Insurance benefits are effective the first of the month following the employee s hire date, unless otherwise noted. New employees have 30 days from their hire date to enroll in NHA s health and welfare benefits. Benefit Enrollment is to be completed through Lawson Employee Self-Service at https://lawson.mynha.com. Employees have 30 days to notify People Services of a status change event in order to make changes to most benefit plans. A status change event may be any of the following life events that affect eligibility for coverage - birth, marriage, adoption, divorce, legal separation, loss of other coverage, or qualifying employment status change. Employees have 30 days from the qualifying event date to make changes/additions to any benefit plan that permits such mid-year changes/additions. In-Network Benefits Coverage can be used for either glasses or contacts $20 exam co-pay Frame allowance: $200 per benefit period $20 lenses/frame co-pay Contact lens allowance: $130 per benefit period in lieu of glasses VSP also provides discounts on many other non-covered services and materials. Exams, lenses and frames are covered once per benefit period. Out-of-Network reimbursement is available but is limited. These types of Flexible Spending Accounts allow employees to set aside tax-free dollars to pay for certain out-of-pocket medical expenses and dependent care expenses. Employees can set aside up to $5,000 annually per account. If the money is not used before the end of the benefit year, the remaining money left in the account will be lost. When employees contribute to a Flexible Spending Account, they will lower their taxes (saving on federal, state, city and social security taxes) and increase their take-home pay. The tax savings is about $0.25 on every dollar. Health Care Flexible Spending Account Through the HCFSA, the IRS allows employees to be reimbursed for most out-of-pocket medical, dental, vision and hearing care expenses. For a full list of reimbursable expenses visit: www.flexadministrators.com/fsa/reimexp.html. This account comes with a debit card for easier, faster claims processing. Dependent Care Flexible Spending Account The DCFSA enables employees to pay for out-of-pocket, work-related dependent-care costs on a pre-tax basis. Both single and married employees can use the account. Claims Benefit Period

Transportation and Parking FSA This benefit is available to all full-time employees regularly scheduled to work 35 or more hours per week. The transportation flexible spending account (FSA) allows employees to pay for expenses related to mass transportation used to get to work. The parking FSA allows employees to set aside tax-free dollars to pay for expenses related to parking at or near NHA s premises or the location from which mass transportation is taken. Employees can enroll in, stop or change contributions at any time during the year. Elections will carry over from year to year, and there is no use it or lose it provision like with the healthcare and dependent care flex plans. When employees contribute to a Transportation or Parking FSA, they will lower their taxes (saving on federal, state, and FICA taxes) and increase their take-home pay, which typically is about $0.25 on every dollar. Mass Transportation Allowable Expenses Allowable expenses associated with using mass transportation to get to work include: vanpools, subways, buses or ferries. This includes any vehicle that seats at least 6 adults (not including the driver), and at least 80% of the vehicle s mileage will be for transporting employees between their homes and work places, with employees occupying at least one-half of the vehicle s seats. Parking Allowable Expenses Expenses associated with parking at or near the employee s work place also are allowable. This includes parking costs associated with parking at or near the location used to take mass transportation. The maximum amount employees can contribute pre-tax to this account changes on an annual basis to allow for inflation for mass transportation and parking. The 2012 limits are $125 per month for mass transportation and $240 per month for parking. Coverage amount equals 1x your base salary with a minimum of $25,000 and a maximum benefit of $50,000. Voluntary Life Insurance UNUM Insurance Customer Service: (800) 343-5406 or visit: www.unum.com Full-time employees regularly scheduled to work 35 or more hours per week may purchase additional, voluntary life insurance in the following amounts: Employee: up to $200,000 in $10,000 increments Spouse: up to $50,000 in $10,000 increments (only available if voluntary employee life insurance is elected) Child(ren): $10,000 per child (only available if voluntary employee life insurance is elected) The bi-weekly cost dependents on the employee s age and the amount of coverage elected. Disability Insurance Short-Term Disability NHA (866) 907-0928 or visit https://my.heritageacademies.com/benefits for more information NHA provides all full-time employees regularly scheduled to work 35 or more hours per week with short-term disability (STD) benefits if they have completed one year of active full-time service in the year preceding the leave of absence or two years of

active part-time service (20 hours or greater per week) in the two years preceding the leave of absence. This benefit is at no cost to the employee. If approved, the first week will be paid out at 100% of pay and the following weeks will be paid out at 67% of pay until released to return to work by a physician. Long-Term Disability Lincoln Financial Group (800) 423-2765 or visit www.lfg.com for more information NHA provides all full-time employees regularly scheduled to work 35 or more hours per week with long-term disability (LTD) benefits after they have completed one year of active full-time service in the year preceding the leave of absence. This benefit is at no cost to the employee. If approved, employees can earn 60% of their pay, up to a maximum of $10,000 per month. Employees turning 50 in the 2012 calendar year may make an additional catch-up contribution of up to $5,500 in 2012. Eligible employees can enroll immediately upon hire or transfer into an eligible position. If no action is taken, employees automatically are enrolled in the plan after working 45 days at NHA. Once eligible, employees may increase, decrease, start or stop 401(k) contributions at any time by contacting New York Life. Currently, NHA matches $0.50 for each $1.00 of employee pay deferral contributions (including automatic contributions) up to 6% of compensation, and NHA matches every paycheck. There is no vesting period for employer match contributions; any match contributions are the employee s immediately. PearlStreet Investment Management of Oppenheimer & Co. Inc. NHA will reimburse employees 50% of their tuition costs upon successful completion of their class ( B 3.0 or better) up to a maximum of $6,000. A maximum of 9 credit semester hours or $6,000 will be reimbursed during the fiscal year,

July 1- June 30. The date on which the class begins determines which fiscal year the reimbursement is based upon. Nontuition related fees, books, recreation fees, and supplies are not eligible for reimbursement. ENCOMPASS is an Employee Assistance Program (EAP) provider, and this benefit assists employees and their household members with obtaining help with life s challenges. Employees and family members can contact ENCOMPASS 24 hours a day, 7 days a week and schedule a face-to-face, phone, or online consultation. There is NO CHARGE for their services. This benefit offers free, confidential, unbiased financial and legal advice to employees and family members, either online, in person or on the phone. Adoption Assistance (866) 907-0928 or visit https://my.heritageacademies.com/benefits for more information To qualify for this benefit, the employee must be an active regular full-time employee with at least one year of active full-time (35 or more hours per week) service to NHA in the year preceding the adoption. This benefit offers assistance of up to $3,000 child for the costs associated with an adoption. The benefit is provided upon proof that a legal adoption or placement is complete. The costs of adoption also must be substantiated. All necessary documents must be sent to the People Services Department for approval and processing. This benefit does not apply to legal fees for adoption of stepchildren.
