SOME TREATMENT RESULTS OF GYNAECOPATHY BY USING LOW POWER SEMICONDUCTOR LASER Bùi Phương Anh, Trần Minh Thái, Cẩn Văn Bé, Trần Trung Nghĩa Laser Technology Laboratory Faculty of Applied Science HCMC University of Technology 268 Ly Thuong Kiet St., Dist. 10 th, Hochiminh City, Vietnam ABSTRACT According to the annual statistic of Ministry of Health, the number of patients increases from 15% to 27% every year: 20% of women over 30 years old having ovarian cyst, over 20% of women over 35 years old having uterine fibroma. Taking care of gynaecological health for women is extremely important. Popular treatment methods have many blemishes such as: hysterectomy, oophorectomy, vaginectomy These methods lead infertility or affect sexual activity of women. Laser Technology Laboratory has carried out a project of research into the application of low power semiconductor laser with multi wavelengths in treatment some gynaecopathy and are successfully manufactured an equipment for treatment of some gynaecopathy basing on the combination both traditional oriental acupuncture and modern medicine. This equipment was applied by some medical centers in clinical treatment and had success results in some diseases, such as: gynaecological inflammations, benign tumours, child enuresis. I. Introduction Unlike the male, the working organs of the female reproductive system are hidden away inside the body, protected within the pelvic cavity. However, the vagina communicates with the exterior and the external genitalia associates external structures. Vaginitis and vulvitis are very common in sexually active women. In many cases, the treatment was so late or with wrong way, so that diseases would be long lasting and leave sequelas, such as: infertility, ectopic pregnancy or abortion. According to the annual statistic of Ministry of Health, the number of patients increases from 15% to 27% every year: 20% of women over 30 years old having ovarian cyst, over 20% of women over 35 years old having uterine fibroma. Taking care of gynaecological health for women is extremely important. Popular treatment methods have many blemishes such as: hysterectomy, oophorectomy, vaginectomy These methods lead infertility or affect sexual activity of women. There were many researches all over the world to find out new methods or improve present methods. Few years ago in Vietnam, some hospitals used HeNe laser (632.8nm) in treatment of gynaecological inflammation and had good results such as: treating adnexa inflammation by HeNe laser associates with magnetic field [1], effect of HeNe laser in treatment cervical gland inflammation [2]. However, these devices have been uncomfortable because of the bulky device, unchangeable power laser Laser Technology Laboratory has carried out a project of research into the application of low power semiconductor laser with multi wavelengths in treatment some gynaecopathy and are successfully manufactured an equipment for treatment of some gynaecopathy basing on the combination both traditional oriental acupuncture and modern medicine. The mentioned equipment was applied by some medical centres in clinical treatment and achieved good results in some diseases, such as: gynaecological inflammations, benign tumours, child enuresis. II. Theoretical fundamentals of treatment method 2.1 Wavelengths selection of low power semiconductor laser in treatment Selecting suitable wavelengths used in this device has been based on the results of simulation of photon propagation in biological tissue by Monte Carlo method. 1/5

The figures below describes the Monte Carlo stimulation results by the 10-4 W/cm 2 fluences of four wavelengths low power semiconductor laser (630nm, 780nm, 850nm and 940nm) in tissue from the skin surface. Figure 1: Comparison between fluences as a function of radius for several z coordinates in breast tissue from the abdominal skin surface According to figure 1, the deepest penetration depth in biological tissue is 4cm with 940nm; the most shallow at 630nm (1cm). With two wavelengths: 780nm and 850nm, the penetration depth is approximately (2.6cm and 3.2cm respectively). Figure 2: Comparison between fluences as a function of radius for several z coordinates in genital tissue from the abdominal skin. According to figure 2, the deepest penetration depth in biological tissue is 3,8cm with 940nm; the most shallow at 630nm (1.2cm). With two wavelengths: 780nm and 850nm, the penetration depth is approximately (2.6cm and 2.8cm respectively). 2/5

Figure 3: Fluences as a function of radius for several z coordinates in genital tissue from the abdominal skin with 12mW of power at 940nm, the depth of photon in tissue is 5cm Figure 3 showed that increasing the power, the path of photon tranversing tissue will be deeper. From the simulated results and safety demands two wavelengths 780nm and 940nm is chosen and used to treat some gynaecological diseases. 2.2 Treatment Biological stimulation effect leads to biological responses Biological stimulation effect happens when the laser interacts with biological tissues, with the fluences about (10-4 10 0 ) W/cm 2 and time of treatment from tens of seconds to tens of minutes. Biological stimulation effect happens through photochemical and photobiological reactions, leads to 7 biological responses [3], consist of: - Response of antiphlogistic (anti inflammation) - Response of anti pain. - Response of cell lession. - Response of regeneration. - Response of immunity. - Response of cardiovascular system. - Response of endocrine system. These responses are the base that we used in treatment some gynaecological diseases of our method. The two simultaneous wavelengths effect is created by using two lasers with 780nm and 940nm simultaneously so that biological stimulation effect occurs faster and more effectively. With chosen wavelengths, the two simultaneous wavelengths effect is not only treat lesions at the skin surface but also at internal lesions. Because of this effect, we can irradiate directly on the skin surface at the nearest position of lesion area. The result of [4] indicated that we can reach the best result when using the two simultaneous wavelength effect for lesions which have area below 100cm 2. Using the acupoints in traditional oriental acupuncture We used semiconductor laser 940nm (GaAs) as an optical needle for treating diseases through acupuncture method [5]. Two treatment capacities: acupoint in traditional oriental acupuncture, biological stimulation effect were applied simultaneously to create a resonant efficiency in treatment. 2.3 Equipment 3/5

There is not complication in treatment process and after treament. IV. Conclusion The quality of clinical results is not enough so that we cannot conclude of this method. However, the preliminary results were statisfactory. Furthermore, treating gynaecological diseases by using our method is better than others with many adventages as: non invasive treatment method. avoid the spreading of HIV, hepatitis B virus and other infectious diseases. treatment technique is simply and widely used in many Health Centers V.
