Life Benefits Custom-designed plans for businesses of all sizes, from 2 lives up (5 in some states as required by law). Basic coverage can be flat amounts or multiples of basic annual salary. Voluntary coverage can be purchased in flat amounts, multiples of salary, or incremental amounts. Variable premium payment options to meet your budget needs and employee benefit objectives, from employer paid to voluntary with little or no cost to you. Insured employees have the ability to purchase limited amounts of life insurance for their spouse and eligible dependent children. Accidental Death & Dismemberment Benefit Coverage provides additional benefits upon an insured employee or spouse s accidental death or dismemberment. Accelerated Death Benefit If an insured person suffers an illness that is expected to result in death within 12 months, then the insured is eligible to receive a portion of the death benefit. Waiver of Premium Payment of premium will be waived for an insured employee s term life coverage (excluding AD&D) in the event of total disability. Conversion An employee may convert all or part of the life insurance to an individual policy under certain circumstances. Portability The insured can elect to continue coverage when coverage under the group policy ends and certain other prescribed conditions are met. AD&D Seatbelt Benefit If a car accident results in the death or dismemberment of insured employee or spouse who was wearing a seatbelt, then an additional benefit will be paid equal to one-half the AD&D benefit. Public Conveyance Benefit An additional benefit equal to the AD&D benefit is paid upon an insured employee or spouse s death or dismemberment while riding as a fare-paying passenger. Easy to Qualify Minimum work requirements vary between 20 30 hours per week. Guarantee Issue Amounts Guarantee issue amounts vary by group. No evidence of insurability is required for amounts up to the guarantee issue limit as long as applications are received within 31 days of initial eligibility. Reductions A variety of reduction schedules are available. The standard schedule provides for insurance to reduce 35 percent on the insured s 65th birthday, 50 percent on the insured s 70th birthday, and 75 percent on the insured s 75th birthday. Coverage terminates upon retirement for employees and upon attainment of age 70 for spouses. This policy has exclusions, limitations, and terms under which the policy may be continued in force or discontinued. For costs and complete details of coverage, please call or write the company.
