2 About Pulmonary Rehabilitation Pulmonary Rehabilitation is for people with chronic lung disease who are limited by breathlessness. This program may be suitable if you have long term breathing difficulties due to COPD, Asthma, emphysema, chronic bronchitis, bronchiectasis, pulmonary fibrosis or after surgery. Pulmonary Rehabilitation aims to: Increase your independence Increase your confidence and ability to cope Improve your exercise ability Reduce breathlessness during normal activities Improve your quality of life Keep you out of hospital Provide practical ways of dealing with ongoing physical limitations The St Andrew s Program: St Andrew s pulmonary rehabilitation program is based on the guidelines from The Australian Lung Foundation and includes a 6 week outpatient program with two visits each week (a total of 12 visits). Each program includes:

3 Your own case manager An individual assessment at week 1and week 6 Two-hour sessions incorporating: - One hour of education - One hour of supervised exercise The education sessions: The education sessions are presented by physiotherapists, occupational therapists, speech pathologists, dieticians and pharmacists and cover topics such as: 1. Creating a home exercise program 2. Lung Anatomy and Physiology (how your lungs work) 3. Energy conservation and relaxation 4. Speech pathology for pulmonary patients (swallowing, coughing, speech) 5. Emotional wellbeing and stress management 6. Breathing retraining, exercise principles and airway clearance 7. Lung pathologies and disease (what can go wrong with your lungs) 8. Lung investigations 9. Healthy eating for your lungs 10. Medication management (getting the most out of your medications) 11. Self management principles how to cope and manage your disease 12. Feedback, surveys and general discussion

4 The exercise sessions: Your exercise program is designed specifically for you based on your initial assessment. Each exercise program includes a warm-up, some aerobic exercises, some strengthening exercises for your arms and legs, then a cool-down period. Oxygen: If you require supplemental oxygen, you will need to bring this with you to the sessions. How to get started You may be referred to us from: your local general practitioner or specialist from any hospital via your treating doctor Additional information for your doctor on how to make a referral is listed on the back of this brochure. New programs commence once we have at least 4 participants. Additional Information How much will the program cost? We hold agreements with all private health funds and have tier one provider status with DVA. Please ask our friendly staff to help you to confirm your rebate entitlements with your health fund before you start.

Information for your referring doctor You might consider Pulmonary Rehabilitation for your patient if they experience any of the following: Recent exacerbations and hospitalisation Increased hospitalisations, ED visits, office visits/calls Disease process causing limitations in physical activity Disease process causing limitations in quality of life Reports of increasing dyspnoea Psychological or emotional changes Any client requiring additional education and support to self manage their disease How to refer a patient Access our website ( sawmh) and print out a referral form listed under: Services/Rehabilitation.
