(French Overseas Departments and Regions):French Guyana, Guadeloupe, Martinique and Reunion Island. Excess: sum for which you are responsible in the settlement of a claim. Hospitalisation: stay of more than 24 hours (with or without surgery) in a public or private hospital as a result of illness or accident. Prior agreement: certain medical treatments and procedures require the prior agreement of our Medical Examiner. The practitioner prescribing these treatments or procedures must provide you with a request for prior agreement and a detailed breakdown of costs before commencing any treatment. Waiting period: period defined in the policy during which no benefits are paid. The waiting period applies from the effective date of cover as mentioned on the membership certificate. 6

How to claim reimbursement of your medical expenses in France When you take out the insurance, we will provide you with a third party payment card. With this card, you will have nothing to pay up front to over 100,000 healthcare professionals (pharmacies, laboratories, specialists, etc.). However, if you do pay the healthcare professional directly, you should claim your reimbursement as follows: > > > > See a healthcare professional Send your medical bills to Social Security Your reimbursement is based on French Social Security rates. The amount of the lump sum is doubled if the death is caused by an accident. In addition, the full lump sum is paid in the case of total and irreversible loss of autonomy see definition. Our benefits top up those provided by French Social Security. If you are not entitled to reimbursement from your statutory scheme, we can cover you from the 1 st euro. DEFINITION Total and irreversible loss of autonomy: where the insured is totally and permanently medically unfit for gainful employment and requires the assistance of a third party to carry out basic daily tasks. 14

4 Income protection A daily income protection benefit or disability pension protects you against loss of earnings as a result of illness or accident. Selecting this cover ensures that you will continue to receive part of your salary for a fixed period. You can take out this cover only if you are already covered for death benefit under the policy. You are free to choose the level of daily benefit (between 20 and 200) on condition that the following rules are followed: the monthly equivalent of the daily benefit must not exceed 70% of your gross monthly income, the amount of daily benefit selected depends on the level of death benefit selected: for example, for a daily benefit of 20, the death benefit selected must be at least 20,000. Our benefits top up those provided by French Social Security. If you are not entitled to reimbursement from your statutory scheme, we can cover you from the 1 st euro. In this case, you will need to provide us with proof that you are by your statutory scheme in France. You must be in paid employment to benefit from income protection cover. Daily income protection benefit: Daily income protection benefit may be paid from the 31 st or the 61 st day depending on the option selected and for a maximum period of 3 years. The number of days paid per month is 30. By selecting this cover, you will be entitled to waiver of premium benefit: this means that, if you experience financial difficulties caused by your absence from work and are entitled to income protection benefit, you will receive free death and disability cover. This benefit will stop when you reach the age of 65. Disability pension: A life-time disability pension provides protection if you are disabled through illness. The daily benefit is converted to an annual pension after a maximum of 3 years' payment of the daily benefit. The pension is paid once your condition has stabilised and until you reach retirement age, 65 at the latest. The amount of the annual pension is in proportion to the actual degree of disability. How the policy operates Useful information before you apply WHO CAN BE INSURED? Any person aged between 0 and 75 inclusive, residing in Metropolitan France or in the D.R.O.M. (and outside their country of nationality) and covered by a French Social Security statutory scheme (general scheme, self-employed scheme, special scheme or agricultural scheme). Applications for repatriation and legal assistance are accepted up to and including age 70 and up to age 64 for death and loss of autonomy and income protection benefits. Notes: Membership can be: - on an individual, couple or family basis for medical cover, - on an individual or family basis for repatriation and legal assistance, - on an individual basis for death and total and irreversible loss of autonomy and income protection benefits. The attached Health questionnaire does not need to be filled out if you are only applying for repatriation and legal assistance. From the age of 65, new applicants will be required to attend a mandatory medical visit at their own expense and complete a medical report which is available on request from APRIL International. WHERE AM I COVERED? For medical expenses, death, total and irreversible loss of autonomy and income protection benefits: Cover is valid for a year at a time in France and in the D.R.O.M. as well as during short stays of less than 90 days per insurance year in the rest of the world (including your country of nationality). For repatriation and legal assistance: Cover is valid for a year at a time in France and in the D.R.O.M. as well as during short stays of less than 90 consecutive days in the rest of the world (including your country of nationality).
