Welcome to the MEPAP Practicum The MEPAP Practicum is designed to complement the MEPAP classes. 8 EASY STEPS 1. You are expected to complete all of the assignments in each section of the MEPAP course. 2. When you complete the practicum assignments in this pack, you can upload the file in the Practicum Assignment Upload Area on the homepage of the course. 3. You should keep all of your work in a binder for future reference. 4. All that you will need to submit is the form concerning each practicum, initialed and signed by your advisor verifying that you completed it. 5. You will need to complete some of your practicum assignments by posting your information in the course forum. 6. You can upload PowerPoint presentations (or use Microsoft Word or Open Office) in the course forum to share with your fellow students. Keep in mind; you should not add music or animation as this increases the file size of the assignment tremendously. I know that you want to, but please don t in order to avoid complications with uploading. 7. If you don t have PowerPoint, do NOT buy it. You can create a Word Document (using Microsoft Word or Open Office) and insert pictures along with a description of the event or program 8. Each MEPAP Course has two sets of Practicum Packs (Midterm & Final). You have 8 weeks to complete each one. Midterm Practicum Pack is due Week 8 and the Final Practicum Pack is due Week 16. The Practicum must sign off on all of your practicum assignment tasks included in your midterm & final practicum packs as they are completed and be able to meet with you at least once a week to discuss the practicum tasks. Name: Course ID#: ME1- Quality of Life Interview Practicum 1. Interview the Director of Nursing or an equivalent staff person in the nursing Department about the Quality of Life of your residents 2. Here are some sample questions Define Quality of Life for themselves and the residents What staff actions do they consider add to the quality of life? What staff actions take away from the quality of life? Which facility systems, policies and procedures or conditions do they believe have a positive effect on the quality of life? What staff actions constitute resident mental/emotional abuse? Physical abuse? Discuss the results with your advisor Continuum of Care Practicum 1. Research 5 of the Settings Listed a. Compare and contrast the activities that they offer b. Gather calendars from each of the 5 settings 2. Interview the Activity Professional from each of the 5 settings a. Care Planning e. Activities Assessment Due to HIPPA, you are to use yourself as a resident and fill in the MDS3.0 sections mentioned above. Pretend you have MS and you are confined to a wheelchair. You have difficulty speaking and have limited mobility in your arms. Write up the MDS3.0, Care plan and 1 progress note for yourself. At a Care plan meeting discuss Quality of Life with the Care Plan team. Find out what they consider Quality of Life for the residents presented at the meeting. Discuss with your advisor Post a brief summary of your care plan discussion on the Forum Documentation Practicum Alternative Assignment for Assisted Living Facilities **Assisted Living Facility folks can use this practicum instead of the other Documentation Assignment as they do not have to do the MDS3.0** Documentation covers the following areas a. Activities Assessment b. Activity Progress Notes c. Care Planning Pretend you have MS and you are confined to a wheelchair. You have difficulty speaking and have limited mobility in your arms. Write up an initial assessment and a service plan (care plan) on yourself using these restrictions. At a service plan meeting discuss Quality of Life with the Care Plan team. Find out what they consider Quality of Life for the residents presented at the meeting. Name: Course ID#: ME1- Quality of Life Practicum Practitioners should address their own quality of life before they can address the quality of life for others. Quality of Life activities for practitioners might include, but not be limited to, attending a concert or play, educational or learning program, practicing a musical instrument, a home show party, joining a club or beginning an exercise program. The purpose of this activity is to expand your appreciation for your personal quality of life. These activities should address their five-fold development: spiritual, physical, intellectual, emotional and social. Participate in at least 2 Quality of Life events Post a PowerPoint presentation (or use OpenOffice) of at least 10 slides in the course forum to share with your fellow students. Note: If you do not have PowerPoint, use a Word Document and insert the pictures and describe the events When posting describe the event and what you did Sign below after all pages and tasks in this Midterm Practicum Packet have been completed.
