APPLICATIONS FOR EMPLOYMENT This is the FIRST STEP in the Interview Process. Please read the information below to make sure you meet the minimum qualifications for employment with MARC: Community Resources and to assist you in the application process. MARC: Community Resources, Ltd. welcomes your interest in employment at our agency. MARC continuously recruits for a variety of full-time, part-time and per-diem positions providing direct care services to people with developmental and intellectual disabilities. These positions may include morning, afternoon, evening, overnight, weekdays and weekend shifts. Prior to filling out application: You must have a valid Connecticut driver license in good standing and with a good driving record to be considered for employment. An essential function of all positions is a Public Service License (PSL) endorsement. You will be provided with information from DMV regarding the qualifications for a PSL. Please review these qualifications to make sure you will qualify prior to completing an application. You must have a high school diploma or equivalent to be eligible for employment. Filling out the application: Please thoroughly complete the application for employment providing all the information requested (even if you attach a resume). Incomplete applications may eliminate you from consideration. A list of job openings will provided to you when you fill out the application. GENERAL DESCRIPTION OF POSITION: The Community Living Assistant will follow agency and house specific plans that are designed to promote social and emotional growth in the residents we serve and foster opportunities for independence and individuality. ESSENTIAL FUNCTIONS OF THE JOB: Adhere to all DDS and MARC policies and procedures. Administer medication, CPR, and First Aid. Attend staff meetings as scheduled. Complete required in-services deemed necessary by the agency. Comply with HIPAA regulations. Document house specific and consumer related information in accordance with state and agency regulations. Drive van as needed. Ensure MARC's mission is understood and implemented. Ensure the highest standards of health sanitation, safety, and dignity for all consumers.

Facilitate inclusion per MARC's missions. Follow all OSHA requirements. Follow individual dietary plans of each consumer. GENERAL DESCRIPTION OF POSITION: The Community Support Specialist will follow agency policies and participate in related goals and plans, which promote inclusion as well as social and emotional growth for participants. The CSS will facilitate community-based activities, including employment, and assist in creating opportunities for inclusion. The CSS shall also implement MARC s mission to the fullest extent possible. ESSENTIAL FUNCTIONS OF THE JOB: Provide supervision to assigned group of individuals with development disabilities. Assist in securing, processing and maintaining community/employment sites within the community. Assign, and set up activities and instruct, and supervise participants in activities of daily living, work skills, social skills, etc. emphasizing achievement of greater independence. Provide transportation to and from sites utilizing own vehicle(s) and/or arrange or teach transportation skills utilizing public transportation, private companies, etc. The employee may be assigned daily driving duties in a company vehicle. The employee will also be required to follow the job duties as described for a Van Driver (see job description of Van Driver for further information). Complete Medication Administration Training and Certification and administer medication at the discretion of the Day Program Manager. Ensure MARC s mission is implemented in the course of the employee s daily activities. Act as a liaison between community/work sites and MARC, including facilitating

positive interactions between program participants and community members. Provide assistance to community/work site personnel as may be required to promote inclusion. Responsible for meeting standards set forth by community site personnel/employers, including quality control measures, safety rules, production standards, policies/ procedures, etc. Maintain neat and accurate comprehensive written evaluation records. Responsible for payroll for assigned workers, ensuring compliance with Federal and State wage and hour rules and regulations. Develop and implement individual program plans for participants, analyze data collection, complete progress reports, etc. within prescribed time lines. Prepare reports for assigned participants and participate in the OPS process. Immediately report all incidents to Supervisor and complete written incident report forms with attached documentation as may be required. Comply with all DDS and MARC policies, including policy addressing abuse and neglect. Consult with and inform supervisor of any pertinent participant and/or community site issues. Maintain strict confidentiality regarding information on all participants and other pertinent information. Attend and participate in all meetings, in-services, and other agency related events as required. Keep informed and implement current trends and methods of working with person with disabilities. Ensure the highest standards of health, sanitation, safety, and dignity for all participants. Follow all OSHA regulations. GENERAL DESCRIPTION OF POSITION: The Supported Living Facilitator provides direct support services to persons who are part of the Supported Living Program and live in their own homes. Ability to assist participants to reach a higher level of independence, self-sufficiency and to take control of their own lives is of the highest priority. Personal integrity, honesty, dependability and the ability to grasp the fundamentals of promoting inclusion are also of prime importance. ESSENTIAL FUNCTIONS OF THE JOB: Adhere to all of DDS's and MARC's policies and procedures. Assist consumer in organizing their personal schedule and encourage involvement in community activities. Assist in budgeting and paying bills, including but not limited to rent, utilities, groceries and credit cards. Assist individuals in coordinating employment, family and personal concerns and problem resolution. Attend staff meetings and in services as scheduled. Comply with all OSHA regulations. Comply with HIPAA regulations.

Drive consumers or instruct them on other methods of transportation such as bus or taxi; transportation could occur in agency or individual's personal vehicle. Ensure MARC's mission is understood and implemented. Explain and demonstrate by lesson and example, various daily living skills including but not limited to cooking, laundry, housekeeping, phone skills, travel skills, shopping and interpersonal relationships.
