Benefits Benefits avaiabe under this poicy are set out in the tabe, and in the event of a caim are payabe to the poicyhoder. It s important to read the information in this tabe aongside the definitions, benefit terms, conditions, excusions and other documents forming the poicy. Benefits Amount payabe / imits Fu benefit We wi pay fu benefit if, immediatey before the start of incapacity you were not unempoyed and after the start of incapacity you are not foowing any other occupation and the deferred period has finished. This wi be the amount stated in your poicy schedue, subject to the maximum annua amount payabe. Restricted benefit We wi pay restricted benefit if, immediatey before incapacity, you were unempoyed and after the start of incapacity you are not foowing any other occupation, and the deferred period has finished. This wi be the amount stated in your poicy schedue. We wi pay benefit for a maximum of 12 months. After you have received benefit for a tota maximum of 12 months your poicy wi end. Back to work benefit We wi pay back to work benefit if, before incapacity, you were not unempoyed and: you now foow a different occupation because of your incapacity; or you return to your occupation but the iness or injury that caused incapacity restricts the scope of your duties, and in either case, your earnings are reduced. To cacuate the back to work benefit, we wi: work out the percentage your earnings have reduced by, and pay you that percentage of the fu benefit. For exampe if your earnings are reduced by 70% you wi receive 70% of the fu benefit. If you are caiming because the iness or injury that caused incapacity restricts the scope of your duties, your incapacity must: ast at east seven consecutive days, and restrict the scope of your duties for the remainder of the deferred period. Hospita benefit We wi pay hospita benefit if, during your deferred period, you have spent at east seven consecutive nights as a patient in hospita because of your incapacity. We wi pay you 75 per night for each night you spend in hospita, up to a maximum of 90 nights. The nighty amount of 75 appies as a tota imit to this and any other poicies in pace with us, for exampe, private medica insurance or a cash pan. aviva.co.uk/heath 3
Benefits Amount payabe / imits Death benefit We wi pay death benefit if: you die due to the condition for which benefit is being paid; and your death happens within 12 months of your first monthy benefit being due. A singe payment of up to 12 times the ast monthy benefit paid, ess any benefit aready paid. Retirement benefit We wi pay retirement benefit if: you have received monthy benefit for incapacity for 12 consecutive months; and we stop paying benefit because you have reached your poicy termination age. A singe payment of three times the ast monthy benefit paid. 4 Income Protection Soutions
Benefit Terms 1 Entitement to benefit We wi pay benefit for incapacity after the deferred period has finished. We wi pay benefit for incapacity monthy in arrears or when your incapacity ends if earier. We wi pay a daiy benefit amount for any periods of incapacity which ast ess than one month. 2 When payment of benefit stops We wi pay fu benefit, restricted benefit or back to work benefit unti: your incapacity ends or in the case of back to work benefit, you no onger satisfy the requirements for payment of benefit; or you reach the poicy termination age; or you reach the end of a imited payment term; or you reach the end of the restricted benefit payment term, or you are remanded in custody (the benefit wi be retrospectivey paid if you are not convicted of the offence) or receive a custodia sentence; or you die, whichever happens first. 3 Maximum annua amount payabe a. The maximum amount of benefit we wi pay is: 60% of your earnings up to the first 25,000; pus 50% of your earnings above 25,000, up to a maximum annua amount of 180,000 uness changes due to the index inking option take benefits above this amount. If you are entited to any income from other sources, we wi deduct these amounts from the maximum annua amount payabe. Where any of this income is taxabe, we wi reduce the gross amount by 20% for the purpose of this cacuation. Income from other sources incudes: continuing income from a business (incuding dividends) continuing income from an empoyer (incuding benefits in kind) income from a pension which is first paid after incapacity any income you receive because of your iness or injury (not incuding State Benefit) reguar income under other insurance poicies (except waiver of premium), whether hed with us or not, for exampe; Income Protection / Permanent Heath Insurance Mortgage Payment Protection Credit card protection Loan protection Exampe: Mr Smith earns 25,000 per year. He is covered for 1,250 per month after a 13 week deferred period. In the event of ong term sickness, his empoyer pays him fu pay for 13 weeks. After 13 weeks he receives haf his norma pay for the next 13 weeks ( 1,042). This wi be Mr Smith s ony continuing income during sickness. We wi reduce the gross amount of 1,042 by 20% as it is subject to tax. So, we wi take into account 834. We then take his maximum monthy benefit of 1,250 and deduct 834, to give him a maximum monthy benefit of 416. After 26 weeks Mr Smith s empoyer stops paying him, so we wi pay him the fu monthy benefit amount of 1,250. b. If we do reduce your benefit because it exceeds the maximum annua amount payabe, we wi not refund any premiums. 4 Waiver of premium Whist we are paying you benefit for incapacity, we wi not charge you a premium. 5 Index-inking If the poicyhoder has chosen the index-inking option, we wi review the benefit at the anniversary date in ine with the retai price index pubished three months before the anniversary date. The poicyhoder wi pay a revised premium for any increase in benefit which we wi cacuate based on whichever option they have chosen, either the: guaranteed option; the premium rates which appied at the start date of the poicy using your age next birthday on the anniversary date; or reviewabe option; our current premium rates for your age next birthday on the anniversary date. aviva.co.uk/heath 5
6 Sick Pay Soutions for NHS Doctors and Surgeons If you are a Doctor or Surgeon empoyed directy by the Nationa Heath Service (NHS) in the United Kingdom and registered with the Genera Medica Counci, and at the date of incapacity: You have been empoyed in that roe for ess than five years, and; Your poicy has been in force for ess than five years, and; You have a singe deferred period of 26 or 52 weeks or a dua deferred period of 26 and 52 weeks, and; Your sick pay entitement from the NHS is ess than fu pay for six months foowed by haf pay for six months. We wi reduce the deferred period, subject to a minimum of 4 weeks, for benefit payabe in respect of your work with the NHS, taking into account your sick pay entitement from the NHS when your incapacity starts. Sick Pay Soutions DOES NOT appy for: Income you receive from private practice; and Any benefit you are eigibe for under Aviva s Temporary Accident Cover. Benefit term 3 Maximum annua amount payabe wi appy and wi determine the date and amount of benefit payabe. Exampe: Dr Smith earns 40,000 per year as an NHS Doctor. He is covered for 541 per month after a 26 week deferred period, pus an additiona 1,334 per month after a 52 week deferred period. Dr Smith has been working as an NHS Doctor for 2.5 years when he is incapacitated and submits a vaid caim. In the event of ong term sickness, after 2.5 years the NHS pays him fu pay for 2 months, foowed by haf pay for 2 months and after 4 months the NHS stops paying him. Dr Smith woud receive benefit from us of 541 per month after a 2 month deferred period, and after 4 months we wi pay him the fu monthy benefit amount of 1,875. 7 Medica supervision We wi ony pay benefit if you are under the reguar supervision and treatment of a medica practitioner for your incapacity. If your incapacity is in reation to drug or acoho addiction you must aso be foowing an agreed rehabiitation treatment pan/ programme prescribed by a medica practitioner. 6 Income Protection Soutions
8 Back to work benefit When we work out the percentage your earnings have reduced by, we wi take into account any change in the retai price index between the start of incapacity and the date when back to work benefit becomes payabe. We wi not reduce back to work benefit due to increases in earnings during the period in which we are paying benefit, provided those increases are not higher than the change in the retai price index. Where we refer to the retai price index, this means the retai price index as pubished three months before the anniversary date. 9 Linked caims We wi not re-appy the deferred period where: you have received benefit; and you suffer from the same cause of incapacity within 12 months of your ast monthy benefit. 10 Linked periods of incapacity We wi ink periods of incapacity if you have a deferred period of 13 weeks or more. Separate periods of incapacity (from the same cause) may be inked provided that: each period of incapacity asts at east one whoe week (7 consecutive days); and the deferred period is competed within a period of time twice its ength (deferred period x 2). Periods of incapacity which ast for ess than one whoe week (7 consecutive days) cannot be combined for the purposes of competing the deferred period. Exampe: Mr Smith has a poicy with a 13 week deferred period. If he suffers the same iness for a combined tota of 13 whoe weeks within a 26 week period (deferred period x 2) he wi be abe to caim. Scenario: He suffers from an iness which eaves him off work for three whoe weeks. He returns to work but suffers from the same iness four weeks ater, this time for a period of seven whoe weeks. He returns to work again for one week and is unfortunatey then off sick again with the same iness for a further three whoe weeks. 11 Extra cover at ife events (guaranteed insurabiity) At certain ife events, the poicyhoder can buy extra cover to better suit your circumstances. This extra cover wi not be subject to medica underwriting, minimum eves of premium, or poicy charges. The poicyhoder cannot buy extra cover if you have been subject to incapacity after the start date, uness: the incapacity did not resut in us paying benefit; and the incapacity ended at east three months before the date on which the poicyhoder wishes to buy extra cover. The poicyhoder can buy extra cover if: you have been accepted on standard terms (with no additiona premiums or excusions); and you have been covered by the poicy for at east 12 months; and you are not within five years of the poicy termination age. Extra cover must start within 90 days of one of the events shown in the tabe: Event You increase your share of the mortgage payment, due to; a home move or improvement a mortgage discount period ending, or becoming soe owner of the property foowing the termination of your marriage or civi partnership You get married or form a civi partnership You become a parent You receive a significant saary increase (at east 20%) as a direct and immediate resut of obtaining a professiona quaification Benefit Limit Up to the eve of the increase in payment Up to 250 per month Up to 250 per month Up to 500 per month There are some conditions which we appy to extra cover. These are: Extra cover cannot incude a deferred period that is shorter than your origina deferred period The poicy termination age is subject to the maximum poicy termination age we wi aow for your occupation at the time the poicyhoder buys extra cover, and can be no more than 5 years higher than your origina termination age We wi cance any extra cover if your poicy ends for any reason other than reaching the poicy termination age 12 Career break If you take a career break and you submit a caim for incapacity, payment of benefit wi be under Restricted Benefit. Pease refer to Benefits on Page 3. 13 Overseas residence If you ive in or trave within: European Union USA Canada New Zeaand Austraia but outside: United Kingdom Channe Isands Ise of Man we wi imit benefit payments to a maximum combined tota of three years throughout the period of incapacity. If you ive in or trave to any part of the word, other than: European Union USA United Kingdom Canada Channe Isands New Zeaand Ise of Man Austraia we wi imit benefit payments to a maximum combined tota of six months throughout the period of incapacity. If you return to iving within the United Kingdom, Channe Isands or Ise of Man and you sti satisfy the definition of incapacity, we wi cacuate the amount of benefit payabe to you in accordance with the terms of this poicy. We wi not backdate the payment of any benefit that we did not pay whist you were iving outside the United Kingdom, Channe Isands or Ise of Man. The combined tota monthy benefit, incuding extra cover, must not be more than the maximum annua amount payabe. To buy extra cover the poicyhoder must contact us and give us the information we need. This woud incude either: a copy of the confirmation etter from the ender confirming the new mortgage arrangements, or the marriage or civi partnership certificate, or the chid s birth certificate, or if the chid is adopted a copy of the adoption papers, or a copy of the exam resuts and a copy of the pay rise etter from the empoyer 8 Income Protection Soutions
Conditions 1 Compiance with poicy terms Our iabiity under this poicy wi rey upon you and the poicyhoder compying with its terms and conditions. 2 Change of termination age The poicyhoder must te us if you pan to retire before the poicy termination age. If you do retire before the poicy termination age, we can either ower the termination age or cance this poicy. If we ower the termination age we wi reduce the premium. If we cance the poicy, we wi give the poicyhoder 30 days notice sent to their ast known address. 3 Change of circumstances a. We have used the answers to the questions we asked as part of the appication to determine your insured amount and any specia terms which may appy to your poicy (e.g. excusions, oading etc). If an answer to any of these questions changes before the date on which your cover begins (as notified to you) you must te us before the poicy commences. If, based on the new answer, our origina decision to offer cover (or the insured amount or any specia terms appied) woud have been different we sha have the right to change the terms of our offer to those we woud have offered based upon the new information. If we woud not have offered cover we may withdraw cover. b. The poicyhoder shoud review your insured amount with your financia adviser if: you become unempoyed, your earnings reduce, you reduce your working hours or choose to stop working, you take out any other insurance for iness or injury, or your continuing income from an empoyer or business changes. c. If your cover is not reviewed due to any of these changes, we wi not backdate any refunds if you choose to reduce your cover at a ater date. 4 Premiums foowing a change of termination age under condition 2, at a premium review under condition 6, or foowing a change of circumstances under condition 3. c. A premiums are payabe in advance of any cover being provided. 5 Canceation a. We wi cance this poicy if premiums are not paid. However, we wi reinstate cover if the premium is then paid within 14 days of its due date. b. We may at any time, in ine with reasonabe underwriting and insurance practice (and retrospectivey where appropriate), change the poicy terms or cance cover without paying any refunds, provided it is not unfair to do so, if you or the poicyhoder has: mised us, for exampe teing us incorrect information or not teing us something that we have asked for, (either in the appication or afterwards); or defrauded or attempted to defraud us, or agreed to any attempt by someone ese to defraud us, or not acted openy and honesty in your deaings with us. c. If we cance the poicy for reasons other than those in points (a) and (b), the poicyhoder may be entited to a refund of premium. 6 Premium review If you have seected the reviewabe option this means that we may change the premium to refect updated views on the expected cost of providing cover. We can ony change the premium on the fifth poicy anniversary date and every anniversary after that during the term of your poicy. The premium may go up, down, or remain unchanged as a resut of the review, and there is no imit to the size of the change. When reviewing premiums we wi consider the future impact of: the cost of a Aviva income protection poicyhoders caims poicy apses a. The reviews wi compare the costs aowed for in the premium with the updated view on future costs. We wi not ook to recoup past osses or redistribute past profits as part of a review, and the premium wi not be directy affected by whether you have caimed or not. We wi give the poicyhoder at east 30 days notice of any change as a resut of such a review. If we advise the poicyhoder of an increase in premium, they can then choose to keep the premium unchanged and reduce the eve of benefit accordingy. 7 Third party interest Benefit cannot be made the subject of any charge, security interest, assignment, transfer or trust. We wi not recognise or accept notice of any third party caim or interest in respect of any benefit. 8 Law This poicy is governed by Engish aw, and wi be subject to the excusive jurisdiction of the courts of Engand and Waes. If we decide to waive any term or condition of this poicy, this does not mean that we have made a permanent change to the terms and conditions. We can sti appy the term or condition that we waived on a future or separate occasion. This poicy is a contract between us and the poicyhoder. No other persons wi have any right under the Contracts (Rights of Third Parties) Act 1999 to enforce this poicy or any part of it. This wi not affect any third party right or remedy if avaiabe apart from that Act. 9 Consents You, and where appicabe the poicyhoder on your behaf, agrees and consents to the computer and other processing and use of a persona and medica detais received by us. This incudes processing by the data controers and reevant third parties (which may incude the poicyhoder, medica and other service providers and reevant intermediaries) for the purpose of: poicy administration service provision reinsurance caims vaidation; and fraud prevention. The poicyhoder aso agrees to provide and obtain confirmations of consent we require. Processing may be in any part of the word. Processing wi be carried out in such a manner that ensures adequate standards of data protection within the meaning of Engish aw is adhered to. Caims Conditions 1 Caims procedure You can ony caim if premiums are up to date. If you are sef-empoyed we need at east: You or the poicyhoder must te us about any caim as soon as possibe, either: before the period of incapacity has asted two months; or before the deferred period ends if it is shorter than two months. If you or the poicyhoder do not te us within these time imits, we may postpone or refuse a caim (where, due to the deay in notification, we are unabe to confirm incapacity. If we have to postpone a caim, benefit payments wi be deayed. 2 Our requirements You or the poicyhoder must compete and return caim forms and any other documentation requested by us, as soon as reasonaby possibe. We may have to postpone or refuse a caim if we do not receive the information that we need to assess the caim. Apart from medica information or medica evidence requested by us, you wi pay for a certificates, information and evidence that we ask for (incuding proof of age) to assess a caim. We wi not be abe to process any caim uness you sign the consent forms provided by us, to aow us access to your reevant medica records hed by a medica practitioner. We wi aso need evidence of earnings. evidence of persona earnings, during the 12 months (or 36 months for benefit based on an annua average over 3 years) before incapacity, as assessed for income tax purposes and decared to and agreed by Her Majesty s Revenue and Customs; and the profit and oss accounts which reate to this. We may need more information. If we do, we wi ask for it. You need to provide a information that we ask for, and: undergo medica examinations or tests (on our behaf and at our expense) by a medica examiner appointed by us in respect of any aeged incapacity; and sign any necessary consent forms to aow us access to the resuts of any reevant medica examinations and/or tests. You must take reasonabe steps to assist your recovery and hep us assist your recovery, incuding: meeting and working with disabiity counseors and/or advisers appointed for our purpose; and signing any necessary consent forms. 3 Payments A payments wi be made in the currency of the United Kingdom on the due date uness we have postponed your caim as detaied above. If you are empoyed we need at east: the ast 12 months paysips immediatey before incapacity or the ast P60 certificate; and A PAYE Coding Notice (or repacement) wi be required if benefits in kind are covered. If you are a Director we aso need evidence of your dividends, during the 12 months before incapacity, as assessed for income tax purposes and decared to and agreed by Her Majesty s Revenue and Customs and the profit and oss accounts which reate to this. 4 Our rights You or the poicyhoder must te us as soon as possibe if you are making a caim against anyone who may be responsibe for your incapacity that aso resuted in a caim under this poicy. For exampe, a road traffic accident that was not your faut which has caused incapacity. You must continue to keep us fuy informed in writing about such third party caims and take a steps we require when making your caim against that third party. In reation to the third party caim, we wi be entited to take proceedings in your name in respect of any caim for oss or damages or otherwise which caused us to pay any benefits or costs under this poicy. in the conduct of any such proceedings and in the settement of any resuting caim. Aternativey, we may aow you, your awyer, or anyone acting on your behaf to conduct these proceedings. In either case, we wi be entited to recover any benefit paid or payabe to you from the proceeds of any third party caim. We wi have no responsibiity for any caim for any other osses that are not reated to this poicy. For these caims you shoud ensure that independent ega advice is taken. We require direct access to anyone appointed on your behaf in respect of your third party caim. If you don t accept the recommendation of the person appointed to act on your behaf to accept a reasonabe offer or payment to sette a third party caim, we may refuse to pay further benefit. 5 Frauduent and unfounded caims If any caim under this poicy is in any respect frauduent or unfounded, then without affecting any other ega rights we may have, any benefit paid for that caim wi be recaimed, and no further benefit wi be paid. We wi aso have the right to cance the poicy. You wi te us immediatey if anyone offers to sette your third party caim or makes a payment to sette your third party caim. You must not agree to sette or withdraw the third party caim without our prior approva. 12 Income Protection Soutions

If you decide to cance your poicy, any money you have aready paid wi be refunded, provided no caims have been made in this 30 day period. If you wish to cance your poicy pease compete and return the canceation form incuded in your wecome pack. If we do not hear from you we wi continue to coect your premium in accordance with instructions previousy provided by you and continue to give you the cover as detaied in this poicy wording. The financia services compensation scheme (FSCS) The FSCS covers your poicy. It cover you if Aviva becomes insovent and we are unabe to meet our obigations under the poicy. For this type of poicy, the FSCS wi cover you for 90% of the tota amount of an existing caim. The FSCS wi aso provide a refund of 90% of the premiums that have not been used to pay for cover whether you are making a caim under the poicy or not. Definitions Throughout this booket certain words are shown in bod type. These are defined terms and have specific meanings when used in this guide. The meanings of these words are set out beow. Anniversary date The annua anniversary of the start date. Appication The appication that you provided and that we have accepted. Benefit The benefit described under the benefits section of this poicy wording. Deferred period The number of consecutive weeks of incapacity which must pass before the poicyhoder becomes entited to receive benefit. Duties The materia and substantia duties required for the performance of your occupation which cannot reasonaby be omitted or modified. Duties do not incude: Duties which are not necessary to perform the occupation within the trade profession (eg duties that are not necessary with another empoyer or within another business) or; The commute to and from your pace(s) of work. Earnings Your empoyed saary or your sef-empoyed earnings. Empoyed saary Your earnings, saary or wage (incuding benefits in kind) before tax from your empoyment during the period of 12 months before the start of incapacity. Benefits in kind The foowing benefits in kind to a combined taxabe tota of up to 10,000 wi be accepted as forming part of empoyed saary: Company Car Living Accommodation Private Medica Insurance Dividends If your income from your trade or business is received in the form of company dividends or distributions, we wi incude this amount as earnings under this poicy providing; 1. The dividends or distributions are paid direct to you instead of reguar wages or saary in the period of 12 months before the start of incapacity; and 2. The dividends or distributions are consistent with a eve of reguar wages or saary that the paying company s trading position aows on a continuing basis. Sef-empoyed earnings Your income from your business before tax, during the period of 12 months before the start of incapacity, ess any aowabe expenses against income tax. (This means your share of pre-tax profits from your insured occupation). Incapacity Incapacity means that you are unabe to perform on a fu or part-time basis the duties of your occupation, because of your iness or accidenta injury. Limited payment term If seected, benefit payments wi be restricted to a tota maximum of five years for a periods of incapacity, whatever their cause. After benefit has been paid for a tota maximum of five years the poicy wi end. Exampe: Mr Jones makes a caim for benefit in January 2008. He continues to caim benefit unti January 2011, and has therefore caimed benefit for three years. Mr Jones then makes a further caim in January 2012, and as he has aready caimed benefit for three years, if he sti meets the definition of incapacity, his benefit payments wi stop in January 2014 as he wi have caimed for a tota of five years. Occupation Occupation means each and every occupation you were foowing in the 12 months immediatey before the start of incapacity. If immediatey before the start of your incapacity you were unempoyed, occupation means the ast occupation you performed before the start of incapacity. Poicy Our contract of insurance with the poicyhoder providing the cover as agreed. The appication, poicy schedue and financia statement form part of the contract and must be read together with this poicy wording. Poicyhoder The person or company named as poicyhoder in the poicy schedue. Retai price index The monthy index used by the Government that demonstrates the movement of retai prices in the UK, or an equivaent repacement of that index which is acceptabe to us. Unempoyed Unempoyed means you were not foowing an occupation for profit or reward immediatey prior to incapacity. This poicy wi not pay benefit because you become unempoyed. Benefit is dependant on satisfying the definition of incapacity. If you are incapacitated during a period of unempoyment your poicy wi ony pay Restricted benefit (see Benefits section). We wi continue cover after unempoyment if premiums continue to be paid, but it wi be on restricted benefit (pease see the benefits section).
