Income Protection Options Poicy Conditions Introduction These poicy conditions are written confirmation of your contract with Aviva Life & Pensions UK Limited. It is important that you read them carefuy together with your poicy schedue and then keep both documents in a safe pace. If you have any questions, you can ca us on 0800 285 1098 (from outside of the UK, pease ca +44 (0) 1603 603 479). The words shown in bod type may be defined terms; we expain these in the Definitions section. Your cover In order for cover to be maintained, you need to pay your premiums. Your premiums are guaranteed, so they stay the same throughout the poicy term uness you change your poicy or you have taken out the increasing cover option. Your benefits The benefits avaiabe under this poicy are set out beow, and are ony payabe if we accept a caim. Benefits Fu benefit We wi pay this if: immediatey before incapacity, the ife covered was in an occupation; and after the start of incapacity, the ife covered is not in any other occupation; and the deferred period has ended. Restricted benefit We pay this if, immediatey before incapacity, the ife covered was not working and, after the start of incapacity, the ife covered is not foowing an occupation and the deferred period has ended. Back to work benefit We pay this if, immediatey before incapacity, the ife covered was in an occupation and foowing incapacity their earnings are reduced because they: are in a different occupation because of continued incapacity; or return to their occupation but the iness or accidenta injury that caused incapacity restricts the scope of their duties or the number of hours that they are abe to work. What we pay / imits The benefit amount, subject to the maximum yeary amount payabe. For more information, pease read the 'Benefit imit' section. We pay the benefit amount for a maximum of 12 months, and then the poicy wi end. We pay a percentage of the benefit amount. To cacuate the percentage, we work out the percentage reduction in the ife covered s earnings. When doing this, we take any increase in the Retai Prices Index between the start of incapacity and the date when back to work benefit becomes payabe into account. We won t reduce the amount we pay if the ife covered s earnings increase as ong as the increase isn t more than the change in the Retai Prices Index. If the iness or accidenta injury that caused incapacity restricts the scope of the ife covered s duties, their iness or accidenta injury must: ast at east seven consecutive days; and restrict the scope of their duties for the remainder of the deferred period. Hospita benefit We pay this for each night the ife covered is hospitaised due to incapacity during the deferred period. We start paying it after the seventh consecutive night, provided the ife covered has spent at east seven consecutive nights as a patient in hospita. Death benefit We pay this if: the ife covered dies due to the iness or accidenta injury we re paying benefit for; and death occurs within 12 months of the first monthy benefit being due. Retirement benefit We pay this if: we have paid fu benefit, restricted benefit or back to work benefit for at east 12 consecutive months; and we stop paying benefit because the poicy has reached the poicy end date. Waiver of premium If we accept your caim, we pay your premiums after the deferred period ends. 75 a night for a maximum of 90 nights. A singe payment of 12 times the ast monthy payment we made, ess the amount we ve aready paid. A singe payment of three times the ast monthy payment we made. You need to pay your premiums unti we accept your caim and your deferred period ends. If your premium was due before your deferred period ended, you sti need to pay it. After that we pay your premiums for you. Benefit imit The maximum amount we pay each year is 55% of the ife covered s earnings. For the increasing cover option, this may increase on each anniversary date after we ve started making payments to you. If the ife covered is entited to income from other sources we deduct it from the maximum yeary amount we pay. If any of this income is taxabe, we reduce the gross amount by 20%. Income from other sources incudes: continuing income from a business, incuding dividends continuing income from an empoyer, incuding benefits in kind and sick pay income from a pension which is first paid after incapacity any income received because of the ife covered s iness or accidenta injury (not incuding state benefit) reguar income or benefits from other insurance poicies (whether hed with us or not) which exceeds 50 per month, incuding: credit card protection oan protection income protection/permanent heath insurance mortgage payment protection pension premium protection. If we reduce the benefit because it exceeds the maximum yeary amount, we won t refund any premiums. When payment of benefit starts We pay fu benefit, restricted benefit or back to work benefit after the deferred period has ended. We pay monthy in arrears. If the ife covered s incapacity ends part way through a month, we pay a daiy benefit amount for that period of incapacity. When payment of benefit stops We stop paying fu benefit, restricted benefit or back to work benefit when: page 2 of 12

the incapacity ends or, in the case of back to work benefit, the ife covered no onger satisfies the requirements for payment of benefit; or the poicy reaches the poicy end date; or the imited payment term ends; or we ve paid restricted benefit for a tota maximum of 12 months; or the ife covered is remanded in custody (we pay the benefit retrospectivey if the ife covered is not convicted of the offence) or receives a custodia sentence; or the ife covered reaches their panned retirement age; or the ife covered dies. Other requirements for the payment of benefits We ony pay benefit if we accept a caim and the ife covered is under the reguar supervision and treatment of a medica practitioner. If the incapacity is in reation to drug or acoho addiction, the ife covered must aso be foowing an agreed rehabiitation treatment pan/programme prescribed by a medica practitioner. Recurring incapacity Linked caims We won t re-appy the deferred period if we ve paid benefit and the ife covered suffers from the same cause of incapacity within 12 months of the ast monthy payment. Linked periods of incapacity If you have a deferred period of 13 weeks or more, we ink separate periods of incapacity for the purpose of competing the deferred period as ong as: each period of incapacity has the same cause; and each period of incapacity asts at east seven consecutive days; and the deferred period is competed within a period of time twice its ength (deferred period x 2). Increasing cover option The increasing cover option can ony be seected when you take out your poicy. It is subject to our acceptance foowing underwriting and may increase your premium. Your poicy schedue wi show whether the increasing cover option is incuded on your poicy. Increasing cover What it means for your poicy You can automaticay increase your benefit amount each year without answering any further medica questions. The increase wi be based on the percentage increase in the Retai Price Index (RPI) over the 12 month period ending 12 weeks before the month of your poicy anniversary date. The maximum increase wi be 10%. Your premiums wi increase each year. We take the current premium and mutipy it by a factor of 1.5 and the rate of increase. We base the factor on the rate of increase and your poicy term, and we won t change it during your poicy term. Your premium won t increase by more than the factor mutipied by the maximum increase in RPI of 10%. If the change in the RPI is 0% or beow, your benefit amount and your premium wi stay the same. We write to you at east eight weeks before the anniversary date to te you how much your benefit amount and premiums wi increase by. You can choose not to increase your benefit amount if you don t want to pay the higher premium. If you do this, your benefit amount, and your premiums, wi stay the same. You must te us as soon as possibe before the anniversary date if you want us to cance the increase. We reinstate the increasing cover option the foowing year. If we accept a caim, we pay the benefit amount. page 3 of 12

Additiona benefits Your poicy schedue wi show which additiona benefits are incuded on your poicy. Life change benefit Can be used six months from the start date. If your circumstances change, you can take out extra cover without answering further medica questions. This additiona benefit wi ony be incuded if we accepted your poicy on standard terms. Life change Marriage or civi partnership. Increased mortgage due to a house move or purchase or carrying out home improvements. Having or adopting a chid. 20% increase in saary due to change of empoyer or promotion. Divorce, dissoution of civi partnership or separation. Evidence needed Marriage or civi partnership certificate. Evidence of new mortgage or increase on existing mortgage, or buiders receipts for work carried out. Birth or adoption certificate. Copy of paysips, dated within 90 days of each other. Decree absoute or dissoution order, evidence of new mortgage, mortgage transfer or new separate addresses. The imit for each ife change event is up to 20% of the origina benefit amount. This is subject to a minimum of 100 per month and the maximum yeary amount appicabe at the time that extra cover is taken out. You can t take out extra cover if you ve been incapacitated during the poicy term uness the incapacity: didn t resut in us paying benefit; and ended at east three months before you want to take out extra cover. You can use the ife change benefit as many times as you ike as ong as: you take out the new poicy before the ife covered turns 55; and the new poicy ends before the ife covered turns 71; and you take out the new poicy within 90 days of the ife change happening; and you send us the evidence we need; and you are not within five years of the origina poicy end date. The extra cover wi be subject to the foowing conditions: The new poicy can t have a shorter deferred period than the origina poicy. The new poicy can t end more than five years ater than the origina poicy. For imited payment term poicies, the new poicy can t have a onger payment term than the origina poicy. We cance any extra cover if your poicy ends for any other reason than reaching the poicy end date. The combined tota benefit amount, incuding extra cover, must not be more than the maximum yeary amount. The new poicy won t be subject to any minimum premium imits and can t incude additiona options which weren t incuded on the origina poicy. Aso, the new poicy won t incude the ife change benefit. The premium you pay for any new poicy wi be based on the rates avaiabe at the time of the request and the persona circumstances of the ife covered. Sick Pay Soutions for NHS doctors and surgeons If the ife covered is a doctor or surgeon empoyed by NHS in the UK and registered with the Genera Medica Counci, we reduce the deferred period, subject to a minimum of 4 weeks. This means we start paying benefit to cover their NHS earnings when their sick pay from the NHS reduces or stops. At the date of incapacity: you must have a singe deferred period of 26 or 52 weeks or a dua deferred period of 26 and 52 weeks; and the ife covered s sick pay entitement from the NHS must be ess than fu pay for six months foowed by haf pay for six months. Sick Pay Soutions won t cover: income the ife covered receives from private practice; or any benefit the ife covered is eigibe for under Aviva s Protection Promise. The maximum yeary amount wi appy. Making changes to your poicy Apart from where stated, you can make certain changes to your poicy six months from the start date. If you ask to make any changes, they wi appy from the date your next premium is due. If you make any of the foowing changes, we amend your poicy without asking you any further medica questions: Decreasing: the benefit amount (change avaiabe in the first six months) the poicy term Increasing: the deferred period (change avaiabe in the first six months) Removing: the increasing cover option the shortest deferred period from a poicy with dua deferred periods so you wait onger before we pay benefit. Amending: your cover by seecting a dua deferred period so the ength of time before we pay benefit is increased. If you remove the increasing cover option, your premium wi stay the same for the remainder of the term, uness you make any other changes to your poicy. For a other changes, we use the premium rates and smoker status avaiabe at the time that you took out your origina poicy, based on the age of the ife covered at the start of the poicy. After you ve made any of the above changes, your premium can t be ower than the minimum premium imit which appies at the time we agree to your request. These poicy conditions wi continue to appy to your amended poicy. If you make any of the foowing changes we amend your poicy. We may need to ask you some further medica questions. Depending on your answers, we may not be abe to carry out the change: Increasing: the poicy term Decreasing: the deferred period Amending: your cover by seecting a dua deferred period so we pay your benefit sooner page 5 of 12

Removing: the ongest deferred period from a poicy with dua deferred periods so we pay your benefit sooner Changing: the ife covered s occupation After you ve made any of the above changes, your premium can t be ower than the minimum premium imit which appies at the time we agree to your request. If you want to change the ife covered s occupation, we use the premium rates and smoker status avaiabe at the time you took out your origina poicy, based on the age of the ife covered at the start of the poicy. For a other changes, we use the premium rates and smoker status avaiabe when we make the change, based on the current age of the ife covered. These poicy conditions wi continue to appy to your amended poicy. If you want to increase the benefit amount (change avaiabe in the first six months), we issue a new poicy for the additiona benefit amount. We may need to ask you some further medica questions. Depending on your answers, we may not be abe to carry out the change: The minimum increase you can make is 100 per month. The tota monthy benefit amount wi be subject to the maximum yeary amount which appies when we agree to your request. If we increase the benefit amount, we use the premium rates and smoker status avaiabe when we issue the new poicy, based on the current age of the ife covered. The new poicy won t be subject to any minimum premium imits. The poicy conditions in force at the time wi appy to the new poicy. Making a caim If you need to make a caim, pease contact us on 0800 158 3105. Lines are open Mon to Fri 8.30am 5.00pm. You need to te us you want to make a caim before the: period of incapacity has asted two months; or deferred period ends if it s shorter than two months. If you or the ife covered don t te us within these time imits, we may postpone or refuse a caim. This is because we may not be abe to confirm the incapacity. Before we can pay a caim, we need to assess it. To do this, we ask for some important information, consents and documents. Apart from any medica evidence we ask for, you have to pay for the information. The kind of things we need may incude, but aren t imited to, the foowing: A caim form signed by you and the ife covered. Proof that the ife covered is i or has been accidentay injured. Proof of who egay owns the poicy. Written consent that ets us: access the medica records or reports of the ife covered receive the resuts of any medica examinations or tests of the ife covered appy for evidence and information from other third parties. Conversations with, and reports from, third parties such as medica practitioners, empoyers and coroners. Evidence of earnings: If the ife covered is empoyed we need: - the ast 12 months paysips immediatey before incapacity; or; - the ast P60 certificate; and - A PAYE coding notice (or repacement) if benefits in kind are covered. page 6 of 12

If the ife covered is a director, we aso need: - evidence of their dividends during the 12 months immediatey before incapacity, as assessed for income tax and decared to/agreed by HMRC; and - the profit and oss accounts. If the ife covered is sef-empoyed we need: - evidence of persona earnings during the 12 months immediatey before incapacity, as assessed for income tax and decared to/agreed by HMRC; and - the profit and oss accounts which reate to their persona earnings. We may postpone or refuse a caim if we don t receive the information, consents or documents we need in a reasonabe time. The ife covered must take whatever steps are necessary to hep their recovery, incuding meeting, and working, with disabiity counseors and/or advisers appointed by us. If we need the ife covered to undergo any medica examinations or tests, we pay for them. We aso appoint the medica examiner. When we assess a caim, we rey on the information we re given. If any of the information is untrue or incompete it coud affect whether we pay a caim or not, and may mean we won t pay a caim. Or, if we ve aready paid a caim it may mean we can recaim the money. If this happens, we won t make any further payments, and we may aso cance the poicy without refunding any premiums. This doesn t affect any other ega rights we have. If we accept a caim, we make any reevant payment to you in the currency of the UK. Paying a caim if the ife covered is overseas If you caim whist the ife covered is traveing or iving in the foowing countries (but outside of the UK, the Channe Isands, the Ise of Man and Gibratar), we pay the benefit for a maximum tota of three years: - Andorra - New Zeaand - Austraia - Norway - Canada - San Marino - European Union - Switzerand - The Faroe Isands - USA - Liechtenstein - The Vatican City - Monaco If the ife covered traves to, or ives in, any part of the word other than the foowing, we pay the benefit for a maximum tota of six months: - Andorra - Monaco - Austraia - New Zeaand - Canada - Norway - Channe Isands - San Marino - European Union - Switzerand - The Faroe Isands - UK - Gibratar - USA - Ise of Man - The Vatican City - Liechtenstein If the ife covered returns to ive in the UK, Channe Isands, Ise of Man or Gibratar and they are sti incapacitated, we carry on paying the benefit to you. We won t backdate any benefit we didn t pay when the ife covered was iving outside the UK, Channe Isands, Ise of Man or Gibratar. Caiming against a third party If you re making a caim against a third party who may be responsibe for the incapacity which resuted in a caim on this poicy, you must te us as soon as possibe. For exampe, a road traffic accident that was not your faut. You must keep us fuy informed in writing about the third party caim. You must aso take a steps we require when making the caim. We re entited to take ega proceedings in your name for any caim on this poicy which caused us to pay any benefits or costs. We make a decisions on the conduct, and settement, of the proceedings. Or, we aow you, your awyer, or anyone acting on your behaf to conduct these proceedings. In either case, we be entited to recover any benefit amount from the money you receive from the third party caim. page 7 of 12

When you take out this poicy, you consent to us to contacting and instructing anyone deaing with the third party caim on your behaf. You shoud te us immediatey if anyone offers to sette your third party caim, or makes a payment into court. You mustn t agree to sette or withdraw the caim without our approva. If you don t accept the recommendation of the person acting on your behaf to accept a reasonabe offer or payment into court as settement, we may refuse to pay further benefit. We won t be responsibe for any caim for any other osses that aren t reated to this poicy. For these caims you need to take independent ega advice. Eigibiity You must be at east 18 to appy for this poicy and no oder than 59 when we accept the appication. You must be a resident of the UK, Channe Isands, Ise of Man or Gibratar, and be empoyed or sef empoyed, either on a fu or part time basis to be eigibe for this poicy. Paying your premiums You must pay your premiums monthy by Direct Debit. A Direct Debits need to come from a current bank or buiding society account in the UK, Channe Isands, Ise of Man or Gibratar, in the currency of the UK. We show the initia premium you pay, and the date the initia premium and subsequent premiums are due, in the poicy schedue. You have 60 days from each due date to pay your premium. If you have to make a caim during this period, we deduct the unpaid premium from the benefit amount we pay. If you don t pay your premiums by the due date, or within the 60 day period, we cance your poicy. If this happens, you won t be abe to make a caim. What to do if circumstances change We use the information you give us in your appication to determine the cover we can offer and how much you wi pay for the poicy. If any of the information changes before your poicy starts, you must te us. If, based on the new answer, our origina decision woud have been different we have the right to change the terms of, or withdraw our offer of cover. You shoud review the eve of cover if the ife covered: becomes unempoyed; reduces their working hours or chooses to stop working; changes their panned retirement date; takes out any other insurance for iness or injury; has a change made to continuing income from an empoyer or business; or has a reduction in earnings. If you don t review your cover, we won t backdate any refunds if you choose to reduce your cover at a ater date. What to do if retirement pans change You must te us as soon as possibe if the ife covered pans to retire before the poicy end date. If they are, we may ower the poicy end date or cance your poicy. If we ower the poicy end date, your premium wi be ower. If we cance the poicy, we give you 30 days notice. Changing your detais You need to et us know if your contact detais, or those of any ife covered, change. page 8 of 12

Acceptance of instructions We can t accept any instruction, request or notice from you unti we receive a the information we need. We te you what kind of information or documentation we need. Canceing your poicy You have a 30 day cooing off period to change your mind. If you cance within this period, we refund any premiums you ve paid. The cooing off period begins on the ater of: the day we give you the poicy start date the day you receive your poicy documents. You can sti cance the poicy after the cooing off period ends, but we won t refund your premiums. If you cance your poicy, you won t be abe to make a caim. Genera Conditions Poicy amendments We may ater these poicy conditions for any of the foowing reasons: To respond, in a proportionate manner, to changes in: the way we administer poicies of this type technoogy or genera practice in the ife and pensions industry taxation, the aw or interpretation of the aw, decisions or recommendations of an ombudsman, reguator or simiar body, or any code of practice with which we intend to compy. To correct errors, if it is fair and reasonabe to do so. If we think any ateration to these poicy conditions is to your advantage, we make it immediatey and te you at a ater date. We aso do this if the ateration is due to reguatory requirements. If any ateration is to your disadvantage, we aim to te you in writing at east 60 days before we make it. However, externa factors beyond our contro may mean we have to give you ess notice. If you re not happy with any ateration we make to your poicy, you can cance it. Incorrect information If the date of birth of a ife covered is wrong, we base the payment we make for any successfu caim on the correct date of birth. We te you if this happens. If, using the correct date of birth, the age of any ife covered when you took out your poicy woud have been outside our imits, we cance your poicy. If this happens, we te you. You won t be abe to make a caim after we ve canceed your poicy. However, we refund a your premiums (without interest). We rey on the information you give us. If any of it is untrue or incompete and woud have affected our decision to provide your poicy, we may: change the terms of the poicy change the premiums you have to pay cance your poicy and refund the premiums you ve paid (without interest). If we cance your poicy, you won t be abe to make a caim. not acted openy and honesty in deaings with us unreasonaby refused to compy with these poicy conditions. Third party rights This poicy does not give any rights to anyone except you and us. We may, with your agreement, amend or cance this poicy without reference to, or the consent of, any other person. Genera You may not transfer ( assign ) the poicy to someone ese. The poicy schedue records the poicy s detais at the start date. Any subsequent changes wi be recorded and confirmed to you separatey and shoud be kept in a safe pace. Definitions Throughout these poicy conditions we have highighted defined terms in bod type (except for persona terms ike we and you ) so you know when they appy. The meanings of these words are set out beow. You or your refers to the poicyhoder(s) named in the poicy schedue, or anyone ese who becomes the ega owner of the poicy. We, us or our means Aviva Life & Pensions UK Limited. Anniversary date The anniversary of the start date shown in the poicy schedue. Benefit The benefits described in the Your benefits section of these poicy conditions. Benefit amount The amount payabe under this poicy in the event of a caim which we accept. The benefit amount is set out in your poicy schedue and is subject to the maximum yeary amount payabe. Deferred period The number of consecutive weeks of incapacity which must pass before you become entited to receive the benefit. The deferred period(s) is shown in your poicy schedue. Duties The materia and substantia duties that are normay required for, and form a significant and integra part of, the performance of the ife covered s occupation that cannot be reasonaby omitted or modified. Duties do not incude: Duties which aren t necessary to perform the occupation within the trade profession (eg duties that are not necessary with another empoyer or within another business) or; The commute to and from your pace(s) of work. page 10 of 12

Earnings The ife covered s empoyed saary or their sef-empoyed earnings. Empoyed saary The ife covered s earnings, saary or wage (incuding benefits in kind) before tax from their empoyment in the 12 months immediatey before incapacity. Benefits in kind The foowing benefits in kind to a combined taxabe tota of up to 10,000 wi be accepted as forming part of empoyed saary: Company car Living accommodation Private medica insurance Dividends If income from the ife covered s trade or business is received in the form of company dividends or distributions, we incude this amount as earnings under this poicy as ong as they are: 1. paid directy to the ife covered instead of reguar wages or saary in the 12 months immediatey before incapacity; and 2. consistent with a eve of reguar wages or saary that the paying company s trading position reasonaby aows on a continuing basis. Sef-empoyed earnings The ife covered s income from their business before tax, in the 12 months immediatey before incapacity, ess any aowabe expenses against income tax. (This means their share of pre-tax profits from their insured occupation). End date The date shown in the poicy schedue on which benefits under this poicy end. Incapacity Incapacity means the ife covered can t perform, on a fu or part time basis, the duties of every occupation they re foowing in the 12 months immediatey before incapacity, because of iness or accidenta injury. Occupations that formed ess than 10 hours of the ife covered s average working week wi be ignored for the purposes of determining incapacity. If, immediatey before incapacity, the ife covered wasn t working, incapacity means the ife covered can t perform, on a fu or part-time basis, the duties of the ast occupation they foowed due to their iness or accidenta injury. Life covered The person whose ife is being covered. Limited payment term If seected, benefit payments wi be restricted to a tota maximum of two years for a periods of incapacity, whatever their cause. After we ve paid the benefit for a tota of two years, the poicy wi end. Medica practitioner A medica practitioner on the List of Registered Medica Practitioners with the UK Genera Medica Counci, or in the case of benefit paid for temporary overseas residence, the equivaent body in the reevant country. Occupation Occupation means work undertaken for profit, pay or reward. Poicy conditions This document, which forms our contract of insurance with you, providing the cover under the poicy as agreed. The appication (that you made and we accepted) and the poicy schedue aso form part of the contract and must be read together with these poicy conditions. Poicy schedue This wi show the specific detai of your poicy, such as who it covers, the benefit amount, how much it wi cost and any optiona benefits and additiona benefits incuded. Poicy term This reates to the period your poicy is in force, from the start date unti the end date. Resident At the time you compete the appication you must be physicay iving in the named territory and you: are a citizen of that territory or a British Overseas Territory citizen; or have been granted permission to sette permanenty in the named territory. Retai Prices Index (RPI) The monthy index cacuated by the government that demonstrates the movement of retai prices in the UK, or an equivaent repacement of that index. Standard terms Foowing an appication being underwritten, we may ony be abe to offer cover with a higher premium than first quoted, with certain benefits excuded, or both. This woud not be cassed as standard terms. We wi have tod you whether you were accepted on standard terms when confirming our decision on your appication. Start date The date on which cover under this poicy starts. It s shown in the poicy schedue.
