With Small Business Online Quoting brokers can produce quotes for groups up to 50 employees, generate rates, view plan descriptions, and print proposals. For groups with over 50 employees, please contact your local UnitedHealthcare Representative to obtain quote information for medical, dental, vision and life products. About the Small Business Online Quoting Home Page The Small Business Online Quoting Home Page offers a variety of options and links to information. It allows you to create new quotes, access your saved quotes, maintain your user profile, and read updates and articles of interest. Menu Bar A menu bar is displayed across the top of every page. On the Small Business Online Quoting Home Page, links on the purple menu bar allow you to create a new quote (New Quote), access your saved quotes (Saved Quotes), and review proposal policies and assumptions (Proposal Policies). Click these options at any time to navigate through the system. News and Rate Alerts This section provides a gateway to new and important rate, product, and web site information including articles of interest. Quick Start The Home Page includes a Quick Start section that allows you to perform several functions: Click New Quote to begin the creation of a new quote. Click View Your Saved Quotes to access the Saved Quotes page. This page lists all your saved quotes. You can use this page to view quote details, complete an unfinished quote, copy a quote to create a new quote, and delete a quote. Click Manage My Account to edit your user profile, change your password, add or delete a customized logo or view your license and appointment information. Click Download Census Template to download an Excel template for importing census data. 1 of 5
Find Saved Quotes The Find Saved Quotes section of the Home Page allows you to search your saved quotes list. You can apply search criteria to view a filtered list of quotes. Use one or more of the following fields to specify your search criteria and then click Search. Field Name Company Name Contains Status Multi-site Quote Quote Tracking Number Effective Date From/To Creation Date From/To Description The company name or a part of the company name for the quote or quotes you want to view. The status (All, Proposed, In Progress, Submitted, Case Initiated, Application Received) of the quote or quotes you want to view. The quote is Single-site, Multi-site or N/A. The unique quote tracking number you want to search on. The date on which coverage begins. Enter a range of effective dates for the quote or quotes you want to view. The date when the quote was created. Enter a range of creation dates for the quote or quotes you want to view. Create a Quote To begin the quote process for groups with up to 50 employees: 1. Click Quote from the primary menu bar of the United eservices Home Page. 2. The Small Business Online Quoting Home Page is displayed. Click New Quote from the Quote menu bar or from the Quick Start menu. 2 of 5

Enter all required information and any other additional information in the Quote Information, Coverage Information, Company Information, Contact Information, Location Information, and Group Information sections including the total number of employees, the number of eligible employees, and the number of Non-COBRA and COBRA employees applying at location for coverage. 4. Click Next to continue with the quote process. 5. The Census page is displayed. Enter employee information. 6. Click Next to continue with the quote process. 7. After entering your census data, Small Business Online Quoting lists the plans that are available for your location and group size. The plan selection pages allow you to view the most popular plans or all available plans, compare selected plans and compare rates. For some pages, you can filter by premium amount. 8. From the Medical Plans page, select the medical plan(s) you want to include in your proposal and click Next to continue with the quote process. 9. From the Dental Plans page, select the dental plan(s) you want to include in your proposal and click Next to continue with the quote process. 10. From the Visions Plans page, select the vision plan(s) you want to include in your proposal to include in your proposal and click Next to continue with the quote process. 11. From the Life Plans page, select the life plan(s) you want to include in your proposal and click Next to continue with the quote process. 12. The Proposal Information page is displayed. You are now ready to generate your proposal. Select the content you want to include in your proposal and if desired, change the order of the selected plans. 13. Click Generate Final Proposal. 14. The Proposal Output Confirmation page is displayed. From this page, you can view your proposal, send it to your printer, save it to an electronic file or export rates and plans to an electronic file. 15. This is the end of the quote process. The employer location with more than 50% of the applying employees. If no location has more than 50% of the applying employees, then this is the company s primary business location. Information about each individual applying for coverage, such as employee name, age, date of birth, etc. View detailed rate information for each group of medical, dental or vision plans selected and compare rate information. View detailed information for each medical, dental or vision plan selected and compare plan benefits. The date when the quote was created. The date on which the quote became effective. A Multi-site employer in which all employer locations are located within the same state. For intrastate employers the quote may be completed as a Singlesite quote. An employer with more than one business location located in more than one market where employees perform their work. A single policy solution for a Multi-site employer. Rx plan is a prescription drug plan that can be selected for a specific medical plan. Standard Industrial Classification Code Small Business Online Quoting is an online, realtime quoting tool that is used for new business preliminary quotes. The Definity HRA with a Split Deductible HRA differs from Standard HRAs in that the employer HRA funds are not accessed until an individual and/or their family has met a specified portion of their deductible we call this the Access Point. Two HRA Access Points will be derived for each policy - one for subscribers with individual coverage, and one for subscribers with family coverage. The Definity HRA consists of an employer-funded account that is used to offset initial member liabilities, including copays, and an annual deductible that can be set somewhat higher than in a traditional plan. It makes use of funds provided by the employer that are used by the member to offset qualified medical expenses.
