Take time to review the information carefully, and refer to the list of contacts on page 14 if you have questions. You must enroll within 30 calendar days from the first day you have access to the MyBenefits website. If you do not enroll, you will receive default coverage in basic life insurance, long-term disability and salary continuation benefits, but you will not have coverage in medical, dental, vision or other benefits. WHAT S INSIDE Know your options Be sure you know the plans available to you and consider your options. Choosing the right benefits and using them effectively and efficiently will save you money and help you have a happier, healthier life. YOUR MEDICAL PLAN OPTIONS Whether you re young or an empty-nester, it s important to have coverage that helps keep you at your personal best with access to 100 percent coverage for in-network preventive care and helps cover the cost of health care services if, or when, you need them. IS AN HSA YOUR PATH TO A MORE SECURE RETIREMENT? Did you know an HSA can help you reach your retirement savings goals? Sure, you can use it now to pay for eligible health care expenses tax free, but you can also save your contributions (including Cargill contributions) and roll them over from year to year until you accumulate a significant balance to use for medical expenses in retirement. And remember, you take your HSA with you if you leave Cargill. Not only does money go into your account tax free, but you never pay taxes on it or the earnings, as long as you use it for IRS-eligible medical expenses. DOMESTIC PARTNER BENEFITS CHANGE BEGINNING IN 2017 When the U.S. Supreme Court required states to permit same-gender marriage beginning June 2015, many companies, including Cargill, revisited their decision to provide benefit coverage for domestic partners. Due to this Supreme Court ruling, as of Jan. 1, 2017, domestic partners (and domestic partner s children) will no longer be eligible for Cargill health and welfare benefits. We are announcing this change well in advance to give those affected employees time to consider their coverage options.

Here s how they work to cover medical expenses. (This example shows in-network coverage only.) The effective rate is determined by dividing the total available incentive by 12 and subtracting that amount from the monthly rate. Note that the wellness incentive is a taxable payment made quarterly. The effective rates do not take into account the tax impact of the incentive payments. The amount you can earn is prorated based on your hire date. 1 Differences in covered benefits for the Primary plan include, but are not limited to: in-network coverage only, no infertility coverage, and Blue Distinction Centers must be used to obtain coverage for certain procedures. There are also resources available to help you manage the demands of work and home. When you re a savvy consumer who reviews all health care and work/life resources, you can help reduce the overall cost of coverage, reduce stress and lead a happier, healthier life. EARN INCENTIVES FOR TAKING HEALTHY ACTIONS Cargill s wellness program offers information and resources to help you understand your risks and take action to help you become your personal best. We ve worked hard to make earning your incentives in the plan as easy as possible. You can earn up to $900 and your spouse can earn up to $300 if you participate in certain wellness activities throughout the year 1. To earn those incentives for 2016, here s what you need to do: Certify your tobacco-free status during your enrollment. Complete the Compass health assessment online at myredbrick.com/cargill or by calling RedBrick Health at 1-866-322-1719. You must complete this step to qualify for further incentives, so employees are encouraged to complete it as soon as possible. Achieve healthy biometric results or complete 60 points worth of RedBrick Healthy Activities each quarter. Go to myredbrick.com/cargill for complete details. 1 Please note: The amount you can earn is prorated based on your hire date. SAVE MONEY AND TIME WITH TELADOC Teladoc is an effective alternative to an office, urgent care or emergency room visit, connecting you with a board-certified, licensed doctor at any time. You either call or log in (where available 2 ) to request a consultation, and you ll be connected with a physician who is licensed in your state, usually within 20 minutes. These doctors can diagnose and prescribe medications, when appropriate, for non-emergency issues like ear/sinus infections, allergies, colds and flu, urinary tract infections and more. The $40 fee per consultation applies toward your deductible and out-of-pocket maximum that s less than the cost of a visit to the emergency room, urgent care or physician s office. And, no driving or sitting in waiting rooms with sick patients even better! 2 Teladoc is not available in Arkansas, and Teladoc s video consultations are not available in Texas. Video consultations are required in Idaho. GET YOUR QUESTIONS ANSWERED AND SAVE MONEY WITH PATIENT CARE Patient Care is a benefit advocacy service that you can use at no cost. Just pick up the phone and call 1-877-344-7474 (Spanish: 1-800-381-5834) to speak to a Patient Care advocate. Have health care questions? Wondering where best to get care, if a claim was paid correctly or if a procedure can be done for less elsewhere? Patient Care can help! They can answer questions about your medical plan and claims processing, help you find low-cost, highquality providers and more! COMPARE PRICES WITH COST LOOK-UP You do your homework when you purchase TVs, cell phones and other big-ticket items like a car. Be sure you re thinking about your health care purchases in the same way! Next time you need to go to the doctor or have a procedure done, arm yourself with quality and cost information beforehand. The Cost Look-Up Tool, part of the Provider Finder Tool, allows you to check the cost of services like an MRI, specific procedures or surgery. This is a great way to compare costs, understand your options and select the provider, facility or service that meets your needs and budget. Sign in to bluecrossmn.com/cargill to take a look. FIND OPTIONS WITH WAYS TO SAVE ALERTS Did you know the costs of health care services and prescriptions can vary as much as 600 percent from provider to provider? But when you re sick, the last thing you want to do is figure out who has the lowest cost. Sign up for Ways to Save Alerts. The alert system automatically reviews any recurring claims you had the prior month(s) to see if the same services can be found nearby for less. If so, you ll get a text or email with that option, such as a lower-cost pharmacy or provider alternatives that are near you with better cost. You can also see a dashboard of your claim experience with more detail of your medical claims and the cost of those services. LEARN ABOUT YOUR BENEFITS THROUGH HEALTHCARE UNIVERSITY Watch videos and play games to learn more about your health care benefits and earn wellness incentive points. Healthcare University offers videos, quizzes and games you can play anytime, anywhere, even on your mobile phone or tablet. This free feature makes it easier than ever to learn how to get the most value from your medical benefits and earn points toward your wellness incentives. Healthcare University is available in addition to the tools available through RedBrick Health to earn points toward your wellness incentives. YES, YOU STILL NEED HEALTH CARE COVERAGE Don t forget that under health care reform, individuals are required to have health care coverage or pay a tax penalty. If you and your dependents choose coverage through Cargill or a qualified plan elsewhere, you won t be subject to the individual penalty. There are some exceptions and exemptions from the penalty. You may choose medical coverage for yourself and your eligible dependents under the Cargill plans. Generally, eligible dependents include: Children, stepchildren, children of your domestic partner and foster children to age 26 (for life insurance, children between 19 and 25 must be full-time students in order to be eligible for coverage) Disabled adult children age 26 or older Spouse Domestic partner (for the 2016 plan year) IMPORTANT: If you are covering a dependent(s), you will be asked to provide documentation to support their eligibility for coverage. The Cargill Benefit Service Center will mail you a packet explaining the verification process. Coverage for these dependents will be pended until verification is received. Once your dependent(s) is verified, coverage and premiums will be retroactively charged to your original effective date of coverage. SPECIAL NOTE FOR HSA PLAN PARTICIPANTS WITH DEPENDENTS The IRS allows you to use your HSA to pay qualified medical, dental and vision expenses for yourself and for dependents who meet the IRS definition of dependent on your tax return. Take this into account when choosing your medical plan and/or if you are using an HSA to pay expenses during the year. Talk to your tax advisor for guidance. DOES YOUR SPOUSE HAVE ACCESS TO OTHER COVERAGE? If your spouse is offered medical coverage through another employer (whether your spouse has elected to participate in that coverage or not) and you choose to cover him or her under a Cargill medical plan, you will pay an additional fee of $100 per month on a pre-tax basis (the domestic partner fee is post-tax, per IRS regulations). KEEP YOUR HOME ADDRESS UPDATED Cargill mails important information to your home throughout the year including your W-2 form in January to report your past year s wage, salary and tax information. Make sure your address is up to date to ensure you receive these mailings. There are two ways to check what Cargill has on file for your home address and update it if needed: Call HR Direct Dial at 1-877-366-9696. Visit HR Direct online at https://hrdirect.cargill.com. DENTAL Good dental health is important to your overall health. Cargill s dental plan through Delta Dental helps you keep your mouth healthy by covering the following care: Service Coverage Preventive care (exams, cleanings 100% in network and X-rays) Basic services (fillings, extractions) 80% in network Major services (crowns, bridges, 50% in network dentures, implants) Dental services annual maximum $1,750 Orthodontia 50% up to a separate lifetime in-network maximum of $2,000/person You may visit any provider you choose, but you ll save money when you use an in-network provider. To find out whether your provider is part of the Delta Dental provider network, go to deltadentalmn.org/cargill and click Find a Dentist or call 1-800-587-6975. You can set aside money to pay for health care or dependent care costs. You contribute to your FSA before taxes are withheld from your paycheck, meaning that every dollar that goes toward your FSA reduces your taxable income. Health Care FSA: You may contribute up to $2,550 to your account and can use the money to reimburse yourself for eligible medical expenses not covered by your medical, dental or vision plans for yourself, your spouse or anyone who qualifies as a dependent on your federal tax return even if you don t cover them on your Cargill benefits. If you are enrolled in the HSA plan, your Health Care FSA may be used for dental and vision expenses only. Dependent Care FSA: Contribute up to $5,000 annually to help yourself pay for employment-related day care expenses for eligible dependents. An eligible dependent is someone you claim as a dependent on your federal income tax return, and includes children under age 13 who live in your home, as well as a spouse or dependent of any age (including parent) who lives with you and is physically or mentally incapable of self-support. This account reimburses you for dependent care expenses that allow you to work or attend school full time; it does NOT reimburse you for dependent health care expenses. Plan carefully! Each of these accounts is separate you can t move money between them. The Health Care FSA allows you to roll over (not forfeit) up to $500, as long as you re-enroll in the Health Care FSA for the next plan year. But it s important to note that IRS rules require that any unused amounts over $500 remaining in your account be forfeited. IRS rules also require you to forfeit any unused amounts remaining in your Dependent Care FSA at the end of the year. DEPENDENT CARE LIMIT FOR HIGHLY COMPENSATED EMPLOYEES (HCEs) HCEs employees making $120,000 or more annually for 2015 will be limited to a $1,000 Dependent Care FSA contribution in 2016. You can purchase voluntary AD&D coverage through The Standard Insurance Company for yourself and your spouse in the following amounts: For yourself: $25,000 to $500,000 in $5,000 increments. For your spouse: $25,000 to $250,000 in $5,000 increments. You must purchase voluntary AD&D coverage for yourself in order to purchase it for your spouse, and your spouse s coverage amount cannot be greater than your coverage amount. LONG-TERM DISABILITY (LTD) Cargill provides employees with salaried benefits with longterm disability coverage that pays two-thirds of your annual salary, up to a maximum annual salary of $75,000, if you are totally disabled for six months or more, as defined by the plan. SALARY CONTINUATION Salary continuation is for qualifying absences due to a physical disease, injury, pregnancy or mental disorder lasting longer than seven calendar days up to 26 weeks. You are automatically enrolled in this company-paid benefit. The amount of salary continuation you are eligible to receive is based on your length of service. CARGILL PARTNERSHIP 401(K) RETIREMENT PLAN To help you set aside money for retirement, all Cargill employees are automatically enrolled in the Cargill Partnership 401(k) Plan once they have been employed for one year. The money you contribute to the plan can be invested in a number of options through Vanguard. Setting aside money in the 401(k) Plan is easy. Here s how you benefit: It s convenient contributions are automatically deducted from your paycheck. It lowers your taxes the money is set aside in the plan on a pre-tax basis, which lowers your taxable income. That means that if you elect 1 percent in pay, you will not see your paycheck decrease by 1 percent. It s portable you are 100 percent vested in your contributions to the plan immediately, which means that the retirement savings are yours to transfer to another company s retirement plan or private IRA (Individual Retirement Account) if you leave Cargill. It s flexible you can also choose to contribute after tax through the plan s Roth feature. In general, those contributions plus any earnings will not be taxed when you take them out of the plan. It s accessible if you experience a hardship and need to access the money in your 401(k) plan before retirement, you may qualify for a loan or hardship withdrawal. Core coverage is company paid, up to a maximum annual salary of $75,000. You can choose to purchase additional LTD insurance that will replace two-thirds of base pay up to a maximum annual salary of $240,000. Evidence of insurability is required if you choose to purchase this additional coverage after you are newly hired. The monthly cost for buy-up coverage is $0.649 per $100 of monthly salary. Your beneficiary designation applies to any Basic, Additional Life, Voluntary AD&D and Business Travel Accident (if applicable) elections you have on file. This means you should have a beneficiary on file even if you don t elect any additional life or AD&D coverage. Please note, if your spouse or domestic partner dies, you will need to provide your marriage certificate or a signed affidavit of domestic partnership in order to receive the benefit. EMPLOYEE RETIREMENT ACCOUNT (ERA) Cargill funds this portable retirement account, which provides an annual contribution of 2.5 to 7 percent of your eligible pay, depending on your age and years of service (see the table). You are immediately eligible for the ERA and are vested after three years of service. You control where the money is invested from a selection of funds. If you do not make an election, Cargill s contribution will automatically be invested in the Vanguard Target Retirement Fund that most closely matches the year in which you will reach age 65.
