4 Medical Homes and Accountable Care Organizations: If We Build It, Will they Come The medical home is a patient-centered model which provides high quality, low cost, and convenient care A transformative shift to place the patient at the center of the delivery system, rather than the clinical team Parkland is a look alike umbrella of an organizational structure like an ACO consisting of individual medical homes
