Alpine School District Annual Request For Statements of Interest and Qualifications for Architectural Services (SOIQ) March 2011

As per Administrative Utah Codes R33-5-510 thru R33-5-650, and the Alpine School District Procurement Rules 5-201 thru 5-217, the Alpine School District is requesting Statements of Interest from Architectural Firms for projects that may be funded and begin this year. Scope of Work: Projects may include new Elementary Schools, new Middle Schools, new High Schools, classroom additions to any existing schools, other additions such as but not limited to gymnasiums, lunchrooms, offices, and design work for any other District owned properties. Proposed Work: Design work will include the following: Preliminary/Schematic Design Cost Estimates (Please note that a firm selected to do the preliminary design and cost estimates may not be the firm selected to design and over see the complete project) Design Development Construction Documents Design Process will include the following: Educational Program Design Development Submit forms, plans and specifications to appropriate agencies for review and approval as required by law Coordination with the Physical Facilities and/or school or department Value engineering sessions Other aspects as needed Construction Administration: During construction, a weekly meeting will be held at the site. The architect will coordinate the meeting, take minutes, resolve issues, provide clarification, drawings, issue change directives/into change orders, etc. The architect will be expected to visit the site a minimum of once per week and his consultants a minimum of once per month or as needed whichever is greater. Final inspections and substantial completion will be part of this section. One Year Inspection: The architect will be required to visit the site one year after substantial completion with the inspector from the Physical Facilities Department to review warranty issues. A warranty punch list must be issued and follow-up visits as required. Reports: Weekly reports will be issued to the contractor and the Alpine School District, including weekly minutes, and weekly site observations. Architect s consultant s reports also shall be submitted in a timely manner.

Occasional reports to the Board of Education will be required to keep the public informed on the projects progress. Architect s Professional Liability Insurance: The architect shall maintain General Liability, and Errors and Omissions insurance with a minimum amount of $1,000,000. The Insurance Company must have a rating of A- or better with a financial size category of Class VIII or larger. All said ratings and financial size categories shall be as published by A.M. Best Company at the time this Architects agreement is executed. Additional Insurance: The architect shall purchase and maintain insurance that will protect them from claims under workmen s compensation act. And other insurance as needed to protect against employee benefits claims for damages because of bodily injury, including death, and from claims for damages to property which may arise out of or result from the architect s operations under an Alpine School District contract, whether such operations be by himself or by anyone directly or indirectly employed by any of them. This insurance shall be written for not less than any limits of liability specified as part of a contract that may be agreed upon, or required by law, whichever is greater, and shall exclude liability insurance as applicable to the architect s professional obligation. Your SOIQ must include the following information to be considered: A. The name of the firm and the location of all of its offices, specifically indicating the principal place of business; B. The age of the firm and its average number of employees over the past five years; C. The education, training, and qualifications of members of the firm and key employees; D. The experience of the firm reflecting technical capabilities and project experience; E. Proof of insurance; F. The names of five clients that may be contacted, including at least two for whom services were rendered in the last year; G. Copy of current license certifying that the firm is licensed to conduct business in the State of Utah; H. Financial statements of the firm (this will be kept confidential and only used by the selection committee).

Evaluation of SOIQ: Evaluation of the SOIQ will be made for each individual project to select the firm best suited for that project, and based on the following criteria. Responsiveness: The completeness and conformity of the proposal response to the SOIQ requirements. 10 points Organization: The administrative and functional structure including personnel, respondent s ability to perform, the number of personnel committed to each type of project, and the description of the proposers local facilities, and the capability to meet schedules. 25 points Experience/Expertise: This includes the technical qualifications and work experience of key personnel assigned to work with the District. This also includes past experience on school construction type projects for clients other than the Alpine School District. 35 points References: This refers to the references given in letter E above. 10 points Past Performance: This refers to past performance as it relates to the Alpine School District. 20 points Selection of the Most Qualified Firms: These selections shall be made pursuant to Administrative Utah Code R33-5-610. Written Agreement: The firm selected for a particular project must enter into a written contract (Standard Form of Agreement Between Owner and Architect AIA form # B141-1997) with the Alpine School District to provide all services required by this SOIQ. Additional legal information It will be required of all architectural firms, their employees, and consultants to comply with State of Utah Code 63G-11-103 concerning identity documents and verification and compliance with status verification. This code can be found on the web at: http://le.utah.gov/~code/title63g/htm/63g11_010300.htm It will be required of all architectural firms, their employees, and consultants to comply with controlled substance rules found in State of Utah Rule R33-10 which can be found on the web at: http://www.rules.utah.gov/publicat/bulletin/2010/20100601/33656.htm Smoking is prohibited on and around all Alpine School District properties.

Alpine School District School Board Policy 4457 states: 1.1 School Authorities are charged with protecting the health and safety of all students. As a means of helping to fulfill this charge, Alpine School District requires prospective employees to have a criminal background check. 1.5 Applicants who have been CONVICTED of any felony shall not be hired by Alpine School District. Applicants who have been convicted of any job related misdemeanor shall be reviewed and may not be hired by Alpine School District. It will be required of all architectural firms, their employees, and consultants to comply with this policy by submitting necessary information on all employees working on the project. District representatives will perform a Name Check through the Bureau of Criminal Identification, a division of the Utah Department of Public Safety, for all employees. The employees must pass the background check to be eligible to work on projects on any Alpine District Property occupied by students. Background checks may not be older than three years to be considered current and valid. A fee of $20.00 per employee name submitted for Name Check must be submitted at the time names are submitted to be checked.
