These benefits are designed to protect your health, your family, and your wealth, and they re a valuable part of the total rewards package Cornerstone offers. Participating in the Plans Some of these benefits are provided automatically to you at no cost as a Cornerstone employee, while you will need to enroll in others when you first become eligible or during the annual Open Enrollment period. You may only change benefit choices during the year if you experience a qualified change event, such as marriage, divorce, or birth or adoption of a child etc. Who s Eligible All full-time active employees who work at least 30 hours per week on a regular basis are eligible for Cornerstone benefits. You must enroll within 30 days of your hire date. When you enroll in many of the Cornertone benefit plans, you can also enroll your; Spouse Unmarried children, up to age 26 (age 23 for Supplemental Life) Children of any age who are disabled. Children include your biological children, your spouse s children, children covered by a Qualified Medical Child Support Order (QMSCO), and children in your guardianship. Please refer to the plan documents for exact terms and conditions of coverage. Medical Plans All eligible employees have access to comprehensive medical coverage through the Blue Cross Blue Shield of Alabama Blue Card Preferred Provider Organization (PPO) Plan: Blue Card PPO Plan With the Blue Card PPO Plan, you have the freedom to receive care from any licensed provider. However, you generally pay less when you receive care from doctors and hospitals that participate in the Blue Card network and have agreed to provide services to plan members at special discounted rates. In general, once you ve met the, you ll pay a percentage of the cost for each service until you reach the annual out-of-pocket maximum. Dental Plans Cornerstone offers you a PPO dental plan provided by Principal Financial Group (Principal). With the PPO Plan, you have the flexibility to choose any dentist and receive benefits. However, if you go to a preferred dental provider (PDP) network dentist, you ll pay less because PDP dentists have agreed to charge lower, negotiated rates. Vision Plan The Vision Plan, provided by Principal, provides a benefit for exams and materials on a yearly basis. You have the freedom to see any vision provider you choose, but the plan generally pays better benefits when you receive care from doctors and hospitals that participate in the VSP network. If you decide to go to an out-of-network provider, you ll be reimbursed for exams and eyewear according to a schedule of benefits. Flexible Spending Accounts (FSAs) With a Flexible Spending Account (FSA), you can set aside tax-free money to pay for eligible expenses. Cornerstone offers two FSAs through Alliance Insurance Group (Alliance): The Health Care FSA The Dependent Care FSA When you participate in an FSA, you decide how much you want to contribute each plan year. The money you contribute is then taken from your pay before taxes are taken out this lowers your taxable income, which means lower taxes for you! There are a few important rules for FSAs, so make sure to review the information from Alliance before you enroll. Health Care FSA You can contribute up to $2,550 to your Health Care FSA each year. You can use the money in the account to cover many expenses that aren t covered by your medical plan, including: Copays, s, and coinsurance Over-the-counter medications prescribed by a physician Smoking cessation programs Vision care not covered by your plan, including contact lens solution and LASIK surgery. Dependent Care FSA You can contribute up to $5,000 per married couple to the Dependent Care FSA. You can contribute up to $2,500 per year if you are single or married and filing taxes separately. This FSA can be used to cover expenses for the care of an eligible dependent (a child or a dependent adult), such as nursery or day care costs, so you and your spouse (if you re married) can work. Life and Accidental Death & Dismemberment Insurance Cornerstone provides all eligible employees with Basic Life Insurance and Basic Accidental Death & Dismemberment (AD&D) Insurance. Each of these benefits is equal to one times your annual salary, to a maximum of $200,000. Basic Life Insurance pays a benefit in the event of your death, while Basic AD&D Insurance provides an additional benefit to you or your beneficiaries in the event of certain accidental losses or death. All life and AD&D coverage is provided through Principal. Basic coverage is 100% paid for by Cornerstone. You pay the full cost if you elect optional coverage. 5
Optional Life and AD&D Insurance You have the option to purchase Optional Life and AD&D Insurance for yourself, as well as your eligible dependents. Employee Optional Life Insurance Coverage is available in the following amounts: $25,000, $50,000, $75,000, $100,000, $150,000, $200,000, $250,000, $300,000, $400,000 or $500,000. Spouse Optional Life Insurance Coverage is available up to 50% of your optional life insurance, up to $250,000. Child Optional Life Insurance Coverage for children is available in the amount of $10,000. You pay the full cost of any Optional Life and AD&D Insurance coverage you elect. If you do not elect during this open enrollment or when you are first eligible, EOI will be required for all amounts. EOI is also called proof of good health and certifies to Principal that you re generally healthy at the time of purchasing coverage. You will be notified when your EOI is approved. Disability Coverage Both STD and LTD are provided by Principal. Cornerstone pays the full cost of STD coverage. You pay the full cost of any LTD coverage you elect. Cornerstone offers all eligible employees two disability plans: Short-Term Disability (STD) Plan The STD plan pays 60% of your predisability earnings, up to $1,000 per week, for up to 26 weeks of disability after you ve been disabled for 14 days. Cornerstone pays the full cost of STD coverage. Long-Term Disability (LTD) Plan If your disability extends beyond 180 days, the LTD plan provides a benefit equal to 60% of your predisability earnings, up to $6,000 per month. You pay the full cost if you elect to purchase LTD coverage. Your Cornerstone disability benefits will coordinate with any state disability benefits available, so the maximum benefit won t be higher than the amounts described above. You ll be responsible for filing any claims directly with the carrier. 6

After this open enrollment, EOI will be required if you don t enroll in LTD insurance when first eligible. Critical Illness Insurance Critical Illness Insurance pays a lump-sum benefit if you re diagnosed with a covered critical illness. You can choose coverage in the following amounts: For yourself: $5,000 increments, to a maximum of $100,000 For your spouse: $2,500 increments, up to 50% of your coverage, to a maximum of $50,000 For your child: $2,500 Payments can be used to cover related expenses, including s, copays, hospital bills, child care, household bills, travel to a treatment facility, and more. Coverage is available for any of the following distinct categories, among others: cancer, heart attack, stroke; heart transplant; coronary artery bypass surgery, angioplasty/stent; major organ transplant, end-stage renal failure, paralysis not due to stroke; burns; coma; loss of sight, speech or hearing; and other miscellaneous diseases. The Critical Illness benefits are provided by Principal. You pay the full cost for these benefits. The coverage is supplemental which means the benefits pay in addition to your health coverage. You pay the full cost of Critical Illness Insurance. This coverage is available at competitive group rates through convenient payroll deductions. Employee Assistance Program (EAP) The EAP is 100% paid for by Cornerstone. Best of all, its available 24 hours a day, 7 days a week. You can reach the EAP offered by Principal at 800-450-1327. As a Cornerstone employee, you automatically have access to our EAP a confidential resource available to help you with life s everyday issues. When you contact the EAP, which is provided by Principal you can speak to a registered nurse, who can help answer your health-related questions. Should you need it, your plan will provide three face to face visits with a licensed practitioner. You ll also have access to licensed counselors who can help with other issues, such as: Parenting issues Work-related situations Relationship problems Substance abuse issues Self-improvement Mental health issues Financial concerns Legal-referral assistance 401(k) Plan The Company shall make a special safe harbor matching contribution to the Plan. This contribution is 100% of the first 3% of your deferred compensation and 50% on the next 2% of your deferred compensation. Enrollment available on the 1 st day of the month following the 90 th day on the job as a full-time employee. You are eligible to enroll at any time. The 401(k) Plan is provided by Brinker. Time-off Benefits At Cornerstone, we know taking time for yourself is important too. We re pleased to offer the following time-off benefits. Holidays At the beginning of each year, the Company will publish a list of the holidays to be observed that year. Cornerstone generally recognizes the following paid holidays each year. Paid Time Off (PTO) Length Of Service PTO Accrual 90 days to 4 years 2 weeks 5 years to 9 years 3 weeks 10 or more years 4 weeks Your PTO bank will roll over each year until your PTO bank reaches a maximum of 280 hours.
