4 Background The team was established on the 1 st April 2012 and is part of the NHS Scotland Data Support and Quality. There are 3 strands of the service Data Quality Data Advice (includes CHH Model, Definitions and Reference Files, and Terminology) Data Support and Monitoring The purpose of this service is: The delivery of data management services across ISD The provision of support to NHS Scotland for submission of national data The modernisation of the national data in line with the national information strategy. The service works with all its customers to ensure consistent and accurate recording of data in healthcare systems. Data Advice Service CHH Model and Tools Data Advice service work with clinical and technical specialists to support the accurate recording of data throughout NHS Scotland. Providing assistance and support tools in aligning customer data elements with national and internationally maintained information sources using the Compound Healthcare Headings model. The Compound Healthcare Headings model was developed for local and national use. It can be used during system development or to healthcheck a legacy system. Compound Healthcare Headings Model (CHH) The CHH Model has two functions 1. The Compound Healthcare Headings (CHH) model provides navigation to a range of national and international information resources. This can help health and care providers understand and share information consistently, effectively and efficiently across all care settings. 2. The model provides a structure with a hierarchy and defined headings. This can aid system management as information can be stored consistently under agreed headings to allow consistent storage to support easier retrieval and sharing of information. Healthcare-Headings-Model/CHHModelHTML4/index.html CHH Information Sources The CHH Model contains a range of information sources listed below are those that are created owned and maintained by NHS Scotland. CHH Values (HTML/Excel) Legacy NCDDP Standards (Excel) ISD Reference Files (CSV flat files) Location Lookup (Web application) External providers of information/ standards have been included because they are recognised, authenticated and have their own governance, e.g. DM&D, egif etc.
5 CHH Model Development CHH model amendment/development will be considered if the navigational structure is agreed to be incomplete, or an information source is thought to be missing by the customer and/or Data Advice (CHH Model). During CHH 6 monthly development cycles, there will be ongoing communication between Data Advice (CHH Model) and the customers. CHH Packets CHH Packets are reusable and are designed to allow for consistency of information across systems and align to the CHH structure. Packets conform to the CHH colour coding and format agreements. Click on link below to view various packets: Principles for creating packets are: Packets may be created in response to customer need. They are not specialty specific or specific to any one professional group (i.e. no business rules). They will remain consistent but with varying levels of granularity according to the system that implements them. Contents of the packets will be created by and receive quality assurance from information standards specialists. Packets will be published on the Data Advice (CHH Model) website. CHH Bundles (Template Support) CHH Bundles are designed and created by Data Advice (CHH Model) to support specific templates (eforms, ehealth standards etc.) and align to the CHH structure. CHH IST CHH ISTs are bespoke mapping to support a specific customer need. XML Schemas XML Schemas describe the structure of an XML document. An XML document can be validated against an XML schema to check whether it conforms to the requirements specified in the schema. XML schemas enable document designers to specify the allowed structure and content of an XML document and to check whether an XML document is valid. The CHH Model schema has been produced and uploaded to Ensemble. XML schemas are produced to support Packets, Bundles and ISTs to meet customer requirements. This functionality offers additional benefits when considered for use within national products like Ensemble which can readily take in and generate XML. The creation of these CHH products for Ensemble users to align to will facilitate: Opportunity for new customer interactions with the product (can transform it from XML to a range of other standards e.g. XSD to meet their needs). Opportunity for constructive customer testing of the CHH product(s) in I.T. environments (end - to end testing). Support for off-the-shelf CHH Bundles and CHH ISTs, complete with schema elements allowing it to be automatically downloaded into a customer s native environment. Benefits Clinicians/Customer/Information Technology
6 The consistent application of these data standards/ items across all person centred care systems will contribute to collaboration, improved quality of information to support person centred care. Users have told us that they need robust, nationally agreed clinical standards as well as administrative standards to capture encounter information at the point of care, irrespective of where in the healthcare continuum the encounter has taken place. The Compound Healthcare Headings model has been created to aid system developers locate, navigate and identify appropriate standards within the healthcare continuum. Development of data definitions, standards and a navigational structure can support the robust identification of a person, their preferences, needs and values, including compliance with various laws in relation to Equality & Diversity. Valuable clinical standards have been developed by clinicians throughout NHS Scotland which adhere to Government and International standards where practical. This will support: Safer and more effective information available for a care encounter and allow information to be available without the need for time consuming retrieval, data entering or mapping. Standardised data presentation permits timely and simple data entry and retrieval of information in order to manage person care across health care teams, aiding efficient information flow between healthcare providers and enhanced use of clinician/ person time. Facilitate faster information exchange reducing the need for manual data entry, manipulation, ensuring interoperability and improved quality of information. Improved record linkage, interpretation and use of data to occur. This results in a safer service as data are interchanged between systems consistently, robustly and securely and so data have the same meaning across systems, which results in less misinterpretation or errors. 7 Appendix 1 The CHH Model Structure and Colours Each main heading within the model has been allocated a unique colour to aid the distinction of each section, the sub headings under each heading are the same colour as the main heading. Who the Data Recording Advisory Service are engaging or collaborating with (can be an NHS Board(s), NHS System developer(s), Commercial suppliers of I.T to NHS Scotland, NSS, and S.G.H.D.). The Data Advice Team was established in April 2012 within the Data Management Department of ISD. In addition to supporting the work of the CHH Model, the team also manages the National Data Definitions and References files service. Appears at the end of each branch of the CHH model. They are web links which contain the name of the information source they are pointing to and send the customer to the appropriate repository which contains the information they are looking for. The information contained at the end of a lower branch of the CHH navigational model encompassed in a rectangular shaped box. Its purpose is to offer you a hyperlink(s) to verified source(s) which contain information and or data standards which relate to the particular CHH navigational heading from which the branch emanated from. CHH ISTs are bespoke mapping to support specific customer need. A template (sometimes referred to as a form) is the organisation of data items to enable standardised and reusable entry, collection and presentation of data for a specific purpose, specialty or group based on protocols and business rules as laid down by the customers. Templates are designed and created by the customer but can be underpinned with standardised data items supported by the CHH model e.g. referral template, discharge template, respiratory assessment template to facilitate greater interoperability.
