Miller, Director of Risk Management Employees Driving Personal Vehicles on District Business, Activity/Field Trips Employees who drive their personal vehicle on district business, activity/field trips need to complete the information below annually, if they regularly drive their personal vehicle, or when they know they will be driving and transporting students in their personal vehicle. The form should be faxed (402-458-3276), emailed to Nancy Summers (nsummers@lps.org) or sent to Box 14, LPSDO. You need to allow enough time for Risk Management to do a driver s license check with the Department of Motor Vehicles. If you have an emergency situation, please note that on the email/fax transmission. You can assume you are approved to drive unless you are contacted by Risk Management. Other driving requirements are: 1. Seat belts will always be used. You will only transport the number of students that you have seat belts for in your backseat. 2. Cell phones/blue tooth will not be used while driving. 3. Smoking is prohibited. 4. Use of illegal drugs/alcohol is prohibited. 5. Use of prescription medication containing a warning about no driving while using is prohibited. 6. You must carry the minimum liability insurance required under Nebraska Law. The district recommends you carry full coverage including collision. Your personal vehicle insurance is primary coverage should you be involved in an accident. The district assumes no responsibility for damage caused to your vehicle while on school business, whether through your personal negligence or the negligence of a third party. 7. If you are involved in an accident while on school business, contact Risk Management as soon as possible after the accident. (402-436-1760) 8. You must have a valid Nebraska Drivers License. 9. If transporting students, you must review the attached emergency evacuation and first aid procedures. Lincoln Public Schools GUIDELINES AND CERTIFICATION FOR PARENTS/STAFF DRIVING PRIVATE VEHICLES ON FIELD TRIPS When personal vehicles are used to transport students for field trips or other activities, the principal should do the following: 1. Verify that the person driving has vehicle insurance ask to see the insurance card required by State Law to be carried in each vehicle. If the driver is not the owner of the vehicle, verify that the owner has given permission for the use of the vehicle. 2. Verify the number of seat belts and that the seat belts are functioning. Do not allow more students to ride than the number of functioning seat belts. REQUIRE SEAT BELTS TO BE WORN WHEN IN THE VEHICLE. If students to be transported fall into the category that requires a car seat/booster seat, verify that appropriate equipment is available for use and used. All children up to age 6 riding in any motor vehicle must ride correctly secured in an appropriate child restraint system that meets Federal Motor Vehicle Safety Standards (FMVSS 213). This means that the child restraint systems must be appropriate for the child's size, age and weight. 1. Do not allow students to ride in the front seat of the vehicle if the vehicle is equipped with a passenger side air bag. 2. Verify that all students have parent permission for travel in personal vehicle form on file. 3. Verify the driver of the personal vehicle understands the liability they are assuming should they be involved in a motor vehicle accident. The person driving needs to complete and sign the following: Name: Driver License No. I certify the following is true and accurate: My driver s license is subject to any restrictions. If yes, list the restriction. I will abide by all rules of the road and any applicable rules of the Nebraska Department of Education and the District relating to driving a motor vehicle. (See attached Department of Education information on emergencies). I will not use my cell phone (hand held or blue tooth) while driving. I certify that I am of good moral character and will not engage in conduct or use language inappropriate for children. I certify that I have a satisfactory driving record without any suspension, revocation, withdrawal or expiration of my license; any ticket or accident which could result in suspension, revocation, withdrawal or my license; or any circumstance which may result in any responses on this form not continuing to be completely accurate or which may indicate that I should not be driving and/or transporting students.
