If you need transportation, and you live in Washington County, you have a variety of options available to you. Transportation fees and fares will vary with each service provider and many require some advance notice to schedule a ride. Other transportation services exist that may not be included here. Check your local phone directory or the internet for more information. This information is subject to change. Allegiance Transportation 651-207- 5211 www.allegiancetransportation.com At Allegiance Transportation, our goal is to try and ease those concerns, and replace them with the comfort of knowing you or your loved ones are in the very best of hands while traveling with one of our professionals. Prior to your first riding experience with our company, we offer to our clients a meeting with one of our staff to introduce you to our company. We ll take time to answer any questions you may have about us, our company, or our drivers. We ll also have some questions for you such as any regular appointments, therapy you re undergoing, or any other transportation needs you may have. We offer the safest, most courteous, respectful, and comfortable transportation experience for seniors, disabled persons, or anyone else who has decided to leave the driving to a professional transportation provider. Caregivers escort patient to appointments and are available to remain with patient during appointments, also assist with meals and light housekeeping. Days/hours of service: 7 days a week, morning through evening. Length of advance notice required: 24 hours advance notice. Wheelchair lift: No. Price: $66 for 3 hours, plus $0.50 per mile paid directly to the caregiver. Payer sources: Private Pay/cash or check, no MA or insurance. 2

American Cancer Society 1-800-227-2345 www.cancer.org Transportation provided to cancer patients who are ambulatory and need rides to doctor appointments and medical treatments. The Road to Recovery Program uses volunteer drivers. Five business days advance notice is encouraged. Days/hours of service: Monday Thursday, (8:00am -5:30pm); Friday 8:00am 4:30pm (varies based on patient need). Length of advance notice required: At least 5 days if possible; the more notice given, the better chance to find an available screened & trained volunteer driver. Wheelchair lift: No patients must be ambulatory. Price: Free service available to all cancer patients based on availability of drivers. Payer sources: Free through American Cancer Society s Road to Recovery Program Blue Cross Blue Shield 651-662-8648 www.bluecrossmn.com A service to BluePlus members who are ambulatory. Transportation provided to medical, dental, physical therapy or eye exams. Days/hours of service: Office hours are 8:00am 4:30pm, Monday Friday. Length of noticed required: 3 business day advance notice Wheelchair lift: No. Price: Free with plan. Payer sources: Insurance. BrightStar Care St. Croix Valley 651-234-0044 24/7 phone support http://www.brightstarcare.com/st-croix-valley/ We accompany the client the entire time with a Companion, Certified Nursing Assistant, Licensed Practical Nurse, or Registered Nurse, depending on the care level needed for each situation. Call for more information on services and charges. Days/hours of service: 7 days a week, 24 hours a day. Length of advance notice required: 2 days. Wheelchair lift: No client must be ambulatory. Price: Dependent on the level of care required, plus $0.56 per mile. No mileage charge if we use the client s car. Please call for more information. Canvas Health (formerly Human Service, Inc.) 651-275-4300 www.canvashealth.org For passengers not ADA certified, our private service is a system for those individuals preferring additional privacy time. Our second option is designed for groups wanting to share a common origination point and a common destination point while enjoying the fellowship of others. Days/hours of service: Monday through Friday, between 8:00am and 4:30pm. Length of advance notice required: 4 days. Price: For individuals: The cost of this service is $25 per ride ($50 round trip) within our service area, if available, Monday through Friday. For groups: The cost of this service is $70 per hour with a two hour minimum. Payer sources: Private pay Community Thread 651-439-7434 www.communitythreadmn.org Transportation provided by volunteer drivers to medical appointments for persons who are live in the Stillwater School District (ISD 834), are ambulatory and have with no other means of transportation. There is a pre-screening process for riders. Qualified riders need to complete a pre-service application form prior to start of service. Days/hours of service: Monday Friday, 8:00am - 4:00pm (varies depending on the availability of volunteer drivers, distance to and from appointment, and city of rider s residence); no holiday, evening or weekend service provided. Office hours: 9:00am 1:00pm, Monday Friday. Length of advance notice required: minimum of 3-5 full business days prior to the day of the appointment. Will take ride requests up to 30 days before day of appointment. Wheelchair lift: No, riders must be ambulatory. Price: Fares apply for non-contract rides. Fares start at $15.00 for a round trip ride and $10.00 for a one-way trip. The cost is priced below commercial taxi services. Call for more information. Disabled American Veterans of Minnesota 612-467-2768 www.davmn.org The Disabled American Veterans of Minnesota strives to provide Minnesota veterans in need with transportation to VA Medical Center appointments through our DAV Volunteer Transportation Program. Disabled veterans who are unable due to financial or medical hardship to secure transportation to their medical appointments in the St Cloud and Twin Cities metro area. Days/hours of service: As volunteers are available. Length of advance notice required: at least a week. 4

Please contact the office to inquire about various medical transport services provided by the company. Days/hours of service: Monday Thursday, 6:00am -6:00pm; Friday, 6:00am - 5:00pm. Length of advance notice required: at least 1 day. We provide transportation and companion services for seniors and those with disabilities in the greater Twin Cities Metro area and south central Minnesota. All riders must be age 55 or older. Additionally, riders must be temporarily disabled and unable to drive themselves. (Note: Service is not available to Assisted Living or Care Center sites that already provide transportation.) Days/hours of service: Monday-Thursday, 7:00am to 5:30pm.; Fridays, 7:00am to 3:30pm. Length of advance notice required: Rides must be scheduled 24 hours in advance and within 20 miles of the East Bank address - 500 Harvard St. SE. Our goal is to make life easier by providing services that help keep seniors and those with needs independent and in their homes. In addition to transportation and in-home services from Home Helpers, we also offer Personal Emergency Response Systems (PERS) through Direct Link. Providing help with just the click of a button. No long term commitment, however a contract for services is required in advance of first service assignment. Our reliable, trained and insured CAREGivers SM can transport ambulatory individuals to physician appointments, religious services, sporting events and other social outings, as well as escort for all shopping and errand needs. A free face-to-face consultation is required before transportation services can be provided. Please contact our office for more details and rate information. Communities served include: Bayport, Baytown Township, Birchwood, Cottage Grove, Dellwood, Lake Elmo, Landfall, Mahtomedi, Newport, Oak Park Heights, Oakdale, Pine Springs, St. Paul Park, Stillwater, Willernie and Woodbury. Days/hours of service: Services hours vary by community. Length of advance notice required: Varies depending on off-peak ($3.00 one way trip) or peak hours ($4.00 one way trip) and day of the week. Call for more information. Metro Mobility also offers a downtown fare zone ($1.00) for trips beginning and ending in the downtown areas. Price: Please call for more information and rates. Free linkage service by telephone available Monday Friday, 8 am -4:30 pm, please leave a message after hours. Many helpful links for senior services on both websites. Minnesota Non-emergency Transportation (MNET) - 1-866-467-1724 or 651-645-9254 www.mtm-inc.net MNET provides transportation to medical appointments for people on Medical Assistance and some pregnant women and children on MinnesotaCare who have no other transportation available to them. Days/hours of service: Monday Friday, 7:00am 6:00pm. Weekend/after hours for hospital discharges only. Length of advance notice required: 3 days for scheduled trips, and urgent trips as needed. Price: Per Medical Assistance rates. Payer sources: Medical Assistance, FFS and Waiver.
