2 Dear Associate, It s time once again for annual benefits enrollment. Last year, Weis Markets made significant changes to the benefits program, much of it driven by the Affordable Care Act, also known as Health Care Reform. I m pleased to announce that this year there are fewer changes to your benefits and just a 5% increase in your premiums. This amount is less than past increases and much lower than the overall trend among employers. Consumer-Driven Health Care Health Care Reform has changed the way all of us think about health care and insurance. Our society as a whole is moving toward a more consumer-driven approach to health care. The websites where medical plans can be bought are actually referred to as marketplaces. At Weis Markets, we know about marketplaces, consumers and smart shopping. We encourage you to take advantage of this new development to find the best medical coverage for you and your family whether that is at Weis Markets or on the public marketplace. Before you make your enrollment decisions, please visit www.healthcare.gov to ensure that a Weis Markets medical plan is truly the best coverage at the right cost for you. In addition to medical plan benefits, Weis Markets has been creating more of a marketplace approach with the addition of voluntary benefits. Voluntary Accident Insurance is new this year and can help you with unexpected costs in case of an accident. Since the marketplace approach is new and you may not be familiar with the voluntary benefit plans, you will have a one-on-one meeting with a licensed Benefits Educator. The Benefits Educator will guide you through the plan information, answer your questions, and enroll you in your 2014 choices. This is an active enrollment, which means you need to meet with a Benefits Educator to be enrolled in benefits for the 2014 plan year. This will also be your only opportunity to enroll in our voluntary benefits offerings. Get Your BALANCE Wellness Plan Rewards As part of our ongoing efforts to support a healthy and productive workforce, the BALANCE wellness plan offers rewards for healthy behaviors. The enrollment materials explain how you can earn up to $300 toward your health care expenses by participating in BALANCE wellness activities. Your benefits are important purchases that provide financial security and peace of mind for you and your family. Just as you would with any major purchase, please read the enclosed enrollment guide carefully and discuss your options with your family members. 3 3 Welcome to Open Enrollment! Weis Markets 2014 enrollment period is your annual opportunity to review and make any changes to your benefits for the March 1, 2014 February 28, 2015, plan year. This guide highlights the benefit plan options that are available to you. Take some time to review this guide along with your other enrollment materials (i.e., Summary of Benefits and Coverage, Preventive Care Services, and the Search and Save tool) before you meet with a Benefits Educator. What s New for 2014 We re making some important changes to Weis benefits for 2014. Please review this guide carefully for details about your benefit options, including the changes highlighted below. All associates must enroll to receive coverage for 2014. Your current benefits will NOT roll over automatically, so make sure you take the time to review your options and attend a confidential one-on-one enrollment meeting with a Benefits Educator! (You will be contacted to schedule your appointment.) There are very few changes to your benefits program this year. Medical Plan o Increased spousal surcharge on premiums - If you have a spouse eligible for coverage through another employer and you choose to cover your spouse under a Weis Markets plan, you will pay more in premiums. The amount you will pay will increase from $1,000 to $1,250 per year. o Increased tobacco surcharge on premiums - If you use tobacco products, you will pay a higher premium. The amount you will pay will increase from $500 to $700 per year. You can avoid this surcharge by completing a tobacco cessation program that is tailored to your needs or become tobacco-free Short-term Disability (STD), Long-term Disability (LTD), Basic and Supplemental Life, Accidental Death and Dismemberment o MetLife will replace The Hartford as the insurance carrier and administrator. o If you are currently on disability, you will continue to be paid through The Hartford. Voluntary Accident Insurance o You may now purchase coverage for unexpected expenses due to an accident.

Key Things to Know Benefits Educators. You will have a private session with a licensed Benefits Educator to answer your questions and enroll you in the benefits you choose. Your manager will provide the schedule at your location. Health Care Reform. Before you meet with the Benefits Educator, check out your medical plan options and premiums on www.healthcare.gov to see if any of the plans being offered under health care reform are better suited to your needs. Working Spouse Surcharge. If your spouse is eligible for coverage through another employer and you choose to cover your spouse under a Weis Markets medical plan, your premium will increase by $1,250 per year (approximately $24.04 per week). Tobacco Use Surcharge. Tobacco is a leading cause of disease and death. As part of our commitment to a healthy and productive workforce, if you use tobacco in any form, including cigarettes, pipe tobacco, cigars or chewing tobacco, your tobacco surcharge premium will increase from $500 to $700 per year. To remove the surcharge you must be tobacco-free for 90 days prior to enrollment or complete a tobacco cessation program during the year. Get on the Road to Better Health. Take your first step with the online Personal Profile. Simply log on to www.capbluecross.com/weis and complete the interactive Personal Profile. Once Enrolled, Accessing Your Benefit Information is Easy. You will gain instant access to a link called mycapbluecross.com which will provide you with important benefit information and valuable online tools and services customized to you all at one convenient location. This website provides access to: o Summary of Benefits and Coverage o BALANCE and Better Health Works health and wellness resources o Secure access to personal benefit information such as medical claims and explanation of benefit notices (EOB) o Nurse Line, A Trusted Source for Health Information Have you ever wished that your doctor s office was open around the clock to answer your questions? Questions about your health can come up at any time, day or night. Everyone is eligible to contact Capital BlueCross Nurse Line. They provide assistance for more than urgent health situations. Use Nurse Line to ask questions about important health concerns at 1-800-452-2583 such as: o Minor illness and injuries o Chronic health conditions o Immunizations and screenings o Weight loss and nutrition Earn up to $300 in an HRA. To encourage you to take an active role in your health and well-being, Weis Markets will contribute up to $300 to a Health Reimbursement Account (HRA) that you can use to pay eligible medical expenses such as your deductible and co-insurance. You earn HRA money through participating in certain BALANCE wellness program activities. Check out the Voluntary Benefits. Weis Markets has been adding a number of voluntary benefits to help provide financial security in case of disability, accidental injury or death. Be sure to learn about these benefits to determine if they may be right for you. 4

5 Benefits for You and Your Family Weis Markets is pleased to announce your 2014 benefits program. The program is designed to provide you with benefit solutions that help you stay healthy, feel secure, and help you maintain a work/life balance. During open enrollment, you will have an opportunity to participate in a variety of available benefit plans. Medical Dental Vision Employer-Paid Basic Life and AD&D Voluntary Term Life Employer-Paid Short-Term Disability Long-Term Disability Critical Illness Whole Life Accident Insurance NEW! The effective date for these benefits is March 1, 2014. Deductions for weekly-paid associates will begin with the paycheck received on March 13, 2014. Deductions for monthly-paid associates will begin with the paycheck received on March 25, 2014. Who is Eligible? If you are a full-time associate, working 40 hours or more per week, and have met the established waiting periods for the insurance plans outlined in this enrollment guide, you are eligible to participate. A few of the plans require that you must be actively at work on the day your coverage takes effect. BALANCE Wellness Rewards As an associate at Weis Markets, you are eligible for certain incentives if you participate in our BALANCE wellness program. Weis will contribute up to $300.00 to a Health Reimbursement Account (HRA) in your name when you complete certain BALANCE wellness activities. When and How Do I Enroll? Open enrollment will be conducted January 15 February 5, 2014. This year we will have Benefits Educators available onsite to meet one-on-one with each eligible associate. These are trained Benefits Educators who will answer questions about the benefits program and help you enroll in your benefits for 2014. Your manager will let you know what dates the Benefits Educators will be at your location. You are required to meet with a Benefits Educator to guarantee your benefits for the 2014 plan year. This will also be your only opportunity to take advantage of guaranteed issue (no health questions) for certain voluntary benefits. Changing Coverage During the Year You can change your coverage during the year only when you experience a qualified change in status, such as marriage, divorce, birth, death, adoption, placement for adoption, or loss of coverage. The change must be made within 30 days of the event. The change also must be consistent with the event. For example, if your dependent child no longer meets eligibility requirements, you can drop coverage only for that dependent.

Medical Insurance 6 At Weis Markets, the group medical plans are offered through Capital Blue Cross. You may choose from the following plan options: PPO I, PPO II or the Traditional Plan (if you are currently enrolled in it). For the PPO plans, Capital Blue Cross allows you to choose from a large list of participating providers for all of your health care needs. If your spouse is eligible for coverage through another employer and you choose to cover your spouse under Weis Markets, an additional $1,250 per year will be added to your medical premium. If you are a tobacco user, an additional $700 per year will be added to your medical premium. The surcharge can be removed with completion of the Ceridian coaching program. The weekly surcharge is outlined below. You must use a Weis pharmacy to receive benefits. You can benefit from significant savings when you choose generic medications. 8 BALANCE Wellness Rewards As an associate at Weis Markets, you are eligible for certain incentives if you participate in our BALANCE wellness program. If you elect the PPO I plan or the PPO II plan, you may receive money that will be placed in a Health Reimbursement Account (HRA) to be used to offset medical out-of-pocket expenses such as the deductible or coinsurance. If you do not use the entire amount rewarded, it will be rolled over to the next year. If you elect the Traditional Plan or if you waive medical coverage, you may receive an incentive through payroll. The steps and reward amounts are outlined below. What is a Health Reimbursement Account (HRA)? A Health Reimbursement Account (HRA) is a special account that allows you to pay for qualified medical expenses with tax-free dollars. By law, only employers may contribute to HRAs, so you cannot make your own contributions. However, Weis will contribute up to $300 to an HRA in your name when you complete certain BALANCE wellness activities. You will not be taxed on any HRA contributions or withdrawals when used for qualified medical expenses. HRAs will be managed by Capital Blue Cross, the medical plan administrator. When your provider submits a claim for a qualified medical expense, Capital will apply your HRA balance toward your deductible or coinsurance automatically, as long as there are funds in your account. If you do not use your HRA balance in the coming year, it will roll over automatically to the following year (unlike a flexible spending account or FSA). However, HRAs are not portable, which means if you leave Weis for any reason, you will forfeit any remaining balance in your account. Visit www.capbluecross.com to identify treatment options, quality ratings and cost estimates for office visits, treatments and procedures.

9 Dental Insurance The dental plan is offered through Delta Dental. The plan offers two options: a Core Plan and a Buy-Up Plan. With both options, you may choose any dentist to provide your oral care; however, if you choose a preferred provider, claims may be paid directly to your dentist. 11 Flexible Spending Account A Flexible Spending Account (FSA) allows you to set aside pre-tax dollars to cover qualified expenses that you would normally pay out of pocket with post-tax dollars. The FSA is comprised of a health care account and a dependent care account. You pay no federal or state income taxes on the money you place in an FSA. FSA contributions should be planned carefully, as any funds not used by the end of the year will be forfeited. A grace period has been added that allows a 2 ½ month extension where a participant can incur and submit expenses. Enrollees will have until March 15th to incur expenses and then 90 days after that to submit claim requests. The FSA program will not roll over into the next year. Re-enrollment is required each year. Open enrollment for the FSA takes place in November for the following calendar year. If you did not elect the plan in November 2013 for the 2014 calendar year, you will have to wait until November 2014 to enroll. Employer-Paid Basic Life and AD&D Insurance Weis Markets provides a basic life and accidental death and dismemberment (AD&D) benefit for you through MetLife. Your Benefits Educator will be able to provide you with the benefit amount and update your beneficiary information during your enrollment meeting. Voluntary Term Life Insurance In addition to your employer-paid life insurance, associates are eligible to purchase additional term life on a voluntary basis through MetLife. You may purchase up to 7 times your annual earnings, in $10,000 increments, up to a maximum of $500,000 with a completed evidence of insurability. If you are enrolled in the voluntary life program, you have the opportunity to purchase life insurance for your spouse and children. You can enroll your spouse in $5,000 increments up to a maximum of $10,000 on a guaranteed issue basis. Your spouse election amount cannot exceed 50% of your life amount. Amounts over guaranteed issue will require completion of an evidence of insurability form. You may also elect to provide coverage up to $10,000, not to exceed 10% of your amount, for your dependent children. Dependents may be covered up to age 25. Your Benefits Educator will provide you with additional information. *Associates living in NJ and NY receive a statutory disability insurance that is mandated by their state. Therefore, their short-term disability benefit amount will be reduced by the statutory disability amount.

Voluntary Long-Term Disability Insurance Long-term disability insurance can provide you with the security of knowing you will have income replacement in the event of an extended disability due to an illness, accident or pregnancy. Upon approval of a disability claim, you can receive up to 60% of your monthly income to a maximum monthly benefit of $10,000 once you have been out of work for 180 consecutive days. Your individual cost varies depending on your age. Your Benefits Educator can provide additional information, including pricing, on this plan. Long-term disability insurance does have a pre-existing limitation of 6/6/12. This means that if you were treated for a medical condition six months prior to the effective date, it will not be covered unless you are treatment-free for six months after the effective date of coverage and are still actively at work for 12 months. You can use this coverage more than once. If you receive a full benefit payout for a covered illness, your coverage can be continued for the remaining covered conditions.
