1 Critical Illness Plan A few of us have actually plan for medical expenses. However, with the rise in medical inflation, one can easily incur medical expenses up to ten-thousands of ringgit. The truth is, completely unforeseen and unavoidable medical expenses can sometimes dry up our savings. 1. What is MCLR? MCLR is a critical illness insurance plan that covers you for a wide range of critical illnesses benefits. It allows you to make multiple critical illness including Cancer claims. You may claim up to three (3) events of critical illness diagnosis including up to two (2) events of Cancer, subject to the terms and conditions of the rider. BENEFITS 2. What are the covers / benefits provided? This plan provides the following coverage: Critical illnesses claims of up to three (3) events from different critical illness groups (as shown in the table below). No benefits are payable for more than one (1) critical illness from the same group except for Cancer. The 3 events include Cancer claims within Group 6 up to two (2) Cancer events. Payment of 125% of sum assured for each Cancer claim up to 2 events. Waiver of future premiums of MCLR upon diagnosis of the first critical illness a. The Critical Illness groups are as follows: Group Payable of Sum Assured 1 10% (Max RM25,000). The balance can be claimed from another critical illness event from Group 1 100% 2 100% 3 100% 4 100% Coverage 1. Angioplasty and Other Invasive Treatments for Major Coronary Artery Disease b (this is not covered under waiver of premium) 2. Coronary Artery By-Pass Surgery 3. Heart Attack 4. End Stage Kidney Failure 5(a). Major Organ Transplant c (Kidney) 6. Stroke 7. Other Serious Coronary Artery Disease 8. Medullary Cystic Disease 9. ELIGIBILITY 3. Am I eligible to apply for MCLR? This plan is available to individuals from 4 weeks to 60 years of age; subject to the approval of our underwriters. PREMIUMS 4. How much of premium do I have to pay? MCLR is available as a rider attachable to a basic plan. Hence, the premium will vary depending on the type of basic plan that you are purchasing. Below are some indicative premium rates: Indicative premium (standard life) per thousand sum assured Male, non-smoker 35 45 MCLR expiry at age 70 attachable to - Basic Term to age 70* RM11.35 RM20.32 MCLR expiry at age 85 attachable to - Basic Ordinary Life Plan* RM16.22 RM29.09 MCLR attachable to Investment linked plan** RM1.31 RM3.96 * Premium rates for premium paying riders depend on age at issue date of your policy ** Cost of insurance for unit deducting rider depends on the attained age when cost of insurance is deducted. Please refer to the Sales Illustrations as provided by the agent if your age, sex, smoker status, health status and plan to which this rider is attached to are different from those illustrated above.

3 5. What is the premium paying term for MCLR? Premium paying term for : i) MCLR attachable to the Basic Term to age 70 is at age 70 ii) MCLR attachable to Basic Ordinary Life plan is at age 85 iii) MCLR attachable to Investment linked plan is at age 85 6. Is the premium for MCLR guaranteed? MCLR premium rates are not guaranteed and the Company reserves the right to revise the rates upon renewal. One of the possible factors contributing to the increase in premium rates is claim experience. The possible factors contributing to premium revision and renewal above are not exhaustive and the premium rates may be reviewed under other justified circumstances. The Company will give ninety (90) days prior notice in writing by ordinary post to your last known address in the Company s records if such revision takes place. Past trends on the increase in premium rates do not necessarily reflect the future trends. For unit deducting versions of the products, the insurance coverage charges that are deducted monthly from the value of your units will be charged. The insurance charges will increase as you grow older. Details of insurance charges are given in the sales illustration. 7. Can my premiums increase as I age? a) For MCLR attachable to the Basic Term to age 70 and Basic Ordinary Life plans, the premium rates are level, based on the age at issue age of your policy. b) For MCLR attachable to the Investment linked plan, the cost of insurance charged will depend on your attained age when cost of insurance is deducted. FEES & CHARGES 8. What are the fees and charges that I have to pay? There are no fees and charges other than the premiums payable. TERMS & CONDITIONS 9. What are some of the key terms and conditions that I should be aware of? Importance of disclosure you must disclose all material facts such as any medical conditions, and state your age correctly. Free-look period you may cancel your policy by returning us the policy contract and giving us a written notice that has been signed by you. Both written notice and policy contract must be received by us within 15 days after you have received the policy contract. The premium that you have paid (less any medical fee incurred) will be refunded to you. Waiting period - you will have to wait for a specified duration (from the commencement date of your policy or last reinstatement date) before your coverage takes effect: i) 60 days for Cancer, Heart Attack, Coronary Artery By-Pass Surgery, Other Serious Coronary Artery Disease, Angioplasty and Other Invasive Treatments for Major Coronary Artery Disease. ii) 30 days for other critical illnesses. iii) Twelve (12) months between diagnosis of each subsequent critical illness except for Angioplasty and Other Invasive Treatments for Major Coronary Artery Disease. iv) Five (5) years between the date of the first Cancer deemed to be in remission as confirmed by Physician and the date of diagnosis of second Cancer. Unless renewed, the coverage will cease on expiry date and the insurance company shall strictly not be liable for any expense that takes place after the expiry date.

4 10. Is there a survival period before a claim is paid out? Yes, you will have to survive for at least 30 days after the diagnosis of the Critical Illness before the claim is payable. EXCLUSIONS 11. What are the major exclusions for MCLR? MCLR does not cover the critical illness events if: i. The signs or symptoms of the critical illness are manifested prior to or within sixty (60) days of the date your policy begin or last reinstatement date for Cancer, Heart Attack, Coronary Artery By-Pass Surgery, Other Serious Coronary Artery Disease, Angioplasty and Other Invasive Treatments for Major Coronary Artery Disease; ii. The signs or symptoms of the critical illness are manifested prior to or within thirty (30) days of the date your policy begin or last reinstatement date for all other critical illnesses; iii. The critical illness arises from a pre-existing condition which existed prior to the date of purchase of this rider; iv. The critical illness is diagnosed due to a congenital defect or disease, which was manifested or was diagnosed before attainment of age seventeen (17); v. Cancer from the following conditions: a. All cancers which are histologically classified as pre-malignant; non-invasive; carcinoma in-situ; having either borderline malignancy; or having low malignant potential b. All tumours of the prostate, thyroid and urinary bladder histologically classified as T1N0M0 (TNM classification) c. Chronic Lymphocytic Leukemia less than RAI Stage 3 d. All cancers in the presence of HIV e. Any skin cancer other than malignant melanoma vi. Brain Surgery from the following conditions: a. Burr hole procedures, transphenoidal procedures and other minimally invasive procedures b. Brain surgery as a result of an accident vii. The critical illness or surgery is caused by a self-inflicted injury; viii. The critical illness that is caused directly or indirectly by AIDS or HIV infection except for HIV due to blood transfusion; ix. Diagnosis of Alzheimer s Disease/Irreversible Organic Degenerative Brain Disorders, Parkinson s Disease and Loss of Independent Existence will cease to be covered critical illnesses after age sixty-five (65). This list is not exhaustive. Please refer to the policy contract for the full list of exclusions of MCLR. COVERAGE 12. When does my coverage start? Your coverage starts from the date on which your completed and the signed application form is approved by us and when we have received the full premium due. CANCELLATION 13. Can AIA cancel my policy? No.

5 14. Can I cancel my policy? Buying a life policy is a long-term financial commitment. if you do not pay your premiums within the grace period, your policy may lapse. You may cancel your policy at any time by writing to us if you haven t made a claim yet. No cash surrender value will be paid upon cancellation of this rider g REINSTATEMENT 15. Can I reinstate my policy after it has lapsed? Yes, you may reinstate your policy subject to the company s terms and conditions. You may be required to provide health evidence to prove your insurability at the time of reinstatement. CLAIMS 16. How do I make a claim? You will need to complete the claim documents and provide us with the necessary doctor s reports as a proof of critical illness diagnosis. Claim forms can be obtained from your AIA Agent, by calling the AIA Customer Service Hotline at 1300-88-1899 or by visiting any AIA branch. 17. How does the Cancer claim work? You are allowed up to two (2) Cancer claims within Group 6, which will be considered as two (2) out of the three (3) critical illness events. The amount payable for a Cancer claim will be 125% of the sum assured. After the first Cancer claim is made, all other illnesses in Group 6 will be dropped while Cancer remains to allow a second Cancer claim. A 5-year waiting period e between Cancer claims is also required. The second Cancer claim cannot be inside the same site or same organ as the first Cancer. For the organs that come in pairs, only the first cancer event claim on the organ may be made. As an example, if the cancer claim is made on one lung, no further claims can be made for Cancer on either of the two lungs, including the healthy lung. 18. How does the Angioplasty and Other Invasive Treatments for Major Coronary Artery Disease claim work? A payment of 10% of the sum assured up to a limit of RM25,000 will be payable for Angioplasty and Other Invasive Treatments for Major Coronary Artery Disease. For any subsequent claim of Critical Illness from Group 1, the sum assured payable will be reduced by the claim made for Angioplasty and Other Invasive Treatments for Major Coronary Artery Disease. The full sum assured will still be payable for other critical illness claims from other groups (subject to applicable limits set out herein). No claim can be made for Angioplasty and Other Invasive Treatments for Major Coronary Artery Disease if there is a prior claim from Group 1. A claim for Angioplasty and Other Invasive Treatments for Major Coronary Artery Disease will not be eligible for premium waiver and can only be made once. The claim does not count as a critical illness event. 19. How does the Major Organ Transplant C work? You can only claim for a major organ transplant once. Subsequent claims cannot be made for major Organ Transplant from any one of the four Major Organ Transplant groupings (Group 1, 2, 4 and 6) regardless of whether it is from the same or a different critical illness group.

6 For example, if you claim for Major Organ Transplant (Heart) under Group 2, you cannot subsequently claim for Major Organ Transplant even if it is for a different organ such as Kidney. 20. What other documentation is required to be submitted together with the claim? In addition to the relevant claims forms, we need additional medical reports (as listed at the back of the claims form) to support the claim. 21. When do I have to notify AIA of a claim? 7 Pelan Penyakit Kritikal Ada segelintir sahaja daripada kita yang merancang untuk perbelanjaan perubatan.tetapi, dengan kenaikan inflasi perubatan, perbelanjaan perubatan seseorang boleh mencecah puluhan ribu ringgit. Hakikatnya, perbelajaan perubatan yang langsung tidak dijangka dan tidak dapat dielakkan adakalanya boleh mengeringkan simpanan kita. 1. Apakah itu MCLR? MCLR adalah pelan insurans penyakit kritikal yang memberikan anda perlindungan untuk pelbagai jenis penyakit kritikal. Ia membolehkan anda membuat tuntutan penyakit kritikal beberapa kali termasuk tuntutan untuk penyakit Kanser. Anda boleh membuat tuntutan sehingga tiga (3) peristiwa diagnosis penyakit kritikal termasuk sehingga dua (2) peristiwa Kanser, tertakluk kepada terma dan syarat rider ini. MANFAAT 2. Apakah perlindungan / manfaat yang disediakan? Pelan ini menyediakan perlindungan berikut: Tuntutan penyakit kritikal sehingga tiga (3) peristiwa dari kumpulan penyakit kritikal yang berlainan (seperti yang ditunjukkan dalam jadual di bawah). Manfaat tidak akan dibayar untuk lebih daripada satu (1) penyakit kritikal dari kumpulan yang sama kecuali Kanser. Tiga peristiwa tersebut termasuk sehingga dua (2) peristiwa tuntutan Kanser dalam Kumpulan 6. Bayaran sebanyak 125% daripada jumlah yang diinsuranskan bagi setiap tuntutan Kanser sehingga 2 peristiwa. Pelan ini boleh disertai oleh individu yang berumur di antara 4 minggu hingga 60 tahun, tertakluk kepada kelulusan penaja jamin kami. PREMIUM 4. Berapakah premium yang perlu saya bayar? MCLR disediakan sebagai rider yang boleh dilampirkan pada pelan asas. Oleh itu, premiumnya berbeza dan bergantung kepada jenis pelan asas yang anda beli. Berikut adalah beberapa kadar premium sebagai penunjuk: Premium penunjuk (hayat standard) bagi setiap seribu jumlah diinsuranskan Lelaki, bukan perokok 35 45 MCLR tamat tempoh pada umur 70 tahun boleh dilampirkan pada - Pelan Bertempoh Asas hingga umur 70 tahun* RM11.35 RM20.32 MCLR tamat tempoh pada umur 85 tahun boleh dilampirkan pada - Pelan Hayat Biasa Asas* RM16.22 RM29.09 MCLR boleh dilampirkan pada pelan berkaitan Pelaburan** RM1.31 RM3.96 * Kadar premium bagi rider pembayaran premium bergantung pada umur pada tarikh polisi anda dikeluarkan. ** Kos insurans bagi rider potongan unit bergantung kepada umur tercapai semasa kos insurans itu dipotong. 9 5. Apakah tempoh bayaran premium untuk MCLR? Tempoh bayaran adalah seperti berikut: i) MCLR yang dilampirkan pada Pelan Bertempoh Asas hingga umur 70 tahun adalah pada umur 70 tahun ii) MCLR yang dilampirkan pada Pelan Hayat Biasa Asas adalah pada umur 85 tahun iii) MCLR yang dilampirkan pada Pelan Berkaitan Pelaburan adalah pada umur 85 tahun 6. Adakah premium bagi MCLR dijamin? Kadar premium MCLR adalah tidak dijamin dan Syarikat berhak menyemak kadar ini semasa pembaharuan. Salah satu faktor yang boleh menyumbang kepada kenaikan kadar premium ialah pengalaman tuntutan. Faktor-faktor yang boleh menyumbang kepada semakan premium dan pembaharuan di atas adalah tidak terperinci dan kadar premium boleh dikaji semula di bawah keadaan lain yang wajar. Sekiranya semakan tersebut dibuat, Syarikat akan menghantar notis bertulis melalui pos biasa ke alamat terakhir anda yang diketahui di dalam rekod Syarikat, sembilan puluh (90) hari sebelum semakan tersebut dibuat. Trend kenaikan kadar premium pada masa yang lepas tidak semestinya menggambarkan trend pada masa akan datang. Bagi rider potongan unit, caj perlindungan insurans yang dipotong setiap bulan daripada nilai unit anda akan dikenakan. Caj insurans juga akan meningkat selaras dengan peningkatan umur anda. Butir-butir caj insurans diberikan dalam ilustrasi jualan. 7. Bolehkah premium saya meningkat mengikut umur saya? a) Bagi MCLR yang boleh dilampirkan pada pelan Bertempoh Asas hingga umur 70 tahun dan pelan Hayat Biasa Asas, kadar premium adalah sama, berdasarkan umur pada tarikh polisi anda dikeluarkan. b) Bagi MCLR yang boleh dilampirkan pada pelan berkaitan Pelaburan, kos insurans yang dikenakan akan bergantung pada umur tercapai anda semasa kos insurans itu dipotong. YURAN DAN CAJ 8. Apakah yuran dan caj yang perlu saya bayar? Tiada yuran dan caj lain selain jumlah premium yang dibayar. TERMA DAN SYARAT 9. Apakah antara terma-terma dan syarat-syarat penting yang perlu saya ketahui? Kepentingan pendedahan anda mestilah mendedahkan semua fakta penting seperti keadaan kesihatan dan umur anda dengan betul. Tempoh tengok percuma anda boleh membatalkan polisi anda dengan memulangkan kontrak polisi anda berserta dengan notis bertulis yang anda tandatangani kepada pihak syarikat. Notis bertulis serta kontrak polisi tersebut perlu diterima terus oleh kami dalam masa 15 hari selepas anda menerima kontrak anda. Premium yang anda telah bayar (ditolak sebarang yuran perubatan yang ditanggung) akan dipulangkan kepada anda. Tempoh menunggu - anda perlu menunggu untuk tempoh masa tertentu (dari tarikh mula kuat kuasa polisi anda atau tarikh pengembalian semula yang terakhir) sebelum perlindungan anda berkuat kuasa: i. 60 hari bagi Kanser, Serangan Jantung, Pembedahan Pintasan Arteri Koronari, Penyakit Arteri Koronari Lain yang Serius, Angioplasti dan Rawatan Pembedahan Lain untuk Penyakit Arteri Koronari Utama. ii. 30 hari untuk penyakit kritikal lain. iii. Dua belas (12) bulan di antara diagnosis setiap penyakit kritikal yang kemudiannya kecuali Angioplasti dan Rawatan Pembedahan Lain untuk Penyakit Arteri Koronari Utama.

10 iv. Lima (5) tahun di antara tarikh Kanser pertama dianggap telah berkurangan atau hilang gejala-gejala utamanya untuk sementara (remisi) seperti yang disahkan oleh Pakar Perubatan dan tarikh diagnosis Kanser kedua. Melainkan diperbaharui, perlindungan akan berhenti pada tarikh tamat tempoh dan syarikat insurans sama sekali tidak akan dipertanggungjawabkan terhadap sebarang perbelanjaan yang berlaku selepas tarikh tamat tempoh itu. 10. Adakah terdapat tempoh terus hidup yang dikenakan sebelum tuntutan dibayar? Ya, anda perlu hidup selama sekurang-kurangnya 30 hari selepas diagnosis Penyakit Kritikal sebelum tuntutan boleh dibayar. PENGECUALIAN 11. Apakah pengecualian-pengecualian utama bagi MCLR? MCLR tidak melindungi peristiwa-peristiwa penyakit kritikal jika: i. Tanda atau gejala penyakit kritikal itu kelihatan sebelum atau dalam tempoh enam puluh (60) hari dari tarikh polisi anda bermula atau tarikh pengembalian semula yang terakhir bagi Kanser, Serangan Jantung, Pembedahan Pintasan Arteri Koronari, Penyakit Arteri Koronari Lain yang Serius, Angioplasti dan Rawatan Pembedahan Lain untuk Penyakit Arteri Koronari Utama; ii. Tanda atau gejala penyakit kritikal itu kelihatan sebelum atau dalam tempoh tiga puluh (30) hari dari tarikh polisi anda bermula atau tarikh pengembalian semula yang terakhir bagi semua penyakit kritikal lain; iii. Penyakit kritikal yang timbul daripada keadaan sedia ada yang wujud sebelum tarikh iv. pembelian rider ini; Penyakit kritikal itu didiagnosis akibat kecacatan atau penyakit kongenital yang telah kelihatan atau didiagnosis belum mencapai umur tujuh belas (17) tahun; v. Kanser daripada keadaan berikut: a. Semua kanser yang secara histologinya diklasifikasikan sebagai pramalignan, tidak menyerang; karsinoma in situ; sama ada mempunyai risiko hampir malignan atau mempunyai potensi malignan yang rendah b. Semua tumor prostat, tiroid dan pundi kencing yang secara histologinya diklasifikasikan sebagai T1N0M0 (klasifikasi TNM) c. Leukemia Limfositik Kronik yang kurang daripada RAI Tahap 3 d. Semua kanser akibat daripada HIV e. Sebarang kanser kulit selain daripada melanoma malignan vi. vii. viii. ix. Pembedahan Otak daripada keadaan berikut: a. Prosedur Lubang Burr, prosedur transfenoidal dan prosedur pembedahan lain yang minimum b. Pembedahan otak akibat kemalangan Penyakit kritikal atau pembedahan itu disebabkan oleh kecederaan diri yang disengajakan; Penyakit kritikal yang disebabkan secara langsung atau tidak langsung oleh AIDS atau jangkitan HIV kecuali untuk HIV akibat transfusi darah; Diagnosis Penyakit Alzheimer/Gangguan Kemerosotan Otak Organik Tidak Boleh Pulih, Penyakit Parkinson dan hilang keupayaan hidup sendiri (berdikari) tidak lagi menjadi penyakit kritikal yang dilindungi selepas umur enam puluh lima (65) tahun. Senarai ini adalah tidak terperinci. Sila rujuk kontrak polisi untuk senarai penuh pengecualian MCLR.

11 PERLINDUNGAN 12. Bilakah perlindungan insurans hayat saya bermula? Perlindungan anda bermula dari tarikh permohonan anda yang telah lengkap diisi dan ditandatangani itu diluluskan oleh kami dan apabila kami menerima premium yang sewajarnya. PEMBATALAN 13. Bolehkah AIA membatalkan polisi saya? Tidak. 14. Bolehkah saya membatalkan polisi saya? Membeli polisi hayat adalah merupakan komitmen kewangan jangka panjang. Jika anda tidak membayar premium anda dalam tempoh ihsan, polisi anda mungkin luput. Anda boleh membatalkan polisi anda pada bila-bila masa dengan menulis kepada kami jika anda belum lagi membuat tuntutan. Tiada nilai tunai akan dibayar untuk rider pembayaran premium MCLR g PENGEMBALIAN SEMULA 15. Bolehkah saya mengembalikan polisi saya jika ia telah luput? Ya, anda boleh mengembalikan semula polisi anda tertakluk kepada terma-terma dan syaratsyarat syarikat. Anda mungkin diminta untuk mengemukakan bukti untuk membuktikan kebolehinsuransan kesihatan anda pada saat penerimaan kembali. TUNTUTAN 16. Bagaimana saya boleh membuat tuntutan? Anda perlu melengkapkan dokumen-dokumen tuntutan dan berikan kepada kami laporan doktor yang perlu sebagai bukti diagnosis penyakit kritikal. Borang tuntutan boleh diperolehi daripada Ejen AIA anda, dengan menelefon Hotline Perkhidmatan Pelanggan AIA di 1300-88-1899 atau dengan mengunjungi mana-mana cawangan AIA. 17. Bagaimana tuntutan Kanser berfungsi? Anda dibenarkan membuat sehingga dua (2) tuntutan Kanser dalam Kumpulan 6 yang akan dianggap sebagai dua (2) daripada tiga (3) peristiwa penyakit kritikal. Amaun yang akan dibayar bagi tuntutan Kanser ialah 125% daripada jumlah yang diinsuranskan. Selepas tuntutan Kanser yang pertama dibuat, semua penyakit lain dalam Kumpulan 6 akan digugurkan sementara Kanser kekal untuk membolehkan tuntutan Kanser yang kedua. Tempoh menunggu e selama 5 tahun di antara dua tuntutan Kanser juga diperlukan. Tuntutan Kanser kedua tidak boleh dibuat untuk Kanser yang berlaku pada bahagian atau organ yang sama seperti Kanser pertama. Bagi organ yang berpasangan, hanya tuntutan peristiwa Kanser pertama ke atas organ tersebut boleh dibuat. Sebagai contoh, jika tuntutan Kanser dibuat untuk sebelah paru-paru, tuntutan tidak boleh dibuat lagi untuk Kanser pada mana-mana satu daripada dua paru-paru itu, termasuk paruparu yang sihat. 18. Bagaimana tuntutan Angioplasti dan Rawatan Pembedahan Lain untuk Penyakit Arteri Koronari Utama berfungsi? Bayaran sebanyak 10% daripada jumlah yang diinsuranskan sehingga had RM25,000 akan dibayar bagi Angioplasti dan Rawatan Pembedahan Lain untuk Penyakit Arteri Koronari Utama. Untuk sebarang tuntutan Penyakit Kritikal dari Kumpulan 1 selepas itu, jumlah yang diinsuranskan yang akan dibayar akan dikurangkan oleh tuntutan yang dibuat untuk

12 Angioplasti dan Rawatan Pembedahan Lain untuk Penyakit Arteri Koronari Utama. Jumlah yang diinsuranskan sepenuhnya masih akan dibayar bagi tuntutan penyakit kritikal lain dari kumpulan lain (tertakluk kepada had yang telah dinyatakan). Tuntutan tidak boleh dibuat untuk Angioplasti dan Rawatan Pembedahan Lain untuk Penyakit Arteri Koronari Utama jika terdapat tuntutan dari Kumpulan 1 yang telah dibuat sebelumnya. Tuntutan untuk Angioplasti dan Rawatan Pembedahan Lain untuk Penyakit Arteri Koronari Utama tidak layak mendapat penepian premium dan hanya boleh dibuat sekali. Tuntutan ini tidak dikira sebagai peristiwa penyakit kritikal. 19. Bagaimana tuntutan Pemindahan Organ Utama C berfungsi? Anda hanya boleh membuat tuntutan untuk pemindahan organ utama sekali sahaja. Tuntutan yang kemudiannya tidak boleh dibuat untuk Pemindahan Organ Utama dari mana-mana satu daripada empat kumpulan Pemindahan Organ Utama (Kumpulan 1, 2, 4 dan 6) tidak kira sama ada ia dari kumpulan penyakit kritikal yang sama atau berbeza. Sebagai contoh, jika anda membuat tuntutan untuk Pemindahan Organ Utama (Jantung) di bawah Kumpulan 2, anda tidak boleh kemudiannya membuat tuntutan untuk Pemindahan Organ Utama meskipun ia adalah untuk organ yang berlainan, misalnya Buah Pinggang. 20. Apakah dokumen lain yang perlu diserahkan bersama tuntutan? Di samping borang tuntutan yang diberikan, kami juga memerlukan laporan perubatan tambahan (seperti yang dinyatakan di belakang borang tuntutan) untuk menyokong tuntutan. 21. Bilakah saya perlu memberitahu AIA mengenai tuntutan? Sekiranya berlaku tuntutan, anda hendaklah memberitahu kami dengan segera + + Tempoh pemberitahuan berbeza mengikut jenis tuntutan-tuntutan yang dibuat. Sila rujuk pada kontrak polisi. PELEPASAN CUKAI 22. Bolehkah saya menggunakan premium yang telah dibayar untuk MCLR untuk tujuan pelepasan cukai? Polisi ini boleh melayakkan anda mendapat pelepasan cukai perseorangan tertakluk kepada keputusan muktamad Lembaga Hasil Dalam Negeri. MAKLUMAT 23.
