PHARMACY TECHNICIAN LICENSING IN MICHIGAN February 27, 2015 LICENSE TYPES and Active Licensees Full License Examination Limited Employer Verification Temporary Preparing to take the examination As of February 25, 2015, the Department has issued the following number of licenses: Full License 1453 Limited 179 Temporary 210 FULL LICENSE APPLICATION PROCESS Submit complete application form and $55.00 fee Pass certified pharmacy technician examination from either of the following: 1. Pharmacy Technician Certification Board (PTCB) or 2. National Healthcareer Association (NHA) 1
LIMITED LICENSE APPLICATION PROCESS Submit complete application form and $35.00 fee Submit a completed Verification of Employment as Pharmacy Technician form. Note: The limited license is only valid if you remain employed by the same pharmacy. If you are no longer employed as documented on the Employment Verification form, the limited license is no longer valid. TEMPORARY LICENSE APPLICATION PROCESS Submit complete application form and $40.00 fee. Note: The temporary license is issued so that the applicant may continue working while preparing to take the examination required for full licensure. The temporary license is only valid for 210 days and cannot be renewed. ETHICAL DISCLOSURES REQUIRED ON APPLICATION Felony conviction Misdemeanor conviction resulting in imprisonment of at least two years Misdemeanor conviction involving illegal delivery, possession, use of alcohol, or controlled substance 3 or more malpractice settlements, awards, or judgments, in any consecutive 5-year period 1 or more malpractice settlements, awards, or judgments, totaling $200,000 in any consecutive 5-year period 2
ETHICAL DISCLOSURES REQUIRED ON APPLICATION Fined, denied, revoked, suspended, reprimanded, placed on probation, otherwise disciplined or subject to adverse action by a licensure, registration, disciplinary or certification board Censure, or requested to withdraw from a health care facility s staff or health care staff privileges involuntarily modified Treatment for substance abuse within the last two years Must provide a written description of incident with application with copies of official or court documents. Separate from fingerprint/criminal background report FINGERPRINTING & CRIMINAL BACKGROUND CHECKS Mandatory for all new applicants for licensure; not required for subsequent applications or renewals Have fingerprints taken about five days after submitting license application to Board Livescan prints by contractor (Identogo ), or any vendor listed at www.michigan.gov/lsvendor) if you are physically in Michigan using Agency ID Number 90905H Contractor sends fingerprints to Michigan State Police who send report to licensing board; report includes a search of FBI records Any hits are reviewed by Bureau What you should know Submit completed application with proper fee. Fingerprinting/Criminal Background Check Arrange for verification and/or certification of license status to be sent directly to our Department if you currently hold or have ever held a licensed or registration in any other state or province 3
What you should know Contact your high school to have a copy of your final high school transcripts sent directly to the Board office or have the GED testing company provide verification. NOTE: Applicants who graduated from high school in Michigan may be able to use Michigan e-transcript service. Information on this service can be found at http://www.docufide.com/contact-us. E-transcripts may then be sent to bhcsdata@michigan.gov. FACTORS AFFECTING PROCESSING TIME Volume of applications for all professions, especially in spring and summer Pharmacy Technician Licensure Deadline is June 30, 2015 Timeliness of educational institutions and examination agencies providing necessary documentation Receipt and review of fingerprint/background check report Do not wait to apply Pharmacy technicians who wait to apply for licensure closer to the June 30, 2015 deadline cannot be guaranteed to be licensed by June 30. The Bureau processes license applications for 24 different professions and the majority of our applications are received between April and June as this is when most students graduate from their educational programs. Pharmacy technicians who are issued their licenses prior to March 3, 2015 will be required to renew their licenses by June 30, 2015. The renewal fee is $60 and the renewed license will be valid for two years. 4
