Introduction At QTMB, you can have as many accounts as you like under the one membership, with the benefits of one statement, one membership number and fewer fees and charges. We try to minimise the impact of fees and charges on our members and provide many services at no charge. These free services are outlined in this brochure. of Fees & Charges Phone and Internet Banking On transactions between accounts owned by you at QTMB (excludes transfers to QTMB third party accounts or transfers to other institutions). Includes BPAY. RediATM, EFTPOS and Visa Debit card transactions on the Everyday account and MyAccount. Cards No charge for initial Visa Debit card and Redicard. Statement Copies When you print from internet banking. No Charge for Cheque Books We do not charge for cheque books. No Early Repayment Penalty No early repayment penalty on car and personal loans. Loans Loan fees for new loans are fully disclosed in your loan contract. The following list outlines our current loan related fees and charges. All Members Receive $1 for every $1,000 held in QTMB savings/loans* Maximum Rebate is $50 *Calculation of the monthly rebate is based upon: Average savings account balance or overdraft + balance of loan at end of month + balance of term deposit at end of month - unauthorised overdraft. For each whole $1,000 in the result obtained above, $1.00 fee rebate will be awarded up to a value of $50.00. For joint loan and/or term deposit accounts, only the primary account holder will be eligible for the rebate. # The Everyday account and MyAccount are not part of the fee rebate scheme and their balances do not contribute to the rebate calculation for the other accounts the member holds. 10

QTMB Zest Financial and Lifestyle Program Annual Membership fee $30 Cash Handling Cash Request Fees Cash withdrawals of more than $5,000 require 24 hours notice Cash delivery charges, levied by the cash provider to QTMB, are passed on at cost for large withdrawals. The charge is determined at the time of the request and may vary according to location Inactivity Biannual Inactivity Fee* $10 * If you don t use your membership for a period exceeding 12 months, an inactivity fee may apply on accounts with a balance of less than $500. The fee will be charged biannually. If your account becomes overdrawn as a result, your share account will be offset, and we may be forced to terminate your membership. For members under 18 an inactivity fee applies after non-use for a period exceeding 24 months where a minimum balance of $100 is not maintained. Financial Planning Interviews (includes GST) First interview No cost^ Subsequent or consultancy interviews: we may charge an hourly fee for work undertaken (not covered by the fees below). Financial Planner Support Staff $275 - $550 per hour $165 per hour ^ All personal advice must be documented in a Statement of Advice (your personal financial plan) and fees apply for this service. Financial Plan Preparation/ Statement of Advice (SOA) Fees (includes GST) Depending on complexity# $990 - $5,500 # The cost of the SOA will depend on the advice provided. More complex SOAs and revisions to SOAs could cost more. 13

Implementation (includes GST) For implementing the strategies recommended in your Financial Plan, the following implementation fees apply to funds invested: First $400,000 0.88% Second $400,000 0.44% Over $800,000 Nil (Max. fee $5,280) Reviewing your Financial Plan and Ongoing Services (includes GST) Reviewing your goals, strategy, circumstances and investments is vital to ensure that you remain on track to meet your objectives. The ongoing fee for our review service is a percentage based fee which is calculated on your holdings and paid via your investment. The fee for this service will be up to 1.50%.
