POLICE PERFORMANCE REVIEW FORM INSTRUCTIONS How to Process the Form On-Line 1. To Access the Form: In Public Folders; click HRD ; HRD Forms, filename is Police Performance Review Form.doc ; Double Click on the file name to open it 2. Complete each field by using your TAB key to move from section to section. When you have completed the EPR, save it to your directory by clicking File, Save As, enter File name using LastName, FirstInitial, ReviewDate. Example SmithJ061703. (NOTE: We may want to work with Bonnie Plane to set up a specific directory for Performance reviews to be saved in.) 3. Routing for Comments: Supervisor will then FORWARD the document to the Watch Commander who will complete the comments section; click on the SAVE icon and then FORWARD to Division Manager who will complete the comments section and FORWARD to the Employee. The employee will enter his or her final comments, click on the SAVE icon and FORWARD the document to the Supervisor who initiated the review form. Saving the Final & Complete EPR and Printing the Form Supervisor: You will need to replace your original saved document (prior to routing) with the now completed form, which will include all of the commanding staff comments. Click on File, then Save As, locate and click on the original file name, and click on Save. Print the form (double-sided); obtain appropriate signatures and forward original to Human Resources. Other Routing Instructions When sending a Performance Review Form via email, always identify the mail in the Subject field as: Performance Review: (Last Name) Mark the email with an Urgent (!) symbol so recipients will be alerted to open first. Mark your email for you to be notified when the recipient reads the item so you know it has been received and read.
