School Year 20-20 Name School WICHITA FALLS INDEPENDENT SCHOOL DISTRICT EDUCATIONAL DIAGNOSTICIAN SCHOOL PSYCHOLOGIST SUMMATIVE EVALUATION REPORT Formative evaluation will occur throughout the school year in both formal and informal observations by the Director of Special Education in consultation with the appropriate Support Specialist and Building Principal. The Director of Special Education will annually complete the evaluation form using a ( ) with appropriate documentation.
