CMRC What we do Individualized post acute medical and rehabilitation therapies for non ventilated patients with chronic health conditions (newborns to geriatrics). Discharge planning solution for acute care hospitals allowing patients to upgrade to a home-like residential setting. Alleviate bed-blocking in acute care hospitals affording greater access to critically needed ICU beds. Reduce out-of-country long term care and rehabilitation avoiding expensive overseas care and travel expense. Reunite families by repatriation of patients to CMRC... almost 20% of our patients were repatriated from abroad (primarily Europe). CMRC International Partners Group of rehabilitation hospitals with its main 132-bed hospital in Massachusetts, the official teaching hospital for Harvard Medical School s Department of Physical Medicine and Rehabilitation. Spaulding is a member of Partners Continuing Care under Partners HealthCare, a non-profit organization that owns several hospitals in Massachusetts. Scope of service focuses on strategic technical support and advisory, including: Knowledge sharing participation in grand rounds, visiting specialists Access to evidence based medicine - clinical practice guidelines Training of staff and assessment of staff competence / expertise Jointly created clinical / quality dashboard - with quarterly assessment/ Quarterly chart audits Participation in annual conferences Technical Board position Based in Boston, MA, Joslin Diabetes Center is an independent not-for-profit institution. It is among the world s largest diabetes treatment, research and education centers. Joslin is a teaching and research affiliate of Harvard Medical School. Scope of service focuses on developing and implementing an action pan on diabetes management:. Phase 1: Four-month assessment, identify gaps, develop action plan for diabetes disease management program & capability development. Completed. Phase 2: Implementation of action plan, including: Diabetes clinical care programming (dashboards, knowledge sharing, development of resources, clinical supervision, etc.) Human capital (visiting specialists, exchange programs, training, etc.) Governmental advocacy (network enrollment, patient care model, etc.)
