1. Introduction There are three Reference Guides for Providers to use during Enrollment or Re-Enrollment: Standard Abbreviations and Postal Information Valid Provider Type-Specialty Pairs Allowed Services by Provider Types In this you will find: Figure 2-1 for allowed rendering providers for provider groups, and Figure 2-2 for Facility, Agency, and Organization ( FAO ) providers that require rendering providers. You will need to refer to this information during enrollment and/or re-enrollment. Use this chart below to determine which table to use. Individual Providers and Facility, Agency, and Organization providers without renderings do not need this Reference Guide. Mapping Tables The mapping tables begin on page 3. It is imperative that providers indicate the correct provider type and specialty (or specialties) on their applications. Failure to do so could result in incorrect claim reimbursement. To use this table, do the following: 1. Choose the correct table for the enrollment type for which you need more information. 2. In the Pay-To Providers column, find the provider type-specialty pair for which you need information about Allowed Rendering Providers. 3. In the corresponding Allowed Rendering Providers, locate the appropriate provider type-specialty pair for the provider you are enrolling. Note the following: When you go online, if only one specialty is shown in the Allowed Rendering Providers Specialty cell, the provider must designate this specialty on their enrollment application assuming they are credentialed for this specialty. MIHMS_RG_0004_v6.0_20150312.doc Page 2

Maine Integrated Health Management Solution If more than one specialty is shown in the Allowed Rendering Providers Specialty cell, the provider must choose the specialty that is correct based on their credentials and enroll that specialty online. If the text No specialty required appears in the MIHMS Specialty cell, the provider should not designate a specialty on the enrollment application. 4. New provider types and specialties are added from time to time and occasionally provider types and/or specialties are also sometimes removed. If you don t see your provider type here and believe it should be here according to a ListServ message or reasonable expectation, you may go on the MIHMS Health PAS Online Portal to see if your provider type is displayed as available (and therefore, valid) in the system or you may call Provider Enrollment for more information.
