MVRPC Senior Transportation Resources: Service Providers Montgomery County Senior Transportation Providers Brookville Area Handivan Ministry, Inc. 833-2260 The Brookville Area Handivan is a nonprofit ministry providing basic transportation for the elderly and disabled persons. Areas served include: Brookville, New Lebanon, West Alex (to 503), Lewisburg, Verona, Phillipsburg, Old Village of Clayton, and the western portion of Englewood. The service is free but donations are accepted. City of Kettering Charles I. Lathrem Senior Center City of Kettering Parks, Recreation & Cultural Arts Department 296-2483 http://www.ketteringoh.org/newweb/departments/senior/senior_transportation.php The Charles I. Lathrem Senior Center offers curb-to-curb weekday transportation for Kettering seniors. Destinations may include medical appointments, grocery shopping, senior center programs, hair salons, banks and pharmacies. Kettering seniors 60 and older in need of transportation must register at the senior center and purchase a riders card. Fees per trip range from $2-$8. Call (937) 296-2483 to register. Greater Dayton Regional Transit Authority (GDRTA) 226-1144 www.gdrta.org Fixed-route buses All RTA vehicles are climate-controlled, clean and comfortable, and every bus includes wheelchair lifts and bike racks. Destination and route signs are easy to read and all major stops are announced. Braille hailer kits also are available by request to visually impaired passengers. Senior Fares: $0.60/ride or $22/month. Project Mobility Since 1977, the GDRTA has offered Project Mobility, a curb-tocurb paratransit service, to individuals with certified disabilities who are unable to use regular fixed routes. Project Mobility service is available upon completion of a Project Mobility application and certification through the Project Mobility Certification Center. The Americans with Disabilities Act defines these categories of eligibility for paratransit service. You may be eligible if:

You live within 0.75 miles of an RTA fixed route. You are unable to independently board, ride or exit an RTA fixed route bus, due to a disability Due to impairment-related conditions you are prevented from traveling to or from a bus stop and therefore unable to complete the trip using an RTA fixed route bus You are unable to complete the trip using the RTA fixed-route buses due to a combination of your disability and architectural and environmental barriers You are the traveling companion of an individual who is eligible for the trip. Senior EZ Ride The RTA has a special bus service designed to fit the special needs of seniors. Senior EZ Ride service provides a simple, recognizable and safe transportation link to many of the destinations seniors prefer, including senior citizen centers, neighborhoods, grocery stores, shopping centers, hospitals and pharmacies. And with an on-board assistant and extra storage, you can keep all your grocery or shopping packages safe and secure. Route 65 covers North-West Dayton. Route 66 covers South-West Dayton and Kettering. Senior EZ Ride Routes

They provide van service available for members at a fee of $4.00 per roundtrip. Destinations include rides to the center and shopping. They have a new personal care sedan service for trips to the doctor, salon, etc. using volunteer drivers for residents of Centerville & Washington Township, started in October 2007. The personal care trips start at $4.00 per one-way trip and increase cost based on distance driven. Jewish Family Services 610-1555 x100, Information@jfgd.net http://www.jewishdayton.org Transportation is available Tuesday, Wednesday, Thursday and Friday from 8 a.m.- 5 p.m. Call by Wednesday for the following week. Cost is $4 per round-trip, paid when the car comes for you. Each additional stop is $1.50 more. Service area is downtown Dayton, Dayton View and north suburbs. Car is not lift equipped. Services to the South Dayton community are on an as-needed basis. Life Essentials (937) 586-0545 homeoffice@lifessentials.org Life Essentials relieves isolation and restores well-being to individuals in Montgomery County through a network of dedicated volunteers. Volunteer Caregivers provide services including transportation for medical appointments, help with daily tasks such as grocery shopping, and friendly visits to ease the loneliness and isolation that far too many people encounter. The service is free. Call 8:30 to 4:30PM, Monday through Friday. Northmont FISH 836-3637 Transports a limited number of seniors in the Northmont school district to medical and dental appointments using volunteers cars. Not wheelchair accessible. Donations accepted. Senior Resource Connection 223-8246 www.seniorresourceconnection.com Provides transportation to SRC adult day service sites for their senior and disabled patrons. Service areas: Montgomery County, Greene, & limited service in Preble. Trotwood Area Handivan Ministry, Inc. 837-1263 The Trotwood Area Handivan Ministry, Inc. is a nonprofit ministry providing basic transportation for elderly and disabled residents of the Trotwood and closely surrounding area. Destinations include medical appointments, retail, banks, social activities, post office and other services. These locations must be in Montgomery County. The van is

wheelchair accessible. This transportation service if offered Monday through Friday 8A.M.-5PM. To request service you must call between the hours of 12:30 and 2:30PM Monday through Friday. A volunteer will take your information. This service is free but donations are accepted. Wesley Community Center 263-3323, http://www.wesleycenterdayton.org/ The fleet of seven vans provides transportation for seniors throughout Montgomery County to approved nutrition sites (including Charles Lathrem, Ellison, Huffman, Jewish, Lohrey, Moraine, Miamisburg, and Wesley). Seniors 60 years and older may call the Transportation Office Monday through Friday, 8 to 3 to schedule their nutrition ride one day in advance. Wesley also offers medical transportation to PASSPORT/ComCare consumers who are scheduled for medical appointments throughout Montgomery County however, all referrals must come through the Area Agency on Aging (AAA). Village of Germantown Senior Center 855-4191 Transports Germantown and German Township residents over age 55 to the senior center, shopping, dining, and other group trips. Service is provided in a 14-passenger van that can accommodate one wheelchair. There is a $2 fee only for trips outside of the normal service area. Greene County Senior Transportation Providers City of Beavercreek Senior Transportation Program Lofino Senior Center 426-6166 Individuals must 55 or older and live within Beavercreek City or Township. Riders will be taken to destinations in the Beavercreek area, Miami Valley Hospital, Greene Memorial Hospital and Dialysis, East Town Dialysis, and Wright-Patterson Medical Center. Requests for transportation must be made 48 hours in advance. Rides are on a firstcome, first-serve basis. Medical appointments always take priority. Service hours are Monday through Friday from 9 a.m. to 4 p.m. If Beavercreek schools are delayed or cancelled due to weather, so are Van hours. Greene CATS 562-6523 or 1-877-227-2287 Transportation for all Greene County residents unable to drive themselves, dependent on available capacity. Operates both demand-responsive service and flex-routes in and between Fairborn and Xenia. Call for fares. Golden Age Senior Citizens Center, Xenia 376-4353

Golden Age provides many different services for the elderly population of Greene County. Programs are offered to Xenia residents age 50 and over, and Greene County residents 60 and over. The transportation service offers daytime trips to medical centers, social services, shopping, & the Center. Non-medical trips are provided only to Xenia destinations. The service is free but donations are accepted. Requests for transportation must be made 48 hours in advance. Senior Citizens Association of Metropolitan Fairborn 878-4141, http://fairbornseniors.org/ Transportation service provides rides for seniors to medical facilities for essential shopping, to social service agencies, the senior center, and other required needs. Volunteers serve as drivers for this service, which operates between 8AM and 4:30 PM. A wheelchair accessible van is available for any senior 60 and over living in Fairborn or Bath Twp. Requests for transportation must be made 48 hours in advance. Yellow Springs Senior Center 767-5751 Transportation service is provided for seniors in Yellow Springs and Miami Township unable to drive themselves. Requests must be made 48 hours in advance and fall within the center s hours, 10AM 4PM, Monday thru Friday. The drivers for this service are volunteers. Local fares are $1 each way, and the fares increase for longer trips. Miami County Senior Transportation Providers Goodwill/Easter Seals 778-3680 (Miami Co.) 275-4626 (Mont. Co.) http://gesmv.easterseals.com/ Provides transportation to medical facilities and meal sites (Miami Co.) & adult day services (Miami and Montgomery counties). Vehicle is a wheelchair accessible van. Cost: $6-12 dollars. Miami County Transit 335-RIDE http://co.miami.oh.us/a55969/mcounty.nsf/all/transit Serves all Miami County residents, regardless of age or disability. Destinations are typically within the county, with some exceptions. A majority of the trips will end in Miami County, In some cases special trips may have destinations outside of Miami County, but within a 50-mile radius. Intercounty connections can be made with Vandalia and Huber Heights. Drivers may assist you from your door into the bus but cannot enter your home. Senior cost: $1 one-way within Troy, $1.50 one-way outside Troy. Schedule trips within county 1 to 2 hours in advance; trips outside county must be scheduled a week in advance. Transports seniors and disabled residents to destinations within a 50-mile radius of Troy. The vehicle type varies depending on available volunteers. There is no fee. Medical and Nutrition transportation are provided under contract with the Area Agency on Aging. Transportation is also provided to and from the senior center and on scheduled group shopping trips. There is no fee, but donations are accepted.
