PLEASE READ THOUROUGHLY BEFORE SUBMITTING APPLICATION Overview The Arizona Sports and Tourism Authority (AZSTA) often receives requests from non-profit organizations seeking financial support for youth sports programs that fall outside the scope/project or deadline schedules of AZSTA s Biennial Grant Program. To help meet these requests and to further AZSTA s youth and amateur sports mission, a Program and Quick Grant Funding application has been established to provide quick solutions to the community s needs. Steps to Apply PLEASE READ 1. Complete the application form and budget template below: 2. Save your completed application in Word Format only 3. Handwritten or faxed applications will NOT be accepted 4. Please submit the completed and signed application, budget template and supplemental information via email to: robin@az-sta.com 5. Applicants will generally receive notification of the status of their request via email within ten business days from receipt of the application. Applications that are not completed correctly may be required to wait six months to re-submit Program & Quick Grants Funding Criteria Applications are accepted on a year-round basis. The Authority sets aside 5% of its annual YAS funding per A.R.S. for the Quick Grants Program. Of those funds, 25% will be available for Program Grants that fall outside of the standard Quick Grant Criteria. AZSTA will award the funds on a first-come, first-served basis providing that the program s criteria are met. Grants are based on a completed Program Grant Funding Application and project budget template. Quick Grant / Program Grant applications are accepted on a year-round basis. The Authority sets aside 5% of its annual YAS funding per A.R.S. for this program and awards the funds on a first-come, first-served basis providing that the program s criteria are met. Grants are based on a completed Quick Grant Program Funding Application and project budget template, the statement of need, demonstration of financial need and the benefit to a local community. Factors considered in awarding Grants will include but are not limited to; Statement of need Demonstration of financial need Sustainability of program Benefit to a local school Benefit to the local community The Applicant must be a Maricopa County agency, municipality, school district or any other incorporated public entity, (including any combination of the foregoing) or a 501(c)(3) or (c)(4) non-profit organization, which has been in operation for a minimum of one calendar year (evidence required). Applicants are limited to one Program or Quick Grant during any twelve-month period. Biennial Grant recipients within the prior two calendar years may not eligible for a Program Grant. Program or Quick Grant recipients within the prior calendar year will not be eligible for a Program or Quick Grant. Applicants who have previously received funding from AZSTA may apply for funds, but may not be as highly considered as others who have not received prior funding. Program Grant funding which typically falls outside of the Quick Grant requirements will be considered on a case by case basis. Prior awards have been awarded for coach and first aid training and expenses associated with training. 1 of 6

Quick Grant funding is primarily focused on equipment-related needs. Program and Quick Grant funding will not be awarded for: fixed overhead/administrative expenses (e.g. salaries, rent, utilities, etc.), loans/endowments/grants, conferences, individuals, tournaments, fundraisers or benefit tables; travel expenses outside of Maricopa County; capital campaigns; funding to maintain the sustainability of an organization or program or anything else deemed as not serving the youth and amateur sports community within Maricopa County. All AZSTA approved items must remain with the organization for future use by the organization and not be allowed to be kept by or given to the individual participants. Program and Quick Grants are limited to a maximum of two-thirds of the cost for the project identified, up to a total of $5,000 from AZSTA. AZSTA, in its sole discretion, may award less than the amount requested. The applicant is required to contribute a minimum of 1/3 of the total project cost. The Authority will not approve requests for items previously purchased or program monies previously expended. Once a grant has been awarded, the Authority s funds will be provided on a reimbursement basis after the Applicant has completed and fully funded the approved project grant request. In order to receive the reimbursement the Applicant must complete both the Program/Quick Grant Project Summary Report and the Program/Quick Grant Reimbursement Request Form. All approved Program and Quick Grant projects must be completed, funded and the forms submitted to the Authority within 90 days of the award or the award shall automatically be rescinded by the Authority. If rescinded, the organization must wait six months to re-apply. Any increase to the project costs shall be the sole responsibility of the Applicant. Any project savings shall be shared on a pro-rata basis between the Applicant and the Authority. The Authority will not provide reimbursement for any items not matching the original Program Grant request. Program and Quick Grants are made at the sole discretion of AZSTA.
