IVF Egg Sharing with anonymous recipient COMPLETE PACKAGE S1 1500 An all-inclusive package for women who need IVF or ICSI treatment and choose to share their eggs with an anonymous recipient, offering great value IVF, peace of mind and no hidden costs. Fees include: Medical and nursing consultations Screening tests for sharer and male partner if applicable Stimulation cycle for sharer Semen preparation of partner sperm if applicable Egg retrieval Attempted fertilisation of at least 4 eggs Embryo transfer Pregnancy scan or follow up appointment within 3 months Also included: All drugs for sharer ICSI if required Blastocyst culture of embryos Embryo freezing and storage for one year Frozen embryo transfer if not pregnant from first cycle (to be taken within 6 months) HFEA Fee 80 Fees exclude: Donor sperm if required 300 Embryoscope 700 Cancellation Policy: Sharer withdraws consent to donate after matching, but prior to stimulation 500 refund Cycle cancelled after stimulation due to insufficient response to stimulation 200 refund No eggs collected or failed fertilisation no refund Sharer withdraws consent to donate after stimulation no refund and full cost of IVF/ICSI plus drugs payable A minimum of 8 eggs is required for the donation to take place. This may appeal to women who are either not eligible for NHS funded treatment or who have had an unsuccessful NHS funded cycle. Cancellation Policy Continued: Donor withdraws consent to donate to anonymous recipient after stimulation no refund and full cost of IVF known recipient cycle plus drugs payable A minimum of 8 eggs is required for the donation to the anonymous recipient to take place. If less than 8 eggs are retrieved, the known recipient keeps all the eggs and the donation to the anonymous recipient is cancelled (at no extra cost) Up to six sessions of counselling are included whilst you are receiving treatment at Complete Fertility and for up to six months after your last treatment cycle. Additional sessions are available at a cost of 60 per session. Failure to attend counselling appointment 50. Administrative fees Request of copy of medical records 25 Failure to attend medical consultation (if less than 24 hours notice given) 100 Failure to attend nurse appointment (if less than 24 hours notice given) 50 Complete Fertility reserve the right to take payment from supplied credit or debit card details in the event of non-payment of invoices by the due date. These prices are effective from 16th April 2016 and supersede all previous price lists. Prices are subject to change without notice. Payment is required within 7 days of the invoice date.
