Important Information about the Insurance This booklet contains your Citibank credit card complimentary benefits, which are effective for purchases where the final payment is made on or after 20 th May 2012 and are only available to cardmembers of eligible credit card accounts. These benefits are provided automatically to cardholders under the Master Agreement entered into between Citibank Berhad ( Citibank ), 165 Jalan Ampang, 50450 Kuala Lumpur and ACE Jerneh Insurance Berhad (9827-A) ( ACE Jerneh ), Level 26 Menara Weld, 76 Jalan Raja Chulan, 50200 Kuala Lumpur is the product issuer of the policies detailed in this booklet. In this booklet, ACE may also be expressed as ACE Jerneh, we, us or our. Although the benefits under the covers are automatically provided to cardmembers, cardmembers are not obliged to take these benefits. However, if a person wishes to claim these benefits, they will be bound by the Definitions, Terms and Conditions, Exclusions and Claims Procedures of the Policies. Therefore, please read the policy document carefully and keep it in a safe place. Please also keep detailed particulars and proof of any loss including the sales receipt and eligible credit card account statement showing any purchases. Citibank is not the product issuer (insurer) of these Policies and neither it nor any of its related corporations guarantee any of the benefits under these covers. These benefits are provided at no additional cost to the cardmembers and Citibank does not receive any commission or remuneration in relation to these benefits. Neither Citibank nor any of its related corporations are Authorized Representatives of ACE Jerneh or any of its related companies. 1.1. Termination of these covers Citibank may terminate the benefits under any one or all of the covers in this document for all cardmembers or an individual cardmember, and if so, will notify primary cardmember of the termination in writing. Purchases finalised before expiry of the notification will still be eligible for cover. However, purchases finalised after expiry of the notification will not be eligible for cover. 1.2. Privacy ACE Jerneh collects personal information about you in order to assess claims or to assess an application for cover for a pre-existing medical condition. If you do not provide the requested information, the assessment of a pre-existing medical condition cannot be considered, a claim may be delayed or we may not accept a claim. We will, in relevant cases, give personal information (other than sensitive information such as health information to Citibank, its service providers and business partners so that they can monitor the claims service we provide, to prevent fraud and to ensure eligibility for cover. We may also give personal information including sensitive information, such as health information, to our service providers (including medical practitioners, other health professionals, reinsurers, legal representatives and loss adjusters). By providing personal details, you agree to those organisations collecting and disclosing personal and sensitive information for the purposes listed above. We may also be allowed and required by law to disclose personal information about you, for example under Court orders or Statutory Notices pursuant to Malaysian Law. 1.3. Complaints If you have a complaint about the covers or about the service you have received from ACE Jerneh, including the settlement of a claim, please call us on 1800-88-2393. We will respond to your complaint within 7 working days provided we have all necessary information and have completed any investigation required. If more information or investigation is required we will agree reasonable alternative timeframes with you.
If you are unhappy with our response or cannot agree on reasonable timeframes with us, the complaint will be registered as a dispute and it will be reviewed by our internal dispute resolution process which is free of charge. We will respond to your dispute within 15 working days provided we have all necessary information and have completed any investigation required. If more information or investigation is required, we will agree reasonable alternative timeframes with you, We will keep you informed of the progress of our review at least every 10 working days and give you our response in writing. If you are unhappy with our response and cannot agree on reasonable timeframes with us, you can refer the matter to the Customer Services Bureau, Insurance Regulation Department at Bank Negara Malaysia (Tel: 03-2698 8044), Fax: 03-2694 5986) or the Financial Mediation Bureau (Tel: 03-2272 2811, Fax: 03-2274 5752). 1.4. Excess what you contribute to a claim Excesses may apply to certain sections of cover. An excess is not an additional fee, charged by us at the time of making a claim. Rather, it is the uninsured first portion of a loss for which you are otherwise covered (i.e. the amount that you must contribute towards each claim). Details of the excess amounts and circumstances in which they will be applied are set out below: Home Away Contents An excess of 1% of sum insured on each and every loss is applied from the total loss payable to you. Purchase Protection An excess of 50% of the loss subject to a minimum of RM75.00 is applied in respect of accidental loss or damage including Theft (other than Burglary, Robbery and Hold-Up) from the total loss payable to you. We shall also deduct RM100.00 in respect of any other loss (including but not limited to Burglary, Robbery and Hold-Up) from the total loss payable to you. Overseas Medical An excess of RM500.00 is applied, up to 5% of Sum Insured for follow-up treatment on each and every loss is applied from the total loss payable to you. Travel Cancellation An excess of 35% on each and every loss is applied from the total loss payable to you. Wallet Guard An excess of RM75.00 on each and every loss is applied from the total loss payable to you. 1.5. Repairing or replacing damaged property/personal goods If an item is damaged, lost or stolen we may choose to: repair the item; replace the item, less depreciation. This means we will replace the item for an amount equal to its original cost, less a depreciation figure which takes into account its age and condition; or pay you the amount it would cost us to replace the item less depreciation Where the item is part of a pair or set, you will receive no more than the value of the particular part or parts lost, stolen or damaged, regardless of any special value that the item may have by way of being part of such pair or set. 1.6. Safety of your property/personal goods You must take all adequate and reasonable precautions (considering the value of the items) to protect your property/personal goods and you are not covered if you do not take reasonable precautions (considering the value of the items ) to protect your property/personal goods. Property/personal good(s) is/are not covered under any of the insurances if left: unattended in a public place; or unattended in an unlocked motor vehicle; or unattended in a motor vehicle overnight; or behind, forgotten or misplaced; or with a person who steals or deliberately damages them A public place includes, but is not limited to shops, airports, bus depots, buses, planes trains, streets, hotel foyer, restaurants, beaches, public toilets, car parks, office areas, behind counters, housing and hostel common areas, unlocked hostel and hotel rooms and any place which is accessible to the public. 1.7. Reporting lost, stolen or damaged items/personal goods In the event that your property/personal goods are stolen, damaged or accidentally lost, you must make a report to the Police or the relevant authority. You must do this within 24 hours of
learning of the theft, loss or damage and the report must list and describe the missing or damaged items/personal goods. If the loss or damage occurs overseas, a copy of this report must be obtained and the authority must sign the copy and write on it that it is a true and accurate copy of the original. 1.8. Pre-existing medical conditions The insurances do not cover you for any event that is caused by or arises as a result of a preexisting medical condition of yours or any pre-existing medical condition of a relative, travel companion or any other person that may give cause for you to claim. In regard to pregnancy, we do not insure you for any expenses that arise due to the normal development and consequences of pregnancy, including, but not limited to, regular or routine medical consultations and tests (such as ultrasounds) and the childbirth itself. 1.9. Please also note: The international travel insurance cover is for a maximum period of 30 days and cannot be extended, and you do not have to advise us that you will be traveling as you are automatically covered, provided you are eligible for this cover and adhere to the definitions, terms and conditions, exclusions and claims procedures of the policy. 1.10. ACE Assistance s team of medical professionals is only a phone call away and is available 24 hours a day, 7 days a week for advice and assistance in the event of a medical emergency and any associated problems for travellers outside Malaysia. 1.11. Insurance exclusions what is not covered In any insurance policy there are situations that are not covered. We are not able to insure some situations because of the costs or types of events involved. In addition to any Additional Exclusions contained in particular benefit in the policy, the following exclusions apply to all the benefits in the policy. War, invasion, act of foreign enemy, hostilities (whether war be declared or not), civil commotion, civil war, riot, revolution, insurrection or military or usurped power; Engaging in military duty with any armed forces of any country or international authority; Suicide, or any attempts thereat, suicide pacts or agreement, while sane or insane, deliberate self-inflicted injury; Engaging in pot-holing, professional sport where the Insured Person s livelihood is substantially dependent on income received as a result of playing sport, racing other than on foot or engaging in any motor sports as a rider, driver and/or a passenger; Engaging in aviation (other than as a fare-paying passenger on a Common Carrier); Engaging in any aerial activity, including parachuting and hang-gliding, except as a passenger in any properly licensed aircraft; Criminal and illegal acts by you or your executors, administrators, legal heirs or personal representatives; Under the influence of intoxicating liquor, including having a blood alcohol content over the prescribed legal limit whilst driving, or being under the influence of any other drug unless it was prescribed by a Doctor and taken in accordance with the Doctor s advice; Nuclear reaction, radiation, or radioactive contamination; Any condition which is, results from or is a complication of infection with a venereal disease; Any condition which is, results from or is a complication of congenital conditions or deformities; Any condition which is, results from a complication of geriatric or psycho-geriatric or psychiatric condition, stress, anxiety and depression; Any condition which is, results from or is a complication of pregnancy, childbirth, miscarriage or abortion; Acupuncture treatment; Pre-existing Medical Conditions; Illness, disease, bacterial or viral infection, even if contracted by accident, other than bacterial infection that is the direct result of an accidental cut or wound or accidental food poisoning.
Travelling contrary to the advice of a Doctor or for the purpose of obtaining medical treatment; In respect of any property specifically insured or any claim which but for the existence of this insurance would be recoverable under any other insurance; Accidental Injury or illness occuring on or after the thirty (30) day of the Cardmembers Journey Any condition which is or result from or is complication of infection with Human Immunodeficiency Virus ("HIV"), any variance including Acquired Immune Deficiency ("AIDS") and AIDS Related Complications ("ARC") or any opportunistic infections and/or malignant neoplasm (tumour) found in the presence of HIV, AIDS or ARC 2. The cover that we provide 3.1. Overseas Personal Accident 3.1.1. Accidental Death We will cover you, whilst Overseas you die as a result of accidental injury (not illness). The death must occur with 365 days from the date of the accident whilst Overseas and the accident must have been caused by violent, external and visible means and must be supported by a death certificates. 3.1.2. Permanent Disablement We will cover you, whilst Overseas you suffer one of the Permanent Disability shown in the Table of Benefits below as a result of accidental injury (not illness). The Permanent Disabilities must occur with 365 days from the date of the accident whilst Overseas and the accident must have been caused by violent, external and visible means and must be supported by a medical report by a qualified and registered medical practitioner. Overseas Medical We insure you for the necessary medical expenses as a direct result from accidental injury and sickness whilst you are on Journey Overseas. We will only cover your medical expenses if: you incur them overseas, during your journey overseas; and you are legally responsible for paying them; and you show us a medical advice, written by a qualified and registered medical practitioner, as proof of your illness or injury and the treatment you need for it; and we assesses your medical expenses are reasonable in amount and reasonably necessary.
3.3. Emergency Medical Evacuation We will indemnify ACE Assistance for all emergency medical evacuation expenses to the nearest hospital where appropriate medical care and facilities are available or back to Malaysia in the event you suffer a life-threatening medical condition as a result of accidental injury or sickness, provided that such emergency medical evacuation is: certified by a qualified and registered medical practitioner to be necessary; and organised by ACE Assistance Please remember that you can only claim for emergency medical evacuation if it is arranged by ACE Assistance (by phoning the operator where you are and booking a reverse charge call to Malaysia +603-7628 3703). 3.4. Travel Cancellation ACE Jerneh covers you for the loss of irrecoverable deposits or charges paid in advance or contracted to be paid as a result of the Journey being cancelled due to the following reasons: unforeseeable accidental injury, sickness or death of you. The accidental injury or sickness as certified by a qualified and registered medical practitioner must be so disabling as to reasonably cause a Journey to be delayed, cancelled or interrupted. unforeseeable circumstances, which are those events or consequences of such events, other than accidental injury, sickness or death, which could not have been reasonably foreseen or expected by you, and beyond your control. 3.5. Compassionate Visit We will pay for up to 2 relatives or friends to where you are if you sustain serious accidental injury or sickness or death whilst on journey overseas which has lasted or likely to last for more than 5 days, as certified in writing by a qualified and registered medical practitioner. We will pay for: travel expenses accommodation expenses 3.6. Missed Flight Connection If your confirmed onward connecting scheduled flight is missed at the transfer point due to late arrival of your incoming confirmed connecting flight and no alternative transportation is made available to you after 4 hours of the actual arrival time of you incoming flight, we will reimburse you the costs for: Meals, refreshments, hotel accommodation, telephone charges and travel expenses incurred to and fro between the airport and the hotel Essential clothing and requisites if your baggage has been checked in This benefit is only payable when you supply the receipts for the expenses incurred and written confirmation from the airline confirming the delay duration. 3.7. Flight Delay If the departure of any scheduled aircraft in which you have arranged to travel is delayed for at least 4 hours due to adverse weather conditions or mechanical breakdown/derangement or due to grounding of an aircraft as a result of mechanical or structural defect we will reimburse you the costs for: Meals, refreshments, hotel accommodation, telephone charges and travel expenses incurred to and fro between the airport and the hotel Essential clothing and requisites if your baggage has been checked in This benefit is only payable when you supply the receipts for the expenses incurred and written confirmation from the airline confirming the delay duration and the reason for such delay. 3.8. Baggage Delay If your baggage that is accompanied and checked in with the airline is not delivered to you within 6 hours of your arrival at the scheduled destination, we will reimburse you the costs for the purchase of emergency essential clothing and requisites. This benefit is only payable when you supply the receipts for the expenses incurred and written confirmation from the airline confirming the delay duration. 3.9. Baggage Loss & Damage We insure you during your journey overseas for the accidental loss or damage to your baggage. We will also reimburse you the costs for the purchase of emergency essential clothing and requisites if your baggage that is accompanied and checked in with the airline is not delivered to you within 24 hours of your arrival at the scheduled destination. 3.10. ACE Assistance You are entitled to obtain assistance from ACE Assistance in respect of the following: a) 24-hour Telephone Access
If The Insured Person require assistance, The Insured Person may make call collect to the dedicated 24-hour Hotline with trained multi-lingual personnel including a panel of ACE Assistance appointed physician for immediate assistance and advice. b) Medical Assistance i) Medical Service Provider Referral ACE Assistance shall provide to the Insured Person, upon request, the name, address, telephone number and, if available, office hours of physicians, hospitals, clinics, dentists, and dental clinics. ii) Arrangement of Appointments with Local Physicians for Treatment ACE Assistance will assist The Insured Person to arrange for appointments with general practitioners or specialised Physicians. c) Travel Assistance i) Inoculation and Visa Requirement Information ACE Assistance shall provide information concerning visa and inoculation requirements for foreign countries, as those requirements are specified from time to time in the most current edition of World Health Organization Publication Vaccination Certificates Requirements and Health Advice for International Travel (for inoculations) and the ABC Guide to International Travel Information (for visas). This information will be provided to the Insured Person at any time, whether or not the Insured Person is the one travelling or an emergency has occurred. ii) Interpreter Referral ACE Assistance will provide the names, telephone numbers and, if possible and requested, hours of opening of interpreters office in foreign countries. iii) Lost Luggage Assistance ACE Assistance will assist the Insured Person if the Insured Person s luggage is lost while travelling outside Malaysia by referring the Insured Person to the appropriate authorities involved. iv) Lost Passport Assistance ACE Assistance will assist the Insured Person if the Insured Person s passport is lost while travelling outside Malaysia by referring the Insured Person to the appropriate authorities involved. v) Embassy Referral ACE Assistance shall provide the address, telephone number and hours of opening of the nearest appropriate consulate and embassy worldwide. vi) Weather and Foreign Exchange Information Services ACE Assistance shall provide information to the Insured Person regarding the exchange rate of major foreign currencies and weather forecasts and temperatures of foreign countries. vii) Emergency Message Transmission Assistance In the event of an emergency or a hospital confinement, ACE Assistance will undertake to keep the Insured Person s immediate family members informed, upon the Insured Person s or the Insured Person s Travelling Companion s request and consent to do so. viii) Legal Referral ACE Assistance will provide the Insured Person with name, address, telephone numbers, if requested by the Insured Person and if available, office hours for referred lawyers and legal practitioners. 3.11. Home Away Contents We will pay you in the event you suffer a physical loss or damage caused by burglary to your household contents within your residence in Malaysia that was left vacant whilst on journey overseas. 3.12. Purchase Protection We will indemnify you for any personal goods purchased worldwide if such personal goods is accidentally lost or damaged within: 30 days from the date of purchase; or 30 days from the date of delivery for items purchased under a merchandising pro gram provided the cost of purchase is charged to an eligible card account. If a claim is to be paid under this cover, We may choose to: repair the item; reinstate or replace the item whether wholly or in part. 3.13. Extended Warranty The cover extends the manufacturer s warranty on any participating products stated below provided the purchase is charged to an eligible card account. This cover does not affect the rights of cardmember against a manufacturer and the cover comes into effect at the end of the manufacturer s warranty period and shall equal the term of the original manufacturer s warranty, up to a maximum of 24 months.

Heater This Extended Warranty only covers the failure of the participating products to operate for the purpose for which they were designed as a result of a breakdown, malfunction or defect, provided the breakdown, malfunction or defect is covered by the terms of the original warranty. The liability of ACE Jerneh for claims made pursuant to this insurance shall not exceed: The actual purchase price of the participating product charged to an eligible card account (not more than RM2,500.00 per item, up to RM5,000.00 per occurrence) In any 12-month period the sum of RM15,000.00 per eligible card account If a claim is to be paid under this cover, We may choose to: repair the item; replace the item, less depreciation (10% for each year or part thereof). This means we will replace the item for an amount equal to its original cost, less a depreciation figure which takes into account its age and condition; or pay you the amount it would cost us to replace the item less depreciation (10% for each year or part thereof). 3.14. Refund Protection We will provide cover for product satisfaction on designated items purchased solely for personal use and is charged entirely to an eligible card account 3.15. Wallet Guard We will pay you if you sustain any loss or damage to Personal Effects, Cash, Valuable Documents and Wallet as a result from theft, burglary, robbery or stealing whilst outside your place of residence where force or violence or threatened are used.
