NNOVATION The year was 1997 and the vision was to, well, to survive and make a difference. The first invention was an external fixation system, targeting device, better ESF pins, and better outcomes. Off we went. Then we got into the habit of obsessing over every orthopedic procedure. The question was never how can we improve the product or procedure it was when! During 2001 the groundbreaking Crimping/Tensioning system was introduced for cranial cruciate stabilization. This revolutionized how lateral suture procedures were done and dramatically improved surgical outcomes. Shortly thereafter, there was the break off bone anchors, then a new low profile TPLO plate, evolutionary designs for TTA followed by the first forkless TTA plate and the first cuttable TTA cage, advancement in coxofemoral luxation, hip resurfacing, fracture fixation and synthetic CCL replacement systems. Eight patents, several pending, many innovations and a lot of hard work and dedication, we have moved on. We are now beginning again, with Everost. Focused on next generation materials, products, and, of course, re-obsessing over every orthopedic procedure. The engineers at Everost introduced the very first low profile TPLO plate that allowed for adequate fixation, as well as, easy closure. Tibial Plateau Leveling Osteotomy (TPLO) Tibial Plateau Leveling Osteotomy (TPLO) is one of the greatest advancements for the surgical repair of cruciate deficient stifles, pioneered by the late Dr. Slocum. The founders of Everost have been designing TPLO implants for more than 10 years and hold U.S. patents in the area. With all the TPLO plates available today we challenged our engineers with the task of designing a NEW range of implants that are strong enough to allow for effective bone healing, but aren t too stiff to cause stress shielding. With today s materials, engineers have become complacent in their designs because many times we don t have to worry about the mechanical strength of the implant (when using 316LVM or Titanium), we just have to worry about the design and fit. We are proud to introduce the Freedom Lock and Liberty Lock TPLO plates. These implants are the very first locking implants that provide fixed angle options with poly axial freedom. Not everyone wants the freedom of sending screws at different angles and would prefer the holes to be at fixed angles. We have designed the Freedom Lock and Liberty Lock plates to have the best of both worlds. If one prefers to used them as fixed angle they would simply do so. If you would like to change the angle by 15 degrees in any direction the drill guide is put in at the desired angle. This technology is truly a break through. At the time the sole supplier of TPLO plates was struggling with quality issues. The technique was a very favorable one and the surgical community simply wanted a well designed, dependable implant and our engineers came to the rescue. We were the only company at the time that was able to mass produce a high quality implants. Since than, we have continued to innovate in the area of TPLO. We are now proud to introduce the Freedom Lock TPLO plates. These plates are the first to offer fixed angle locking with the freedom to change the angel if desired. The unique locking mechanism is set at a fixed angle but the angle can be changed by 15 degrees if desired allow for ideal screw placement. The shape of the plate is novel, as well. The cranial nose on the distal part of the plate allows for desirable bone purchase in the center of the tibia. It also offer s an additional angle to apply compression. The Freedom Lock implants are made from a unique Ti-6Al-4V alloy with a proprietary surface treatment that allows it to out perform the standard titanium implants. This combination of the unique Ti Alloy and the proprietary surface finish has been recognized as providing greater strength, biocompatibility and enhanced stiffness. The surface treatment also increases fatigue strength and improves surface characteristics. Freedom Locking Screws Locking Holes, Freedom Lock The novel feature of the Freedom Lock Screws lies in the head. The head has a tapered geometry; double helix threads and the threads have interrupts. The interrupts of the thread allows the screw head to enter into the plate threads at different times, therefore, allowing for 15 degrees of freedom in the locking angle. FreedomLock Fracture Implants The engineers at Everost have been working on locking systems for many years. Our founder was the first to offer a polyaxial system to the veterinary market. Freedom Lock is the latest in locking technology and is an innovative mechanism that allows for 15 of freedom with screw placement. Unlike other systems, the screws do not cut their threads (leaving small chips of metal floating around), but instead have a unique thread profile on the screw head that allows the screw to enter the plate threads at different angles. Internal Fixation Fracture treatment for animals started to take shape in the early 1940 s. Since then, it has been a slow evolving product range for the animal market. For much of that time patient specific sizes and contours simply didn t exist and veterinary surgeons were forced to use inadequate implants from the human side. The engineers at Everost were some of the first to concentrate on designing and manufacturing veterinary specific trauma implants. Manufacturing Everost Cortex Screws are uniquely designed to minimize heat produced during insertion, essentially protecting the patient from developing thermal necrosis. Everost Cortex Screws are more than capable of fulfilling expectations. The minimal damage they afflict to surrounding tissue and bone makes them the preferred choice of Cortex Screws by veterinary surgeons. Bone Plates Features & Benefits: The geometry of the cutting flutes (radius, length and depth) is very important. Here is where you would find variations between a quality screw and poor screw. If the geometry isn t ONE optimal the screw will not be able to efficiently tap the hole during insertion. This will cause excessive heat and thermal necrosis. Everost only offers hex head screws. Some companies offer cruciform and torx head screws; however, Hex historically is the most reliable and universally accepted. All of the Everost screws are AO style and can be used with AO equipment; they are fully compatible. 60

Coxofemoral Luxation Management A common method for the management of coxofemoral luxations is with toggle pins. This technique has been a standard of care for many years. Our engineers were the first to introduce solid toggle rods to be used with both monofilament suture and UHMWPE orthopedic suture, in conjunction with NC State. When the toggle rods were introduced we also provided the essential instrumentation for drilling the tunnels, placing the toggle rod, and retrieving the suture. Today we continue to improve on our original designs and offer new toggle rods that are unique in a few ways, but most importantly the material and material finish. Material Overview for the Xorption TTA Cage & TTA-One From permanent metal implants to fully-resorbable composites The propreitary bioabsorbable composite used for the Everost Xorption TTA cage and TTA-One contains 30% biophasic calcium phosphate dispersed in a bioabsorbably polymer matrix-base predominately on Poly (L, DL-lactide). This composite formulation has been used in implantable devices for over 7 years and has a proven biocompatibility track record particularly in humans. Material Degradation The graph (below) depicts a typical trend curve for molecular weight vs. time and mass loss vs. time for a semi-cystalline polymer. Also, overlaid in the graph is the projected support curve over time. Up until a critical point, the observed mass loss is relatively low, but increases as the material becomes more friable. Support Molecular Weight Mass Loss TIME EuroIntervention 2014; 9:1272-1284 Poly(lacitdes) exhibit bulk degradation via hydrolysis of the ester group. The rate at which the product degrades is governed by the level of crystallinity in the structure. Because the composite formulation used in the Xorption TTA cage and the TTA-One is amphorous and lacks an ordered crystalline structure, a faster more uniform degradation profile would be expected. Preliminary studies project a 12-15% reduction in molecular weight (determined through inherent viscosity) between 0 and 6 weeks, a 31-32% reduction after 12 weeks and an 88-92% reduction after 48 weeks. Products such as the Everost TTA-Track and the Everost TTA Wing Plate, although semi-crystalline, are processed in such a way that the leverl of crystallimity in the final product is low. As a result, it is expected that they would exhibit a slightly slower degradation rate than the Xorption TTA cage and TTA-One. 104

Mechanical Performance The load at failure for the Xorption TTA cage and TTA-One could not be determined due to limitations in the testing equipment. However, it was observed that both products experienced compressive loads greater than 1000N even at 24 weeks agin under accelerated conditions without failure. This loading is comparable to the mean load at failures determined in the study by Balligand et al. Bioabsorption The TTA and TTA-One cages degrade via bulk erosion. During this process, hydrolysis of the ester groups in the polymer chain causes a breakdown of the composit structure. This allows lower molecular weight moieties to be removed from the system via metabolix pathways opening the structure to further breakdown. Some breakdown moieties enter the Krebs cycle and are broken down into CO2 and H20 and expired through the lungs. Other moieties may be excreted by the kidneys in urine or feces.
