3 Section 1: Application Information Instructions and Checklist on the Submission of Application The items required for the purpose of applying for admission into the Master of Science in Supply Chain Management (M.Sc. (SCM)) programme are listed below. Please read and follow the given instructions carefully. Please submit the indicated number of copies of documents requested for. Original documents (except University transcript) should not be submitted. Submitted original documents will not be returned. The institute will not be responsible for lost or missing items. Please note that an application fee is levied on each application to this programme. Successful applicants will be asked to present original documents for verification on registration day in December All application materials must be submitted to the following address through mail or courier before the closing date. 4 Items To be Submitted (Do not staple any of the documents) Item 1: Application Form for Admission Submit the application form online. Print out the completed application form, sign it and mail it together with supporting documents to The Logistics Institute - Asia Pacific before the closing date. Self-Check ( ) Item 2: Objective of Pursuing Course This form must be completed. Item 3: Receipt Receipt of Application Fee Payment Item 4: Photocopy of Passport The page containing the applicant s photo and personal particulars should be photocopied and submitted. Item 5: Photocopy of Identify Card This applies to Singaporeans and Singapore Permanent Residents (SPR) only. Photocopies of both the front and back of the identity card should be provided. Item 6: Photocopy of Entry Permit This applies to SPR only. Item 7: Photocopy of Employment Pass This applies to foreign applicants who are working in Singapore. Item 8: Documentation Proof of Financial Support International applicants must submit documentary evidence of their ability to support their study (e.g. an original bank statement) indicating the availability of funding of at least S$18,000 or its equivalent. Item 9: TOEFL or IELTS Score Sheet Graduates from Universities where English is not the main medium of instruction (e.g. China, Myanmar, Indonesia, etc) are required to demonstrate their English proficiency by means of Test of English as a Foreign Language (TOEFL) or IELTS scores. Visit or for details on how to apply and sit for the TOEFL or IELTS tests. Applicants should submit valid TOEFL or IELTS results. Please note that TOEFL/IELTS results are valid for only 2 years after the test and should be valid at the beginning of the application period for the designated M.Sc. degree. You may either submit a photocopy or the original copy of your TOEFL/IELTS scoresheet together with other parts of this application or request ETS to send your official scores to NUS. 5 Item 10: Photocopy of Degree Scroll(s) For each degree course successfully completed and indicated on the application form, applicants must furnish the above listed item. All copies of degree scrolls should be signed by the university s Registrar s Office as Certified True Copy. If the scroll is in a language other than English, an official translation must be included. This should be certified at a Notary Public. Item 11: Original Transcripts or Certified University Results from First to Final Year of Study For each degree course successfully completed and indicated on the application form, applicants must furnish the above listed items. Applicants may use the Transcript Request Form to request for transcripts from the respective universities. Transcripts should bear the stamp of the University and the name and signature of the Registrar or an authorised staff member. If the transcript is in a language other than English, an official translation must be included. This should be certified at a Notary Public. Item 12: Photocopy of University s Report or Relevant Documents on Co- Curricular Activities, Scholarships, Prizes, Achievements and Awards This should be furnished if available. Item 13: Acknowledgement Slip Only for applicants residing in Singapore. This form must be completed and if handwritten, please write legibly. Please affix with Singapore stamp of 50 cents. Photocopy of University s Report or Relevant Documents on Co- Curricular Activities, Scholarships, Prizes, Achievements and Awards if available 12) Original Bachelor Degree transcripts or certified university results. 13) Photocopy of Bachelor degree scroll(s) 14) Acknowledgement Slip (Only for applicants residing in Singapore) *Reference letters are optional. 7 Section 2: General Information Documentary Evidence IMPORTANT: An ORIGINAL official transcript of academic records, or its certified true copy, is required from each university you have attended. Candidates with qualifications other than those from NUS are to request the transcript(s) from your relevant university. Please note that we will only accept official transcript(s) which is sent directly from the issuing university to NUS. Transcripts must be enclosed in an official SEALED envelope with its flap bearing the security seal of the university and the signature of the Registrar or representative. Graduates from NUS can submit copies of their result slips or official transcripts of their academic records. All certificates should be attached to the completed application form. Certificates submitted in a language other than English must be accompanied by a translated and certified copy. Further documentary evidence may be requested at a later date. TOEFL and IELTS Tests The quantity and quality of academic work at NUS cannot be accomplished without the mastery of the English language. The point is deliberately emphasized because graduate students at NUS submit assessments, take written examinations and are expected to participate actively in the classroom discussions. Applicants who graduated from Universities where English is not the main medium of instruction (e.g. China, Myanmar, Indonesia, etc) are required to demonstrate their English proficiency by means of TOEFL* / IELTS** test scores. *TOEFL is the abbreviation of the Test of English as a Foreign Language. **IELTS is the abbreviation of the International English Language Testing System. Applicants can either: (a) submit photocopies of their TOEFL/ IELTS scoresheets; or (b) request Educational Testing Services (ETS) to send original TOEFL scores to NUS. (Institution code of NUS for TOEFL is 8813). The minimum score expected is: i. 600 (for TOEFL paper-based test) or ii. 250 (for TOEFL computer-based test) or iii. 92 (for TOEFL internet-based test) or iv. 6.5 score for IELTS. Candidates who do not submit a satisfactory official TOEFL/IELTS score by the application closing date will not be considered. Page 7 of 15

TOEFL score or IELTS result is only valid for 2 years after the test and should be valid at the beginning of the application period for the designated M.Sc. degree. Successful candidates whose TOEFL/IELTS scores fall within the following ranges must register to take a Diagnostic English Test (DET) conducted by NUS Centre for English Language Communication (CELC) : i. 580 TOEFL paper-based test < 600 ii. 237 TOEFL computer-based test < 250 iii. 85 TOEFL internet-based test < 92 iv. IELTS = 6.0 Candidates who do not achieve an expected proficiency level upon taking the DET will be required to read and obtain a satisfactory grade for the assigned English Course(s) conducted twice weekly. Classes are available both during the day and in the evening. Subject to availability, part-time students will be given priority to register for evening classes. Further details will be provided upon successful admission. Application Fee An application fee of S$20 (inclusive of 7% GST) for applying to a programme is applicable. The application fee is non-refundable. Page 8 of 15

9 Tuition Fees Session 2015/2016 While you are pursuing your studies in NUS, you are liable to pay fees. The amount payable may depend on your nationality as well as whether you have previously enjoyed a Singapore government subsidy or sponsorship by a government agency. Category 1 Full Tuition Fees Students who have previously enjoyed Ministry of Education (MOE)/government subsidy or sponsorship by a Singapore government agency (such as scholarships offered by the Ministries, Public Service Commission and Statutory Boards) in a graduate programme (either Masters or Ph.D degrees) will not be eligible for subsidy in another graduate programme at the same or lower level. Instead, such students will be liable to pay Full Tuition Fees. For example, students who had previously enjoyed subsidy/scholarship in a Master s programme and had attained the Master s degree will not be eligible for subsidy in another programme at Masters or lower level. The Full Fees rates determined by MOE in Academic Year 2015/2016 are shown in Table 1. The Full Fees rates payable for subsequent Academic Years are subject to revision. For the latest updates, please visit the website : TABLE 1 FULL TUITION FEES Payable (Per Semester) - regardless of nationality for M.Sc. programmes: Full-time (S$) 19,275 Part-time (S$) 9,650 With effect from Semester 2, 2008/2009, students (all nationality) who are re-admitted within NUS, or across NUS, NTU and SMU, will be eligible for the subsidy up to the maximum course duration for the new course less the number of semesters of government subsidy/scholarship received for the previous incomplete course(s). Category 2 Subsidised Tuition Fees Students who had never enjoyed/received any Ministry of Education (MOE)/government subsidy in any graduate programme of study at Masters or PhD level in Singapore can enjoy subsidized tuition fees as shown in Table 2. However, the student can only apply for service obligation with MOE on condition that they have not previously enjoyed MOE s government subsidy in another graduate programme (see Note 4 below). International students who have signed the Service Obligation Agreement are required to work in Singapore-based companies for 3 years upon graduation. Singapore-based companies refer to local and international companies that have a base in Singapore that is registered with the Accounting & Corporate Regulatory Authority (ACRA) as well as companies of such local and international companies registered with ACRA that are based overseas. All fees shown here are in Singapore dollars and, except for tuition fees, include the prevailing GST. The GST for tuition fees is absorbed by the Ministry of Education (MOE). 2. Unless otherwise indicated, all fees are payable on a semester basis. 3. Fees published in this document are indicative only. Fees for subsequent academic years are subject to review by the Ministry of Education. 4. For more information and the latest updates on fees and service obligation, please visit the website Deferment International Applicants Once an offer of admission is made to a successful applicant, no deferment of registration will be granted to candidates who are currently residing in Singapore. Successful applicants who are unable to register will have to reapply for admission. Candidates who are pursuing full-time study and who are presently not residing in Singapore may, subject to approval, be granted deferment for at most one semester. All requests are considered on a caseby-case basis. Overseas applicants who are not residing in Singapore or not working in Singapore are only eligible to apply for admission to pursue graduate programmes on a full-time basis. For candidates who are not applying for any scholarship, do accompany your applications with any one of the following as proof of financial support : documentary evidence of financial support in the form of a bank statement. The bank statement should indicate a total of at least S$18,000 ; or a letter of confirmation from a sponsor and documentary evidence of financial support from the sponsor Page 10 of 15

11 Please refer to the section on 'Guide for International applicants' for details on immigration matters, accommodation and estimated cost of living. Financial Assistance (1) Tuition Fee Loan Financial assistance is available in the form of the (1) Tuition Fee Loan, the (2) NUS GSA scholarship for relevant full-time graduate candidates who are nationals of ASEAN member countries, except Singapore. Registered full-time students who are pursuing government-subsidised graduate programmes may apply for a loan under the Tuition Fee Loan Scheme. Details and application forms for the loans may be obtained from the Student Service Centre (Cashier s Office), Yusof Ishak House, Level 1, No. 31 Lower Kent Ridge Road, NUS. Details of the Tuition Fee Loan Scheme are also available from the website (2) NUS Graduate Scholarship for ASEAN Nationals (NUS GSA) The National University of Singapore (NUS) offers NUS GSA scholarships to engineering students each year. These awards are financed by the People of Singapore and the University. The NUS GSA will provide each scholar with: (i) monthly stipend of S$1,350 throughout the period of the award (ii) One-time book allowance of S$500; (iii) Visa and student Pass fee (iv) Medical Examination fees for Student s Pass application (capped at S$60); (v) Tuition and miscellaneous Student Fees at NUS, throughout the period of award; (vi) Cost of economy travel directly from the home country to Singapore upon award of the scholarship (capped at S$500 and 20kg baggage per trip); and (vii) Cost of economy travel directly from Singapore back to the home country upon successful completion of the course of study leading to the award of the degree (capped at S$500 and 20kg baggage per trip). The tenure of the award is for an academic year (2 semesters). The scholar has no obligation or bond to be employed by NUS or in Singapore but is expected to complete the designated M.Sc. degree and return to the home country. The NUS Graduate Scholarship for ASEAN Nationals (NUS GSA) is only applicable for August intakes. Candidates who apply for January intakes will not be considered for this scholarship. Hence, please apply for the M.Sc. programmes for the August intakes instead. Eligibility for the NUS GSA (Scholarship) 1. Nationality The candidate must be a national from any of the 10 ASEAN member countries. ASEAN is abbreviation for Association of South East Asian Nations and it comprises Brunei, Cambodia, Indonesia, Laos, Malaysia, Myanmar, Philippines, Singapore, Thailand and Vietnam. However, Singapore citizens and permanent residents are ineligible. 2. Excellent Academic Record The candidate must have an excellent academic record (top 10% of the Page 11 of 15

12 class) 3. Successful admission The candidate must be admitted or will be admitted to pursue a NUS GSA designated M.Sc. degree (by coursework) by full-time study. For the NUS Faculty of Engineering, all M.Sc. programmes are considered as NUS GSA designated programmes. Successful scholarship applicants pursuing the M.Sc. in Intellectual Property Management will receive sponsorship only for Part 2 of the programme. Part 1 will continued to be self-financing. 4. TOEFL or IELTS scores The candidate is expected to have a good command of the English language. A candidate's English language proficiency will be determined by: a. the Test of English as a Foreign Language (TOEFL) score of at least 600 (paper-based test) or 250 (computer-based test) or 92 (internet-based test) ; or b. the International English Language Testing System [IELTS] result of at least Relevant Work Experience A minimum of 2 years of relevant working experience as at the application due date (i.e. for this scholarship) Ineligibility A candidate will not be considered for the NUS GSA if the candidate does not fulfil the nationality and all other criteria as specified in the paragraph on Eligibility for the NUS GSA (Scholarship) or if the candidate's application is incomplete or is received after the due date. For further details on the Scholarship, please refer to our website at: Other suitability tests Candidates may, at the discretion of the Executive Committee for GSE, be required to undergo such other tests as may be set to assess their suitability for University study and to appear for interviews. Confirmation / Notification of application An acknowledgement card will be issued to you to acknowledge receipt of your application and to inform you of any outstanding documents you should submit, if any. Processing of applications of International applicants may take a longer period. As such, under special circumstances, International applicants may be requested to defer their applications to the next Intake. All applicants can expect to be informed on the outcome of their application sometime in mid May / Nov. Until then, we regret that we are unable to advise applicants on the status of their applications due to the large numbers of applicants seeking admission. Page 12 of 15

13 Appeal for Reconsideration of Application Unsuccessful applicants must submit their appeals by end November (for January Intake) and end May (for August Intake). Each appeal should be sent via to preferably be in letter format, stating the M.Sc. programme(s) that the applicant would like to appeal for admission to and the reasons for appeal. University Academic Calendar The University's academic calendar for Semester 1, & Semester 2, are as follows: Page 13 of 15

14 Language of Instruction The language of instruction for the graduate courses is English. Course Structure Graduate courses are usually conducted in the evenings from 6.00 pm to 9.00 pm from Mondays to Fridays. Registration Students either register as full-time or part-time candidates. However, International candidates who are not residing in Singapore or not working in Singapore are only eligible to apply for the full-time M.Sc. programmes, and if offered admission, they are to be registered as full- time students only. All registered full-time students are not permitted to be employed without permission from the University. Those found doing so may have their candidatures terminated immediately. No student may concurrently be a candidate for more than one degree or register as a candidate of the University and of another University or Institution. Applicants who are Grad. Dip. (Env Eng / Aviation Mgt / Maritime & Port Mgt) holders and/or former M.Sc. students who withdrew from their course of study and who wish to be considered for exemption of module(s) in their new M.Sc. candidature must apply in writing through their respective Department s Office only after they gain successful admission to their new M.Sc. programme. Pre-taken Graduate Modules Please note that any modules taken to meet the requirements of one degree cannot be used to count towards another degree. NUS B.Eng. or B.Tech. graduates may apply for a transfer of credits of their approved pre-taken* graduate modules read during their undergraduate studies for their current M.Sc. studies in NUS. Modules pre-taken (maximum of 2) should be read less than 5 years prior to admission of their M.Sc. degree and are not counted towards their Bachelor s degree. Upon approval, the pre-taken modules and grades will be transferred and included in the CAP computation of their current M.Sc. programme. All applications for transfer of credits must be submitted to GSE within the 1st month of their new M.Sc. candidature. *Pre-taken graduate modules must be as indicated in the transcripts of academic records for their Bachelor s degree (i.e. B.Eng. or B.Tech.) and not included in the Bachelor s degree CAP computation. Selection of Modules & Class Time-Table Information on available modules for each semester and class time-table will be issued by the DEPARTMENTS to candidates who are successful in gaining admission to the courses. Successful candidates should be notified of this information about two weeks before the commencement of the Semester. Page 14 of 15
