Introduction The is a coordinated comprehensive system that strives to provide Iowa s frail elderly consumers with the opportunity to make their own choices regarding long-term care and to receive home and community-based services in the home and community setting. CMPFE gives the Iowa frail elderly population an alternative to a nursing facility and other forms of institutionalized services. CMPFE coordinates the delivery of needed community services which allow Iowa s frail elderly to remain safely in their own homes. Each consumer is assisted and monitored by an individual case manager. The case manager provides assistance to the consumer in making the appropriate choices that best fits the consumer s needs. The Iowa Department of Elder Affairs (DEA) is the state agency responsible for the. Area Agencies on Aging administer the program at the local level. In October 2006, case management became a service under the medical assistance elderly waiver, thereby allowing utilization of federal Medicaid dollars. Twelve of the 13 area agencies on aging enrolled with the Department of Human Services (DHS) to become Medicaid case management providers. types of service agencies also enrolled as Medicaid case management providers. All of the area agencies on aging continue to serve elderly consumers who are not eligible for Medicaid but need case management to help them remain in their homes. The CMPFE 2008 report deals only with those consumers who were served by the area agencies on aging. On June 30, 2008, there were 9,086 active consumers participating in CMPFE with 12,212 total consumers served by our department and the area agencies on aging during the year. During fiscal year 2008, the process was begun to modify several administrative rules governing the program with the intent being to streamline operational processes. Among the changes were: 1

Extending from 30 to 90 days the time period a consumer can be in a nursing facility before the consumer is discharged from the case management program. Allowing local discretion in the development of area agency policies on prevention and management of conflicts of interest, consumer appeals, consumer complaints, and screening of consumers. Clarifying case manager training requirements and service plan contents. Currently all Iowans over the age of 60 can access CMPFE and its services through their local area agency on aging. Services offered through this program make it possible for thousands of Iowa s frail and/or low-income elderly to stay in their home and local communities. 2
