College Counseling Program Application & Guideline 3 IMPORTANT - PLEASE READ!! Thank you for your interest in the University Extension College Counseling Certificate Program. Enclosed is an application, which you should complete and return. The College Counseling Certificate is designed to bring to participants the latest in college counseling techniques and to emphasize the personal dimension of working with families during one of their most crucial transitional experiences. Skilled and enthusiastic instructors make the program a professionally valuable and personally meaningful learning experience. Please see the attached page outlining the specific information, which must be included in your application package in order for it to be considered complete. Submission of a completed application does not imply automatic acceptance into the program. A total assessment of the file will be made, and a decision to accept or deny will then be determined. If you have any questions regarding the application procedure, please call (858) 534-9286 or email unexeduc@ucsd.edu ADMISSION REQUIREMENTS Each student is required to submit a formal application to the College Counseling Program. The qualifications for admission to the program are as follows 1. Submission of completed application packet including: a. Signed, dated, and completed application form b. Statement of Intent i. On a separate sheet, please write a one-page typed essay describing your reasons for participating in the College Counseling Certificate Program and how it may be relevant and applicable to your goals as an educator. c. Verification of a Bachelor s degree or higher from and accredited institution including degree awarded and date. i. Must provide either an official letter from the school or an official, sealed, unopened transcript from an the accredited institution which awarded your degree. 2. All Candidates After your application packet had been received and reviewed, you will be sent detailed information to pay the application fee. a. Non-Refundable application fee of $60.00. ***Please submit all required documents in one packet.*** Important: Retain copies of all materials submitted for your files and reference.
College Counseling Program Application & Guideline 4 Guidelines for Candidacy to the UCSD College Counseling Certificate Program A completed application must be on file before enrolling in the core courses. Please read the instructions below before completing and submitting the application. In addition to the application form, please submit all supporting documentation with your application. Applications will not be processed without the supportive documents. Fees A $60.00 nonrefundable application fee is required. You will not need to pay the fee until after your application has been received and reviewed. Once the Education Department determines your application is complete you will receive detailed information in order to pay the application fee. Application Acknowledgement Applications will be reviewed by the Application Review Committee. Student will be notified of acceptance into the College Counseling Certificate Program by mail within approximately 4 weeks. Once officially enrolled in the program, the participant s course history will be monitored by the program representative. Upon successful completion of the 12 required quarter-units (three required courses) a Specialized Certificate from the University of California, San Diego Extension will be awarded. It is strongly recommended that students complete all the program requirements within two years from the date of enrolling in the first course. Requesting Certificate Upon Program Completion After completion of the requirements in your College Counseling Program, candidates must finalize one of the following to Student Services to initiate the issuance of the UCSD Certificate from Student Services Calling Certificate Processing (858) 534-3400 Mailing a completed Intent to Complete Certificate Program form Submitting the Online Intent to Complete Certificate Program form If you would like to request more than one certificate there is a $25 duplication fee for additional certificates.
