ADDENDUM No. 3 May 25, 2016 GREENWICH PUBLIC SHCOOLS 1 WESTERN JR. HIGHWAY, OPENING DATE: JUNE 2, 2016 OPENING TIME: 9:00AM BID NUMBER: 2076-16 The following ADDENDUM is issued to clarify items appearing either in the specifications or on the Drawings, said items having been called to the attention of the Architect by one or more of the Contractors estimating the work: The Specifications and / or drawings are amended as follows: 1. SPECIFICATIONS SECTION 00 04 00 BID FORM A. Refer to attached revised Bid Form dated, Revised: May 25, 2016. Add alternates have been revised. 2. RESPONSES TO MECHANICAL & ELECTRICAL RFI S A. Are any of the mechanical units in the building larger than 15,000 CFM? Response: Take-off of devices for fan shutdown and duct detectors associated with air movers, regardless of CFM rate, to be as shown on drawings. See E-000 for project allowances in the event additional devices are needed. B. Is a State elevator inspection required for the new elevator recall devices? Response: Services of elevator service company for testing and inspections by authorities having jurisdiction for acceptance are to be included in bid. It is not anticipated that State approval is required, however if required at the request of the local fire marshal or building official, costs for such inspections must be included. All project requirements, except for those that are specifically added or modified by this Addendum, shall remain in full effect. Eugene H. Watts

SECTION 000400 BID FORM The undersigned hereby proposes to furnish all labor, materials, devices, appliances, supplies, equipment, services and other facilities necessary to complete all of the work of the above referenced Contract, as required by, and in accordance with, the provisions of the Instructions to Bidders, the Conditions of the Contract, the Drawings and Specifications, all as prepared by Antinozzi Associates, P.C., and dated April 22, 2016; and that, if this Proposal is accepted, the Undersigned agrees to enter into an Agreement with the Owner to perform this work for the sum(s) as follows: SUBMITTED BY: Bidder s Full Name Address City, State, Zip 1.1 BASE BID: A. ADD ALTERNATE #2: For all costs associated with upgrading the existing Fire Alarm System as specified on the drawings. Costs for Add Alternate #2 shall not include escalation costs, in the event that the fire alarm work will be completed during the 2016 summer break along with the Public Address System. Note: The base bid shall only include costs associated with the Public Address system which must be completed during the 2016 summer break. ADD ALTERNATE #3: For all costs associated with upgrading the existing Fire Alarm System as specified on the drawings. Costs for Add Alternate #3 shall include any overtime costs, if applicable, in the event that the fire alarm work will begin during the 2016 summer break and continue on into the 2016/2017 school year with work being performed after normal school hours following the summer break. Note: The base bid shall only include costs associated with the Public Address system which must be completed during the 2016 summer break. 1.2 ACCEPTANCE: A. If this bid is accepted by Greenwich Public Schools within the time period stated above, we will: 1. Execute the Agreement within seven days of receipt of Notice of Award. 2. Furnish the required bonds within seven days of receipt of Notice of Award. B. If this bid is accepted within the time stated, and we fail to commence the Work or we fail to provide the required Bond(s), the security deposit shall be forfeited as damages to Greenwich Public Schools by reason of our failure, limited in amount to the lesser of the face value of the security deposit or the difference between this bid and the bid upon which a Contract is signed. 1.3 CONTRACT TIME: A. That no member of the Greenwich Public Schools or any officer or employee of the Greenwich Public School or person whose salary is payable in whole or in part from the School District treasury, or the spouse of any foregoing is or shall be or become interested, directly or indirectly, as a contracting party, partner, stockholder, surety or otherwise, in this bid, or in the performance of the Contract, or in the supplies, materials or equipment and work or labor to which it relates, or in any portion of the profits thereof. 4. That he/she has carefully examined the site of the work and that, from his / her own investigations, he/ she has satisfied him/ herself as to the nature and location of the work, and character, quality and quantity of material, and all difficulties likely to be encountered, the kind and extent of equipment and other facilities needed for the performance of the work, the general and local conditions, and all other items which may, in any way, effect the work or its performance. 5. That if a corporation, this bid or proposal containing the Non-Collusive Binding Certification and the foregoing Affirmation and Declaration has been authorized by the Board of Directors of such Corporation, which authorization includes the signing and submission of this bid or proposal and the inclusion therein of the said Certificate of Non-Collusion and Affirmation and Declaration as the Act and Dees of the Corporation.
