the Movember contact listed at the end of this document. All questions will be addressed in a timely manner.

ALLOWABLE COSTS INELIGIBLE COSTS COMMUNICATION REQUIREMENTS PERFORMANCE MEASUREMENT 04 ALLOWABLE COSTS The following expenditures will be considered eligible for funding received through this funding opportunity: Research operating costs for the proposed collaborative research project, which must be distinct in its objectives from those for which groups members currently receive funding. Costs of data collection, database management, database integration and maintenance. Costs of regional, national or international networking activities, including collaboration, planning, and knowledge exchange activities. Salaries of research assistants, coordinators, technicians, administrative staff, and other personnel who will enhance the collaborative research productivity of the team. Costs involved in linkage with and dissemination of research findings to those who benefit from the new knowledge generated by the team. INELIGIBLE COSTS Institutional overhead costs Tuition and professional membership dues Capital costs, including costs associated with construction, renovation or rental of offices, laboratories or other supporting facilities. COMMUNICATION REQUIREMENTS Funding recipients are required to acknowledge Movember in all communication or publication related to this funding opportunity. In addition, project funding recipients are also required to adhere to Movember branding requirements as a condition of the project award. The award name must be used in all communication and promotion related to the Movember Award. An award name must be proposed as part of the application. Recognition guidelines including instructions on Movember logo use will be provided to successful applicants as part of the approval package. PERFORMANCE MEASUREMENT Movember is committed to, collecting, evaluating and disseminating information on the outputs and impacts of the research it funds. This information is an important part of Movember s accountability and transparency with its donor community. The team leaders and nominated independent investigator(s) will be required to submit an end of project report 6 months following the end period of the project. The format of the report will be made available to the team leader(s) at the beginning of the project funding period and can be filled in as the research progresses. In addition, the team leader(s) must contribute to the monitoring, review and evaluation of this funding opportunity by participating in requested evaluation studies, surveys, audits, and workshops as required by the funder for the purposes of collecting information to assess progress and results. The team leader(s) will be required to submit to Movember annual written progress reports, as well as a list of all publications resulting in whole or in part from this funding opportunity. The research project team will be required to create a robust evaluation plan, ideally done in combination with a specialist who has extensive experience and credentials in program evaluation

PERFORMANCE MEASUREMENT KNOWLEDGE TRANSLATION STRATEGY REVIEW PROCESS AND EVALUATION CRITERIA 05 (e.g. a certified evaluator) to ensure knowledge transfer and future project sustainability - sufficient budget should be allocated in the application in recognition of the importance of this component. KNOWLEDGE TRANSLATION STRATEGY The project requires a comprehensive knowledge translation strategy that will take the knowledge achieved by the project and use it to advance the projects research aims. The project team should identify potential users of knowledge generated by the project and explain how they will identified and engaged in the translation process in order to optimize the results the project will achieve. REVIEW PROCESS AND EVALUATION CRITERIA An international peer review panel (external to Denmark) will be convened to evaluate the full applications. The panel will be created specifically for this funding opportunity. Panel members will be selected based on their breadth of knowledge and expertise in prostate cancer. Applications will be rated and ranked based on their excellence as judged against the following four evaluation criteria. Because different projects will emphasize different research approaches and knowledge translation strategies, it is understood that reviewers may weight criteria differently from one application to another. Each individual component will be evaluated, as well as the project as a whole. 1. Research Approach 2. Significance/Impact of the Proposal 3. Researchers/Research Team 4. The Research Environment To support the strategic objectives of this funding opportunity, the following factors will be considered: 1. Research Approach a. Coherence of the research project. b. Synergies between investigators is evident c. The soundness and rigor of the science. d. The extent to which the proposed project brings together researchers from different institutions and disciplines in a meaningful way through collaboration. e. The extent to which the health question, problem, or issue is being approached by the project team through an original, integrated, and feasible research plan will generate valid, reliable, and useful knowledge. f. The extent to which the team has a knowledge translation plan for linking with and disseminating research findings to end-user groups who will utilize and apply the results.

REVIEW PROCESS AND EVALUATION CRITERIA HOW TO APPLY: IMPORTANT DATES SUBMISSION GUIDELINES 06 2. Significance/Impact of the Proposal a. The extent to which the proposal is advancing scientific knowledge of prostate cancer. b. The translational aspects of the project are clear and the potential to improve outcomes for prostate cancer patients is evident. c. The research question is important and the results from the project will have broad interest and applicability. d. The originality of the hypotheses or questions being addressed by the project are clear and well formulated. e. The extent to which the project challenges existing paradigms and engages in novel concepts, approaches, methodologies or tools. 3. Researchers/Research Team a. The team leader(s) and supporting team members are appropriately trained and well suited to carry out the work described in the proposal. b. The expertise and experience of the project team leader(s) is complementary and synergistic. c. Senior and junior investigators are engaged as active team members. 4. The Research Environment a. The essential elements required for the provision of a high-quality training environment exists - new staff, students and post-doctoral fellows are present and are likely to be developed during the funding period. b. The degree of institutional support for the project team. c. A quality research environment is evident and it will contribute to the probability of success for the project. Applications that do not comply with these instructions may not be accepted for review. By submitting an application, the applicants accept the Terms and Conditions set out below. Both the notice of intention to apply and the full application must be submitted electronically via Movember RFP system - which can be accessed via https://movember.myreviewroom.com.

SUBMISSION GUIDELINES 07 Hardcopy and emailed notices and applications will not be accepted. Applicants will need to create an account through the RFA system, which will then allow them to complete the Intent to Apply and also submit the Full application. The Full Application can be edited any number of times up until the closing date and time. The Notice of Intent to Apply must be submitted by 5:00 PM CEST on May 10, 2013. The Full Application must be submitted by 5:00 PM CEST on July 31, 2013. Movember is not obliged to consider applications received after the closing time but may do so at its sole discretion. Identification Notice of Intent to Apply due 5:00 PM CEST on May 10, 2013 All applicants are required to submit electronically via the RFA system (https://movember.myreviewroom.com ) by 5:00 PM on May 10, 2013 a notice that an application will be forthcoming. A project title and the priority area must be identified, as well as a list of the team members with their affiliation and expertise. (Note; a final list at this stage is not required and can be adjusted for the full application.) All Notice of Intent submissions are eligible for the Full Application step in the process. Full Application due 5:00 PM CEST on July 31, 2013 On July 31, 2013, by 5:00 PM CEST, Applicants are required to submit project information and supporting documentation through the same RFA system (https://movember.myreviewroom.com). a. A project proposal up to a maximum of Twenty-five (25) pages maximum (excluding table of contents, letters of support, publications, CVs of team members, and letters of collaboration.) The proposal must clearly outline each of the elements below: Project rationale, originality and innovation of the project The research approach The significance and impact of the proposed project. The areas of research (as detailed on page 3) the proposed research initiative falls and how the research proposal is relevant to that area. The Applicant and Team Members- detailing their background and expertise A comprehensive knowledge translation strategy. An evaluation plan. The requested budget, the justification for the budget request and supporting documents. Documentation of existing resources and a description of how the new project funds will complement existing project infrastructure/funding are required. In addition to the project proposal, the following information and documents will need to be included. Publications a maximum of fifteen (15) publications relevant to the proposal may be uploaded. Letters of Support (mandatory - minimum 3) signed letters of support are required from all organizations supporting the proposed project. If applicable, their specific contribution must be clearly indicated in the letter. Letters of Collaboration signed letters of collaboration are required from all collaborators on the project. The necessary required signatures identification of which organization will receive and administer the award, is required.
