8 NEW Rewards benefit With cash rewards for takin healthy steps, you ve ot a plan that can help you feel your best. You can et reward cards worth up to $75 for takin steps to improve your health. Challene yourself and earn up to $75: $50 for completin a Total Health Assessment This online questionnaire ives you a picture of your overall health, plus an action plan that s based on your answers. $25 for completin an online healthy lifestyle proram Get help to reduce stress, quit smokin, lose weiht, and more. You can take a proram when it s convenient for you, at your own pace. Startin January 1, 2015, visit kp.or/feds to et started. There you ll find information and links to the Total Health Assessment and healthy lifestyle prorams. The rewards proram runs from January 1, 2015, throuh December 31, 2015, and is open to all Kaiser Permanente FEHB subscribers and their enrolled spouses who are 18 and older. You can take the Total Health Assessment as often as you d like and as many healthy lifestyle prorams as you d like, but you can only earn rewards for them once durin the reward period. You must complete the activities durin the calendar year. Rewards will be issued 4 to 6 weeks after you complete your activity. 6

Hih Option Standard Option Included with both You pay hiher premiums, but lower copays for most services and prescription drus. If you expect to et care often, this may be the riht choice. Your supplemental dental options You can also choose from 2 dental plans to supplement your health coverae. 4 Kaiser Permanente Insurance Company (KPIC) Dental Insurance Plan This plan ives you the freedom to see any licensed dentist you choose, with no deductible for preventive and dianostic services. When you visit a participatin Delta dentist in California, all claims paperwork will be handled for you. DeltaCare USA With this dental HMO plan, you ll select a dentist from the DeltaCare USA network. There are no claim forms to fill out and no deductibles. Most preventive and dianostic services are available at little or no cost to you. Coverae for certain outpatient follow-up care and outpatient continuin care outside of Kaiser Permanente service areas throuh the FEHB travel benefit. Save on prescription drus Save time and money by orderin prescription refills online, on your smartphone, or by callin the phone number on your prescription label. Prescription refills can be mailed to your home at no extra chare. And when you order refills by mail order, you et a 100-day supply and pay only twice the 30-day supply Plan pharmacy copay. Chiropractic without a referral You can visit any American Specialty Health Plans of California, Inc. (ASH) participatin provider with no referral. Chiropractic services are covered if they re needed to dianose or treat neuromusculoskeletal disorders, as prescribed and provided by an ASH Plans participatin provider. 12 Kaiser Permanente Senior Advantae (HMO) for Federal Members If you re eliible for Medicare, you have an option to choose Kaiser Permanente Senior Advantae (HMO) for Federal Members. Added benefits Get more information If you decide to enroll in Senior Advantae (HMO) for Federal Members, you ll et richer benefits than with your FEHB coverae alone. You ll et all of your FEHB benefits, plus: Lower copays for office visits and for most other services, such as inpatient hospital admissions and most prescription drus. Up to a 100-day supply of prescription drus at Plan pharmacies for one copay (Hih Option only). Extra credit toward the purchase of eyelasses and contact lenses.
