MyFloridaNet-2 ITN No: DMS-13/14-024 Attachment D Price Workbook Instructions Return all required Price Sheets in the Price Workbook with the reply to this solicitation as described in Section 2.15, How to Submit a Reply, of the ITN document. The Price Sheet Checklist in the Price Workbook shall identify the name of the Respondent, and bear the signature of a business/corporate representative. Rates submitted for all items in the Price Workbook cannot be raised, but may be reduced, in the BAFO pricing submittal. Complete the Price Workbook without qualifications, exceptions, counter offers, edits or deviations. General Instructions and Stipulations 1. E-mail the Procurement Officer identified on the cover sheet of the solicitation document for an electronic version (Microsoft Excel) of the Price Workbook. 2. Complete all cells (highlighted in ) in all Price Sheets; otherwise the Price Sheet submission will be incomplete, with the exception of Column F and K in Price Sheet 1 (CPE Router, VPN, and Firewall). Please see instructions below for Column F and K in Price Sheet 1 below. 3. If a zero is placed in any cell the price of that service will be evaluated as no charge. 4. Do not alter any cells in the Price Workbook. 5. Do not modify formatting in the Price Workbook. 6. Refer to the Statement of Work for detailed explanation of the services. 7. Pricing and services shall be made available statewide except where expressly stated by DMS. 8. The Price Sheets shall become part of the final contract, as modified during negotiations. 9. Submission of a reply under this solicitation constitutes a binding offer of the prices submitted. 10. The Price Sheets shall be utilized to order services. 11. All rates provided will be in US dollars. 12. Provide monthly recurring charges (MRC) and non-recurring charges (NRC) as specified for each service in the respective Price Sheets and instructions. 1 13. All rates shall be inclusive of any applicable government-sanctioned surcharges and fees. Respondents must comply with subsections 2.3.16 and 2.6.5(a) of the Statement of Work. 14. There shall be no one-time or non-recurring charges such as installation, disconnection, cancellation, or service order fees except where expressly stated by DMS. 15. There shall also be no quantity commitments or guarantees from DMS or the customers. 16. Price Sheet Checklist Due to the amount of information required in this portion of the solicitation document, a checklist has been provided to ensure all required fields have been filled in. Enter the name of Respondent in the space provided. This information will automatically populate on subsequent Price Sheets. For each price sheet, Respondents will see one of the following indicators: Unattempted Attempted, but incomplete Complete Ensure the Checklist indicates prior to submission of your reply. for each Price Sheet included in the Price Workbook The rows in the bottom portion of the Price Sheet Checklist show each customer s total monthly spend. As the Price Sheets are completed, those monthly spend figures will update based on the Respondent's proposed prices. The intent of this ITN is to avoid increasing the monthly spend for all customers. If any customer is projected to experience an increase in their monthly spend, this will be indicated by a red box around the customer s name. Price Sheet 1 - CPE Router, VPN, and Firewall This Price Sheet provides a list of currently installed Customer Premises Equipment (CPE) configurations. DMS has not dictated a specific access type for the proposed solution as long as it meets the requirements outlined in the Statement of Work. Stipulations 1. Do not propose a CPE that has an End-of-Sale announcement published by the equipment manufacturer. 2. Charges for CPE rental for each of the CPE configurations will be calculated using the formulas provided under instructions for columns I below. 3. The CPE formula and values submitted for each of the variables listed below will be used to price all CPE routers, VPN appliances, and Firewalls. 4. The Respondent is responsible for proposing CPE that meets all applicable requirements, performance, and remediation service levels defined in the Statement of Work. Page 2 of 10
Instructions for Rows 6-12 1. Hardware/Software Discount (percentage off MSRP): Provide the guaranteed discount off the Manufacturers Suggested Retail Price (MSRP) for all hardware/software configurations. The current MFN discount is 49%. 2. Maintenance Discount (percentage off MSRP): Provide the guaranteed discount off the MSRP for one year of maintenance on rented hardware/software configurations. The current MFN discount is 10%. 3. Hardware/Software & Installation Divisor (Number of Months): Provide the number of months over which hardware/software costs will be amortized. The current MFN divisor is 36 months. 4. Installation (Non-Recurring Charge): In the installation cost, include staging, initial setup and configuration, delivery, rack mounting, and on-site turn-up by the Respondent s trained, professional technician. The current MFN installation cost is $693.02. 5. CPE Router and VPN Appliance Standard Configuration Management: In the cell provided, indicate the rate that will apply to all CPE Router and VPN Appliance configuration options. Do not bundle Configuration Management with other rates provided on subsequent Price Sheets. Do not provide a Configuration Management rate for firewall CPE. The firewall Configuration Management rate is to be placed in the Ancillary Managed Security Service Price Sheet. Instructions for Columns Labeled as A J A. Configuration Title: Each configuration has been given a title for evaluation purposes. B. Access/Bandwidth: DMS has indicated which bandwidth speeds and access type are relevant for each current configuration listed. C. Product Part Number: Based on the list of currently installed CPE, DMS has outlined the product part number for each item in the configuration. D. Description of Product Part Number: Provides a description for each product part number. E. Quantity: Provides the quantity of each product part number. F. Proposed Configuration including Product Part Number, Description, Make/Model and Quantity: For each Access/Bandwidth type, propose a configuration which includes the make/model with related product part number and specifications. Even though the list of currently installed CPE includes several End-of-Sale CPE, do not propose a CPE that has an End-of-Sale announcement published by the equipment manufacturer. The completion of Column F for each Access/Bandwidth type, is required however, not all rows may be needed based on the proposed configuration. If more rows are needed for a configuration, continue the description in Column K. When reviewing the Progress Indicator in the Checklist Tab for the Complete status of Price Sheet 1 CPE Router, VPN, and Firewall, the status may indicate complete even if Columns F and K are empty. Therefore, check to ensure the proposed configuration is included for each Access/Bandwidth type. G. Hardware/Software Cost (MSRP): Provide the MSRP for the hardware and software included in each configuration. While the purchase of CPE will not be an option on the MFN2 contract, this information is necessary to determine the CPE rental rate per the formula provided below (see instructions for column I ). H. List Price for 1 Year of Maintenance (MSRP): Provide the MSRP for one (1) year of maintenance for each configuration option listed. Propose CPE maintenance that meets all applicable requirements, performance and remediation service levels defined in the Statement of Work. I. Price Sheet 2 WAN Service This Price Sheet is provided to capture rates, by flat rate and mileage band, for statewide Wide Area Network (WAN) services. Instructions for Columns Labeled as A G A. Bandwidth: This column lists all of the required bandwidth speeds the Respondent is required to price. B. Core Port: Provide the MRC for the relevant bandwidth speed. Do not bundle Internet access with the core port charge. Internet access shall be priced separately in the column provided. C. Local Loop Access: Provide the MRC for local loop access for each bandwidth speed. Rates must include any third party and/or independent company local loop access charges. o For bandwidth speeds up to 12Mbps, rate must be flat rate. o For bandwidth speeds 15 Mbps up to and including 45 Mbps, the first mileage band will cover 0-25 miles. o For bandwidth speeds 50 Mbps and greater, the first mileage band will cover only 0-10 miles. o For bandwidth speeds 50 Mbps and greater, the second mileage band will cover o >10 miles, up to 25 miles. For all bandwidth speeds, local loop access pricing for the following mileage bands must be provided: i. > 25 miles, up to 35 miles ii. > 35 miles, up to 50 miles iii. > 50 miles Alternatively, Respondents may choose to provide a flat rate for all or a combination of the bandwidth speeds in this Price Sheet specific to mileage bands. To do so, place the same rate in each mileage band for each applicable bandwidth speed. D. Internet Access: Provide the MRC for Internet access for each bandwidth speed. MAN Service: MAN service includes the local loop access that is the physical link or circuit that connects from the demarcation point of the customer premises to the edge of the carrier or telecommunications service provider's network. MAN service includes all the necessary components such as a Respondent's port used to switch traffic from location to location. The service also includes any necessary equipment such as termination device(s) required at the customer premises and the carrier s network. Provide the MRC for MAN service for each bandwidth speed. Rates must include any third party and/or independent company local loop access charges. C. E-rate Eligible Portion: Evaluation points for each bandwidth speed are listed in the Eligible Points Possible column. The E-rate Eligible Charges column will be automatically populated. Price Sheet 11 - Snapshot 1 Current Services: The Snapshot tool is an assessment of pricing for services currently installed under MyFloridaNet and the Florida Information Resource Network (FIRN). The Snapshot is for sites with service greater than 12Mbps. Place your cursor in any cell of the mileage band column to reveal the drop down menu. Select from the drop down menu the appropriate mileage band for each local loop access. The Respondent's rate from the WAN Service Price Sheet for local access will be populated. The remaining elements and services are listed in a separate tab. All current inventory service elements are linked to the appropriate proposed pricing and then summed. The total cost is populated at the bottom of the Snapshot. A list of current service locations is provided for reference. B. Price Sheet 12 - Snapshot 2 Education Standard: The projected education standard Snapshot is based on the current state standards as defined by the Florida Department of Education in comparison with the current rate for services.
