Marriage Registration and Certification: The following documents are required to complete the application:- Documents Required Affidavit of (a) Bride (b) Groom (c) Joint (Bride and Groom) Statement (a) Bride (b) Groom (c) Bride's Father/Mother (d) Groom's Father/Mother (e) Two Local Witnesses (individual, not in blood relation) Proof of Residence (Voter ID Card, Ration Card, Passport, Driving License etc) Passport sized photographs of all six members Date of Births proof of both husband and wife (Metric Certificate or Passport's Photocopy) Marriage Invitation Card of Both Sides Minimum four marriage ceremony photographs preferably of Jaimala, Family & Pandit Five photographs of couple of size 3"X2" (Bride and Groom) Hotel/ banquet hall bill (if marriage took place at Hotel/ Banquet Hall) Original copy of divorce decree/ order in case of a divorcee and death certificate of spouse in case of widow/ widower A caste conversion certificate in case one of the parties belong to other than Hindu, Buddhist, Sikh or Jain In case of marriage of any Indian Citizen with a person having a foreign citizenship, a domicile from the embassy of the country which he/she claims to be the citizen of, is required. Note: 1. It is to be noted that the all the documents to be submitted should be notarized/attested by the Executive Magistrate.
