How and when can I sign up for Harken Health? Access to a Harken Health Insurance plan is available to individuals via an independent insurance broker or the Health Insurance Marketplace. Small group employers may provide the Harken Health plan to employees through an independent insurance broker. Individuals may also sign up directly online at harkenhealth.com. People can sign-up through the Health Insurance Marketplace during the Open Enrollment period November 1, 2015 through January 31, 2016. Where are Harken Health Centers located? Can I visit any Harken Health Center to receive care? If I am out of town, what are my service options? Members can visit any Harken Health Center. If you are out of town, you have the option of scheduling appointments, asking questions or getting support from the Care Team during office hours via the Harken Health portal, phone, e-mail or video chat. After hours, Harken doctors are available to talk with members via phone to determine a care plan. However, if in-person care is needed on a nonemergency basis and a member is not near a Harken Health Center, Membership Assistants can connect them with a reputable provider located in their visiting areas. For emergencies, Harken Health members are able to access a national network of hospitals, specialist and emergency care centers. What services does Harken Health provide? Each member is assigned a Care Team made up of a Care Provider, a Health Coach, a Behavioral Health Specialist and a Membership Assistant. Additional benefits include: 24-hour access - Late night, early morning and weekend access is triaged to the on-call Care Provider. Member website access Members have access to Harken Health s member website to manage medical and insurance information and communicate with the Care Team. Consultations with a Behavioral Health Specialist - Harken Health s specialists provide short-term counseling, assessments and referrals, as well as consultation to other team members, all integrated with their Care Team and within the scope of primary care. Harken believes in the importance of mental health to overall health and well-being. Access to group wellness classes held in the Health Centers, such as nutrition, cooking, health care finance, fitness and yoga.
What are my options if I need treatment beyond what Harken can offer? Harken Health members have access to a network of approximately 850,000 physicians, medical professionals and hospitals nationwide. The individual Harken Health Insurance plan will provide more details about accessing such care. Where do the physicians come from/who staff Harken centers? Harken Health has a staff of Physicians, Physician Assistants, Nurse Practitioners and Health Coaches for its Health Centers who are not only highly-qualified and board-certified (where applicable), but who also strongly value member relationships and quality of service. The Harken Health Care Teams work together closely, collaboratively and creatively to support members and their health journey. What if I m not sure I want to change doctors right now? Come visit a local Harken Health Center to find out if this is the right change for you. You can meet with a Care Team Member who can show you around the facilities, introduce you to Care Providers and Health Coaches, and answer any questions you have about Harken Health.
Can I keep my specialist outside of Harken Health? Yes. How much you are charged for visits will depend on your specialists network status. We have both a preferred and general network. Even if your provider is not a Harken network provider, we do provide out-of-network benefits. See your specific plan to understand the deductible for each level of our network. Do Harken Health centers accept walk-ins? Harken Health firmly believes that a membership model of care is the best way to ensure patients receive quality care, and therefore, does not accept walk-ins from non-members. Members will have easy access to Harken Health Centers, so as to eliminate the need for walk-ins; however, Harken will not turn away members without appointments. Do I have to pay right after my visit? Outside of the monthly membership payment, members will not have a copay for visits to Harken Care Centers and will not have to pay while at a Health Center. Why do I have to get rid of my current insurance? Harken Health follows all applicable coordination of benefit laws and provides full insurance coverage to all of its members. What are the different membership levels, and how do I determine which level is right for me? Harken Health has multiple insurance plan options. Choose from Gold, Silver or Bronze plans based on your health needs and budget. Regardless of the plan you select, you ll get great benefits. Harken Direct Sales and Brokers are available to address questions regarding coverage options. They work with potential members to help them decide which level best addresses their health care needs. Will Harken Health members have access to the UnitedHealthcare network? Yes, the Harken network includes the UnitedHealthcare network. If further care is needed beyond what the member s Harken Health primary care physician can provide at a Harken Health Center, members will be able to seek out health care services provided by a number of specialists within the Harken network.
