Enrollment Checklist Review this Enrollment Guide and other information at www.bannerbenefits.com. If you don t have access to the Internet, contact the Service Center at Banner Plan Administration for assistance (see Banner Health Benefits Contacts in this guide). GO TO BENEFITS CONTACTS SECTION Use the Benefit Modeling Tools in MyHR. Log in to Employee Manager Self Service (EMSS) and use the tools in MyHR to compare plan options and estimate costs in order to make the best choices for you and your family. Enroll before time runs out. If you miss this window, your next opportunity to enroll will be during any qualifying life event or the next Annual Enrollment in the fall. You may enroll online or over the phone (see Benefit Enrollment in this guide). GO TO BENEFIT ENROLLMENT SECTION Designate a beneficiary for your life insurance. Since Banner Health automatically provides you with a basic level of Life and AD&D coverage, be sure to designate a life insurance beneficiary in the online enrollment system even if you do not plan to enroll in any other benefits. Review your confirmation statement. After you enroll, you ll receive a confirmation statement. Read it to verify your coverage is accurate. If you find a discrepancy, you must notify the Service Center at Banner Plan Administration immediately to correct any errors. Make sure you receive your Benefits ID cards. You will receive your benefit cards for medical after your enrollment is complete. (see Enrollment in this guide). ID cards are not issued for dental and vision coverage. Please refer to the dental and vision sections of this guide for additional information. GO TO ENROLLMENT SECTION Contact Fidelity to enroll in or review your retirement plan options. Our 2015 benefits have a lot to offer and this guide will help you determine which programs are best for you and your family. We strongly encourage you to take an active role in your health and the selection of your benefits. Taking advantage of the information and tools we offer can potentially save you hundreds of dollars each year on your medical expenses! The information contained in this booklet provides a general overview of all of our programs. More detailed information can be found on our employee benefits website at www.bannerbenefits.com and in MyHR. GO TO MYHR SECTION While many other companies have elected to reduce or eliminate benefits due to increasing costs, Banner Health has continued to make adjustments and enhancements to our benefits programs and has absorbed most of the costs. Banner Health pays over 75% of our employee benefits. Whenever and wherever possible, we strive to keep your costs affordable while providing the best possible value and benefit. We ve made investments in benefits, programs and services that will help you and your family stay healthy and enjoy a better quality of life. This enrollment guide is designed to help you understand our benefit offerings for 2015 and how to complete your benefits enrollment. Benefit Enrollment 2015 When do I need to enroll? Annual Enrollment Current employees may enroll each year during the designated Annual Enrollment period in the fall, with choices taking effect on Jan. 1 of the next year. New Hires/Newly Eligible New hires and newly eligible employees have 31 days from your hire date (or from the date you move into a benefits-eligible position) to complete your benefits enrollment. Whether you enroll on day one, day 10 or day 31 of your enrollment period, your benefits coverage will begin on the first day of the month following your hire/eligibility date. If your hire date falls on the first day of the month, your coverage would begin on that day. If your hire/ eligibility date falls on the first day of the month, you still have 31 days to enroll, but your coverage would be effective on your hire/eligibility date. If your hire date is Jan. 23, you have until Feb. 23 to complete your benefits enrollment. Even if you don t enroll until Feb. 15, your benefits coverage will begin on Feb. 1 and you will be responsible for all of February premiums. What happens if I don t enroll during the enrollment period? If you do not enroll during your enrollment period, you will not be able to enroll during the year unless you have a qualifying event (see page 7). You may be able to enroll during the next Annual Enrollment period in the fall and your benefits will be effective Jan. 1, 2016. www.bannerbenefits.com 5

Benefit Enrollment 2015 Who can I enroll? Many of the benefits plans allow you to add eligible dependents to the same plan in which you are enrolled. Depending on the plan, the eligible dependents may include: Your spouse Your same-sex spouse if your marriage took place in a state that recognizes such marriages Your common-law spouse if you live in a state that recognizes such marriages Your same-sex or opposite sex domestic partner and eligible dependent children (enrollment is limited to medical, dental, vision, life insurance and legal plan) Your children, stepchildren, or those for whom you have legal guardianship up to age 26 Your children who are at least age 26 and are incapable of self-support because of a mental or physical disability. Verification of your dependents eligibility is required. The specific documents you must provide are listed on the Dependent Eligibility Matrix at www.bannerbenefits.com. Where can I find the employee benefits premiums? You can view the premiums sheets by logging in to MyHR or the online enrollment system. How do I enroll in benefits? You can enroll either online or by phone. Online Go to www.bannerbenefits.com and click the Enroll button. This will take you to our online enrollment system. You can also find a link to the enrollment system by logging in to MyHR. Enter your Banner Health employee ID number located on the back of your ID badge. Enter your eight digit password, which is your full date of birth. For example, if you were born Jul. 17, 1958, your password would be 07171958. You will need to follow the instructions and create your own unique password once you have logged in to the enrollment system. Follow the on-screen prompts to enroll. By phone Call the Service Center at Banner Health Plan Administration at (480) 684-7070 in the Phoenix area, or (800) 827-2464 in all other areas during the hours listed in the More Information section. Benefit Enrollment 2015 How do I pay for my benefits? You and Banner Health share the cost of your benefits coverage, with Banner Health paying the majority of the cost for most employees. Throughout the year, 401(k), Flexible Spending Account and Health Savings Account contributions are automatically deducted from each paycheck. Your remaining benefits contributions (medical, dental, vision, life and legal) will be deducted from your paycheck, 24 of the 26 pay periods. Benefits contributions are taken on a pre-tax basis (excluding coverage for life/ad&d and legal plan). This reduces your taxable income, which lowers the amount of federal and state taxes withheld from each paycheck. The portion of your premiums for coverage of your domestic partner and his/her children must be paid with post-tax dollars and imputed income will be charged to you. Be sure to check your pay stub to ensure your premium deductions are accurate. When can I make changes to my benefits? You can only change your benefit elections during the year if you have a qualifying event (proof will be required). All changes must be made within 31 days of the event and will take effect the first day of the month following the event other than a newborn who is effective the date of birth. If your event date falls on the first day of the month, your changes will be effective on the event date. Eligible qualifying events include: Changes in your legal marital status Changes in the number of your dependents Changes in employment status Changes in a dependent s eligibility You or your dependent become eligible or ineligible for federal or state premium assistance program funded coverage You or your dependent lose other coverage due to exhaustion of COBRA, loss of eligibility or employer termination of coverage Change in residence or work site that creates a change in your health plan coverage For details about qualifying events, please refer to www.bannerbenefits.com. What happens after I enroll? To receive an electronic confirmation statement, you must provide either a Banner Health or a personal email address during the enrollment process. Otherwise, you will receive a paper confirmation statement in the mail. However, if you elect to receive your confirmation statement listing your 2015 benefits, it is very important that you review the confirmation for accuracy and follow the instructions for making any needed corrections. www.bannerbenefits.com 7

Benefit Enrollment 2015 How do I receive my benefits ID Cards? Depending on your enrollment choices, you will receive the following cards, which will be mailed to your home address: If you are enrolled in one of Banner Health s Choice Plus or Select medical plans, the plan you selected and the dependents you enrolled will all be listed on a single Benefits ID card. The card will be sent by Banner Plan Administration (BPA). You will receive a second copy of the Benefits ID card for your enrolled dependents to share; additional copies may be requested as needed from BPA. You will need to show your card when you visit a doctor or dentist, or when you get a prescription filled. If you are enrolled in the Banner Health Savings medical plan, you will receive an ID card from Blue Cross Blue Shield of Arizona and a debit card for your Health Savings Account from HealthEquity (if new enrollment or your card has expired). If you are enrolled in a Health Care and/or Dependent Care Flexible Spending Account, you will receive a debit card from Discovery Benefits (if new enrollment or your card has expired). If you are enrolled in a dental plan no ID cards will be sent, but you may print a card or find the ID Card App for your smart phone at www.mycigna.com. Please note that no ID cards are issued for any of the other plans; your confirmation statement serves as your proof that you are enrolled. Enrollment Tips Dependent Enrollment All dependents must either have a Social Security number, or you must verify that they do not have one as part of the enrollment process. You must notify the Service Center at Banner Plan Administration immediately when your dependents no longer meet the requirements to be enrolled in a Banner Health benefit plan. If you and your spouse/domestic partner are both employed by Banner Health and eligible for benefits, you can t be covered as both an employee and a spouse/domestic partner. There is no reason to pay twice for coverage when Banner Health will only make one payment for your claims. For the same reason, only one of you can enroll your dependent children in the plans. Even if your dependents are enrolled in your medical plan you can make separate coverage decisions for other benefits. For example, you can elect to cover you and your eligible dependents for medical however you may want to cover only yourself and your spouse for vision, and only yourself for life/ad&d. www.bannerbenefits.com 8

Benefit Enrollment 2015 Dependent Verification Banner Health must ensure that everyone enrolled in our plans meets the eligibility requirements. During enrollment you must make sure your dependents are eligible to participate in our plans. Details about dependent eligibility and required documentation are available on www.bannerbenefits.com. Tobacco Usage and Premiums The enrollment system will assume that every enrolled member uses tobacco products, and will charge you at Tobacco User rates. During enrollment, you may change that status by affirming that each covered individual does not use tobacco products. If you or a covered dependent uses a tobacco product, then you may still qualify for the Non Tobacco User discount, if you or your dependent quits using those products during the year or participates in a tobacco cessation program offered as part of the Health Plan s wellness program. The tobacco user surcharge is intended to encourage you to stop using tobacco products. If you successfully complete the program (offered at no additional charge), you may be able to avoid the surcharge effective as of the first day of the following month. Contact the Service Center at Benefit Plan Administration to learn more about the wellness program and how to lower your monthly premium. Debt Repayment Authorization Banner Health currently faces challenges in collecting payment for services from our own employees, which impacts the financial health of our company. As a result, Banner Health s policy requires payroll deductions for delinquent accounts for services provided to employees and their enrolled dependents. As a condition of participation in Banner Health s medical plans, employees in Arizona and Colorado will be required to sign an authorization allowing Banner Health to collect overdue payments through payroll deduction when necessary. Banner Health employees located in other states will not be required to sign this authorization at this time. Enrollment in other benefits will not be affected. www.bannerbenefits.com 9

Benefit Enrollment 2015 Find a PCP If you are enrolling in the Choice Plus medical plan you will need to enter a primary care physician (PCP) code for each individual at the time you enroll. This is the only medical plan that requires you to select a PCP during enrollment. You may change your PCP at any time during the year by calling the Service Center at Banner Health Plan Administration (see Banner Health Benefits Contacts on page 39). A list of PCPs who participate in the Banner Health Choice Plus and Select medical plans is available at www.bannerhealthplans.com. A list of PCPs who participate in the Banner Health Savings medical plan is available at www.azblue.com. Consider choosing a PCP who works within Banner Health Centers and Clinics. Choosing these physicians, who are employees of Banner Health Medical Group and are contracted with all of Banner Health s medical plans, gives you access to excellent medical care while also supporting Banner Health. Same-sex Spouses and Pre-tax Coverage The Internal Revenue Service (IRS) issued guidance that same-sex couples, legally married in jurisdictions recognizing same-sex marriages, will be treated as married for federal tax purposes. The ruling applies regardless of whether the same-sex couple lives in a jurisdiction that does not recognize same sex marriage. Banner Health s medical, dental and vision plans that cover an employee s validly-wed samesex spouse will treat the coverage as non-taxable, at least for federal tax purposes, just as the plans treat coverage of opposite-sex spouses. This is true even if the state where the same-sex spouse resides does not recognize the marriage. These same-sex spouses are also entitled to COBRA protection upon a loss of coverage due to divorce or legal separation from the employee. www.bannerbenefits.com 10

MyHR Benefit Modeling Tools Choose the health plan that s right for you Selecting the right medical plan is not as easy as one would think. There are so many factors to consider when making this decision. Choosing the right plan takes time, planning and analysis for your specific circumstance. To assist you in this process, we developed Benefit Modeling Tools, available on MyHR, to make this process much easier and effective! MyHR is an online service available every day to provide you with timely HR assistance. You ll be able to access tools to help you compare benefit plans and calculate costs so you can make the best choices for you and your dependents. You can access MyHR by logging in to Employee Manager Self Service (EMSS), or by using the links at www.bannerbenefits/myhr. Once logged in to MyHR, you will find: A link to your benefits premiums on the front page of MyHR A link to a Medical Cost Calculator to help you estimate your expenses under each of the medical plan options A Compare To tool that allows you to make side-by-side comparisons when more than one plan is offered. To use the tool, select Benefits and then select one medical, dental or vision plan from the list. Click on Compare To in the upper right corner to select additional plans. You can compare specific features of the plan using the list in the navigation bar on the left side. The Benefits Modeling Tools cannot account for your personal situation such as lifestyle, family situation, and health care needs (today and in the future). Medical Plans Banner Health makes it a priority to provide you with health plans options that encourage and reward you to take personal accountability for your health. Banner Health offers three medical plans: Banner Health Savings Plan, Select 500 and Choice Plus. The Choice Plus plan is available only in Arizona, Colorado, Nebraska and Torrington, Wyoming. The main difference between our three medical plans is the monthly premiums and the amount you pay out of your pocket at the time of service. Each plan provides excellent coverage with preventive services at 100% coverage, no pre-existing condition exclusions and in and out-of-network benefits. It is important to use in-network providers whenever possible to pay the least amount out of your pocket. To locate providers in the Banner Health network for the Select and Choice Plus plans, access BannerHealthPlans.com and select Provider Search. Banner Health also offers the Banner Health Savings Plan, which qualifies for a Health Savings Account. Information about contracted providers for the Banner Health Savings plan, which includes the Blue Cross Blue Shield of Arizona network of providers, is located on www.azblue.com/bannerprovider. Pharmacy coverage is included in your medical plan election. The pharmacy coverage features one pharmacy network and one formulary (list of covered drugs); copay or coinsurance varies depending on the medical plan you select. Banner Health s pharmacy coverage is administered by MedImpact, and the network includes Banner Health Family Pharmacies that offer discount copays and coinsurance compared to regular prescription pricing at our network-contracted pharmacies. www.bannerbenefits.com 12

Medical Plans Banner Health Savings Plan Banner Health is excited to offer a medical plan that empowers you to build health savings through a lower premium coupled with a Health Savings Account (HSA). This plan encourages you to take a more active part in how you spend and save your health care dollars while managing your health. When reviewing this plan, you should look at this as a multi-year approach to health care that could have future savings advantages. The Banner Health Savings medical plan is a high deductible health plan which empowers you to build health savings through lower monthly premiums. We understand that an HSA-qualified plan may not be as familiar and may seem intimidating with a higher deductible than you are used to. However, the savings for you can be very significant. You may think you cannot afford to pay a higher deductible; however, these types of plans can be affordable due to the lower monthly contributions and Banner Health s contribution to your HSA account. It just takes a little research and understanding of how this plan works when it comes to your out-of-pocket health care expenses. All claims are administered by Blue Cross Blue Shield of Arizona (BCBSAZ), and the plan includes an expanded network made up of Banner Health providers and the BCBS provider network. Deductible A deductible is the amount you need to pay for medical and prescription services before your health plan begins to pay. Deductibles in the Banner Health Savings plan work very much the same as traditional medical plans but with higher dollar amounts. Once you have met your deductible, any claims incurred will be paid at the applicable coinsurance percent. Please see the Medical Summary Charts for in-network for in-network and out-of network coinsurance percentages. GO TO MEDICAL SUMMARY CHART When you visit physicians and other health care providers in-network, your deductible, coinsurance and out-of-pocket maximum will be lower. Your costs will be based on fees that have been negotiated in advance by your medical plan, and the savings can be substantial. Out-of-network providers will charge fees that have not been discounted. In addition, some may charge more than what the network will allow (otherwise known as reasonable and customary charges), and the plan does not provide any benefits for amounts above reasonable and customary charges. As with all our medical plans, we want you and your family to go for your annual physicals and other preventive care services. With the Banner Health Savings plan preventive care is covered at 100%, even if you haven t met your deductible. There are no out-of-pocket costs for services such as annual physicals, well-woman and well-child exams, preventive immunizations and diagnostic services such as mammograms, PAP tests, colonoscopies, and PSA tests. Medical Plans If you enroll dependents in coverage in the Banner Health Savings plan, you must meet the entire family deductible before the plan pays any benefits. For example, if only one member of your family needs medical care, that family member must incur the full family deductible before the plan begins to pay benefits. It is important to understand this point because in the Choice Plus and Select plans, each individual can meet the individual deductible for coinsurance to apply even if the family deductible has not been met. Coinsurance Coinsurance is your share of the costs for health care service once you have met your deductible. It is a percentage of the total charge for the service provided. Once you have met your deductible, any claims incurred will be paid at the applicable coinsurance percent. GO TO MEDICAL SUMMARY CHART Prescription Drug Coinsurance Under the Banner Health Savings plan, you are responsible for the full costs of prescriptions until you meet the annual deductible. Once you have met the annual deductible, your prescriptions are covered at the coinsurance amount. Then once the annual out-of-pocket maximum is reached the prescriptions will be covered at 100%. Banner Health offers employees enrolled in the Banner Health Savings medical plan access to a network of Banner Health Family Pharmacies and other contracted pharmacies and Banner Health Family Pharmacies. Out-of-Pocket Maximum The annual out-of-pocket maximum is the highest or total amount you will be required to pay towards the cost of your health care in the current plan year. It is there to protect you against large medical expenses. The deductible, coinsurance and copays apply to your out-of-pocket maximum. Once you reach your out-of-pocket maximum, Banner Health will cover 100% of the costs considered to be medically necessary. Please see the Medical Summary Charts for the out-ofpocket maximum. Many people are concerned that their out-of-pocket expenses will increase because of the deductible associated with these plans. Qualified HSA Expenses You can use your HSA to pay for qualified health care expenses incurred on the Banner Health Savings Plan. Any expenses that are covered under your medical plan will apply toward your annual deductible and out-of-pocket maximum. You can use your HSA for other qualified health care expenses too, such as dental or vision expenses, but these expenses will not apply toward your medical plan s deductible or out-of-pocket maximum. You can use the contributions Banner Health makes to your HSA to help pay your qualified out-of-pocket expenses. And, since the money in your HSA rolls If you use HSA money for anything other than over each year, many people will save money to use qualified health care expenses, you will have to pay for future qualified health care expenses or even for regular income tax and a 20% IRS penalty. Medical Plans Select 500 This medical plan is similar to the Choice Plus medical plan, however, payment by the plan is different. When you use services, you will be required to pay out of your own pocket until you meet the $500 deductible. If you cover yourself, your spouse and five children, and you and your spouse meet the deductible of $500 ($1,000 per family deductible) then the rest of the children have no deductible to meet if they are ill or injured and require medical services. Using an in-network provider for service is the best way to pay the least amount of out of pocket costs. Please review the Medical Summary Charts in this guide. Medical Plans Choice Plus Medical Plan The Choice Plus plan has the highest monthly premiums of all of our medical plans, however has the lowest deductibles. Unlike the Select 500 plan, which requires coinsurance, this plan uses co-payment or copay (a flat amount due) at the time you see a physician or seek other services. A co-payment or copay is a set amount of money determined by the plan each year that is due at the time of the appointment and Banner Health will pay all of the remaining costs (reasonable and customary charges apply) for that service over the co-payment. Co-payment for services varies. For example, under this medical plan it will cost $25 to see a Primary Care Physician (PCP) each time you visit their office, except for preventive services. If your PCP feels you require medical services from a specialist, your PCP must provide the specialist within the Banner Health Network with the needed referral. The co-payment to see a specialist is $50 for each visit. Should you require hospitalization during the calendar year under the Choice Plus medical plan, you are required to first pay a deductible of $250 for individual only. Once you have paid the deductible amount you would then be responsible to pay 15% of the hospitalization costs at a Banner Health Hospital. If you are hospitalized at a non-participating facility you would pay a $250 individual deductible and 30% of the hospitalization costs. If your total charges for all services reach $4,000 for the individual or $8,000 for your dependents, Banner Health will pay the remaining amount of the bill. Flexible Spending Accounts (FSAs) Flexible Spending Accounts Banner Health s Flexible Spending Accounts (FSAs) can help you save money. By paying for eligible health care and dependent care expenses for you or your qualified dependents with an FSA, you reduce your taxable income and pay no federal, state or Social Security taxes on the money used for those expenses. You can contribute from $100 up to $2,500 into the Health Care FSA or $5,000 in the Dependent Care FSA. Health Care FSA Save money and spread your out-of-pocket costs over the year with a Health Care FSA. You can use the Health Care FSA for eligible medical, pharmacy, dental and vision expenses for you and your dependents that are not covered by your plans. This includes the cost of copays, coinsurance, eyeglasses, contact lenses, orthodontia, chiropractic care, and eligible over-the-counter drugs. Dependent Care FSA (Day Care Expenses) You can use the Dependent Care FSA to pay eligible child care (up to age 13) and elder care expenses that you incur because you and your spouse work. Please note this account cannot be used for health care expenses for dependents. FSA Benefits Debit Card - If you enroll for a Flexible Spending Account for 2015, you will automatically receive a free Benefits Debit Card to present at the time you pay for eligible expenses. You pay no money up front, and you don t have to wait to be reimbursed. You may still need to mail or fax proof of your expenses, so be sure to keep your receipts. Flexible Spending Accounts (FSAs) How the FSAs work 1. You decide how much to contribute from $100 up to $2,500 into the Health Care FSA, or up to $5,000 in the Dependent Care FSA. 2. Your contributions are made through convenient payroll deductions throughout the calendar year on a pre-tax basis. 3. When you have an eligible expense, you may submit a claim form along with an itemized receipt or proof of the expense. You will be reimbursed with tax-free dollars through your choice of a mailed check or direct deposit. 4. You may use your Benefits Debit Card to pay for eligible expenses. This eliminates the need to submit the claim form for reimbursement because the amount will be deducted from your balance automatically. You may be required to submit your itemized receipts to show proof that the charges are on the approved list of eligible expenses. Health Care The total amount you choose to set aside for the Health Care FSA is immediately available to cover eligible expenses. For example, if your coverage begins Jan. 1 and you receive a new pair of eyeglasses in January, you can be reimbursed for the full cost as long as your annual contribution amount is enough to cover the full cost. Dependent Care Your claims for dependent day care expenses will be reimbursed only if there s enough money in your Dependent Care FSA. 5. You must use all of the money you contribute to the FSAs for expenses incurred during that same plan year, or you will lose the money left in your account at the end of the year. Flexible Savings Accounts Tips Health care expenses for your dependents are reimbursed from your Health Care FSA, not the Dependent Care FSA. Estimate carefully! You will forfeit any money left in your FSAs at the end of the year. Go back through your receipts from last year to help you estimate, or contact your providers for help in determining how much you ve spent. In some cases, the federal Child Care Tax Credit may provide a better tax break than the Dependent Care FSA, particularly if you are in a lower tax bracket. Consult a tax advisor to determine the best approach for you. For the Dependent Care FSA, special rules apply if you are married and your spouse is disabled or a full-time student, or if your spouse has set up a similar spending account with his/her employer. Wellness & Health Management Know your Numbers Earn 2015 Wellness Incentives As part of our ongoing commitment to employee wellness, personal accountability and health management, Banner Health offers an incentive in 2015 for completion of a Know Your Numbers biometrics screening. Banner Health has expanded eligibility for biometrics screenings to include spouses or domestic partners who are enrolled in one of the 2015 Banner Health medical plans. If your spouse/ domestic partner will be enrolled in your 2015 Banner Health medical plan, both of you must complete biometrics screenings in 2014 in order for any incentive to be paid, and all incentive dollars are payable to the Banner Health employee. You have two options for completing biometrics: Free screenings are offered at Banner Health locations system-wide by members of our ECHO team. Under this option, your screening, including your blood draw, must be completed by the end of Annual Enrollment. If you are enrolling for 2015 benefits during the Annual Enrollment period in the fall of 2014. Employees who are hired or become eligible after the annual enrollment period may complete screenings by Nov. 20, 2015; the sooner you complete the screening, the sooner you will receive the incentive. Life/AD&D Insurance Life/AD&D Insurance In the event of the death of your covered spouse or child(ren), benefit payments are made to you. In the event of your death, unless you choose a different beneficiary, benefit payments are made in the order listed below: Your spouse Your children (including legally adopted children, but excluding step-children) Your parents Your brothers and sisters Your estate If you want your life/ad&d benefits to be paid in the order listed, you DO NOT need to designate a beneficiary. However, if you wish to change the order of your beneficiaries or choose someone not listed, you must provide a beneficiary designation at the time you enroll online, or by calling the Service Center at Banner Health Plan Administration. If you and your spouse/domestic partner are both employed by Banner Health and eligible for benefits, you can t be covered as both an employee and a spouse/domestic partner for life insurance. Retirement Plans If you do not specify the investment funds when you enroll your contributions will be defaulted into the Fidelity Freedom Fund that is close to when you will turn 65. You are immediately 100% vested in all contributions to your account, which means you own the funds in your account (including the Banner Health match)! Roth 401(k) Highlights: Banner Health s 401(k) plan gives you the option to contribute post-tax dollars to a Roth 401(k). Contributions to the Roth 401(k) use post-tax dollars. You must contribute 4% pre-tax dollars to the 401(k) first before any percentage can be designated for post-tax contributions in the Roth 401(k). The Roth 401(k) contributions are NOT matched by Banner Health. Only the first 4% of the 401(k) pretax contribution is matched by Banner Health. The Roth 401(k) is NOT your typical IRA (Roth) plan. It is a component of the 401(k) plan offered through Banner Health. Since this is a component of the 401(k) it will not show as a separate line item on your statement or online at Fidelity. It is a combined total of your contributions to the plans. Any dollars you contribute during the current year are totaled with the 401(k) for the combined IRS allowed maximum contributions. However, the company match is not part of the annual limit. You can contribute to $18,000 if under 50 or $24,000 over 50 to the plan for 2015. IRS Annual Limits and Retirement Plans Enrollment IRS annual contribution limits for 2015 are $18,000 if you are below the age of 50, and $24,000 if you are age 50 or older on Dec. 31 of the plan year.
