GROUP MEDICARE SUPPLEMENT PLAN OPTIONS SENIOR GOLD Group Senior GoldSM provides a basic level of health coverage. Choose all four additional options to build the most comprehensive plan. By selecting all four options, members have no deductibles, or coinsurance for Medicare-eligible services and supplies, and they re protected against catastrophic expenses. Option 1: Covers Medicare Part A inpatient hospital deductible Option 2: Covers Medicare Part B annual deductible Option 3: Covers 100 percent of eligible medical expenses and supplies that exceed Medicare-approved charges Preventive: Covers hearing and vision screenings and other medically necessary routine screenings not covered by Original Medicare PLAN PLAN L K Group Plan L Enhanced is ideal if you want to offer a Medicare supplement plan with a lower monthly premium in exchange for retirees paying a greater share of their health care expenses. Group Plan L Enhanced pays 75 percent of most hospital and medical expenses that remain after Original Medicare coverage is applied. The plan includes catastrophic protection with an annual out-of-pocket maximum. Group Plan K Enhanced Our lowest-cost Medicare supplement plan, Group Plan K Enhanced includes the same features as Group Plan L Enhanced and pays 50 percent of most hospital and medical expenses that remain after Original Medicare coverage is applied. Earn up to a $20 credit toward your monthly health club fees when you exercise at a participating fitness center at least 12 times per month. Weight Watchers discounts. Complementary care discounts. NAVIGATING MEDICARE. Original Medicare is a health insurance program for people age 65 and older and people with certain disabilities, including permanent kidney failure. Established in 1965, Medicare is administered by the Centers for Medicare & Medicaid Services (CMS), an agency of the U.S. federal government. Original Medicare has two parts: Medicare Part A is hospital insurance that helps pay for inpatient care in hospitals and skilled nursing facilities. Medicare Part B is medical insurance that helps pay for doctor visits, physicians services, certain preventive services and screenings, lab tests, durable medical equipment and outpatient hospital treatment. Medicare does not cover all of your retirees health care expenses. There are deductibles, and coinsurance that your retirees still have to pay. A Group Medicare supplement plan provides coverage that can help reduce or eliminate these out-of-pocket costs for your retirees. ADD PRESCRIPTION DRUG COVERAGE FOR A COMPREHENSIVE HEALTH CARE PACKAGE Group Senior Gold can be paired with Group MedicareBlueSM Rx (PDP), a stand-alone prescription drug plan with a Medicare contract. Your retirees can live anywhere in the United States and have their prescriptions filled at more than 64,000 participating pharmacies. 3
